    Compassionate skilled personalized rehab care

My mother received genuinely caring, team-oriented care here - from skilled therapists (Tripp in PT was terrific) to attentive nurses and staff who treated her like family. They tailored a personalized rehab plan, improved her mobility, and helped her prepare to go home. The facility is clean, welcoming, and active, with compassionate staff who know residents by name. I'm very pleased and would recommend this place for skilled rehab and warm, attentive care.

    Summary of reviews

Lillian Carter Health & Rehabilitation elicits a mixed but clear profile. Many reviewers describe a warm, family-like environment with staff who know residents by name and deliver compassionate, personalized attention. The facility is repeatedly praised for its welcoming common areas, engaging activities (including memorable social events), and a focused rehabilitation program; examples include positive physical-therapy outcomes and specific therapists noted for effective care. Admissions and enrollment processes are described as helpful, and several families emphasize the genuine personal touch from nurses, reception staff, and the activities team.

Clinical-care impressions are varied. Positive accounts highlight attentive nursing, individualized care plans, and strong therapy services that aid recovery and mobility. However, other accounts raise significant clinical-safety concerns, most notably inconsistent medication administration and instances that suggest inappropriate dosing or incorrect medication delivery. Additional clinical gaps cited include wound-care and pressure-injury prevention deficiencies and mobility/transfer challenges that contributed to reduced independence for some residents.

Operational and daily-care patterns also show contrast. Supporters report clean, well-maintained rooms, pleasant meals, and active resident engagement. Conversely, some families describe irregular personal-care scheduling (for example, inconsistent bathing), lapses in housekeeping or odor control in particular rooms or storage areas, and problems with linen and property handling. There are instances of missing personal items and difficulties accessing belongings during transitions, indicating weaknesses in personal-property and discharge processes.

Communication and management consistency appear to be a recurring theme. Several accounts praise individual staff members and describe warm, supportive interactions; others describe delays in responsiveness, uncharged phones, and strained family communication around care concerns. Reviewers also express perceptions that operational or efficiency priorities sometimes overshadow individualized clinical attention, and that staff experience and training can be variable across shifts.

Takeaway for prospective families: the facility shows clear strengths in therapeutic rehabilitation, social programming, and genuinely caring staff members who create a home-like atmosphere. At the same time, there are concerning patterns around medication management, wound care, hygiene scheduling, property handling, and communication consistency. Prospective residents and families should tour the facility, meet the care and therapy team, review clinical protocols (medication administration, pressure-injury prevention, and property/discharge procedures), and ask for recent inspection or quality metrics before making decisions.

About Lillian Carter Health & Rehabilitation

Lillian Carter Health & Rehabilitation is a for-profit nursing home and rehab center with room for about 100 residents, though, on average, around 50 people live there each day. The center has a management team that's been in place since May 2023, including William C Davis III, William Davis, David Orne, David Parker, Ashton Windham, and Spencer Windham. You'll find a broad range of services, like skilled nursing care around the clock, physical and occupational therapy, speech-language therapy, memory care, restorative and palliative care, hospice services, pharmacy support, and even telemedicine and transition care programs. The facility's also Medicare and Medicaid certified, and offers secured memory care for those who need extra support. There are specialized rehabilitation programs and restorative programs available.

The nurse staff turnover rate is about 51.8%, and each resident gets, on average, 3.70 nurse hours per day. There's a business office open weekdays from 8:00 a.m. to 5:00 p.m. Visits can be arranged, and there's a virtual tour option for families who want to see what the place looks like before coming in. Lillian Carter Health & Rehabilitation does focus on creating a home-like feeling and supporting seniors with social care services, and it's part of a broader social care platform that links residents to more resources.

The center has had some issues noted in recent inspections, including problems with resident assessment and care planning, such as not fully screening for mental disorders or intellectual disabilities and trouble providing and following a full infection control program. Even with these problems, the staff tries to offer consistent and compassionate support with a focus on building a supportive community, and they do their best to answer questions and help families make care decisions. There's a contact form for those who want to schedule a visit or ask about the different programs.
