    Compassionate staff restored my life

I'm very pleased with the compassionate, attentive staff here - Diane, Mike Lilly and Michelle deserve special thanks - whose teamwork and dedication helped me through rehab after mini-strokes. The place feels like family: clean, recently refreshed rooms, excellent activities and a top-notch TBI/head-injury unit; I'm enjoying life again and would recommend it.

    Summary of reviews

Reviews of Integrity Healthcare of Carbondale present a notably polarized picture: while many commenters praise the compassion, teamwork, and rehabilitative successes of the caregiving staff, others raise serious operational and clinical concerns. Families who had positive experiences highlight attentive nurses, engaging activities, successful short-term rehab outcomes (including TBI/head-injury care), and recent facility refreshes that create a pleasant, non-institutional environment. These accounts emphasize a family-like atmosphere, respectful treatment, and staff members singled out for extra care and communication.

Conversely, a substantial set of concerns centers on clinical processes and responsiveness. Multiple accounts describe delayed or inconsistent medication administration, including late decisions about antibiotics and gaps in diagnostic follow-up. Reviewers also describe situations that led to escalation of infections and end-of-life concerns; these narratives point to weaknesses in monitoring, timeliness of clinical intervention, and family communication around critical events. Access to medical records and timely disclosure of clinical information is characterized as limited by some families, which compounds frustration when clinical decisions are disputed.

Operational issues are another recurring theme. Staffing levels and shift variability are presented as contributing to slow response times to call lights and uneven care practices; several comments link these staffing constraints to inconsistent hygiene and sanitation oversight, and to food-handling practices such as meals left out and reheated. While some reviews describe clean, well-maintained interiors and remodeled rooms, others note sanitation and pest-control concerns in particular areas, producing a mixed impression of environmental quality across the facility.

Management and organizational concerns appear in the feedback as well. Reviewers note an opaque ownership or organizational structure with multiple entities, and there are strong allegations regarding theft and financial misconduct as well as references to litigation and unpaid judgments. These issues raise questions for prospective families about transparency, oversight, and financial/administrative governance. At the same time, several staff members are credited by name for positive interactions, and ongoing renovations and community engagement were cited as favorable signs.

For prospective residents and families, the pattern suggests performing focused due diligence: tour multiple units at different times of day, inquire about staffing ratios and shift continuity, request examples of infection-control protocols and recent inspection results, confirm processes for medication administration and access to medical records, and ask for clarity about ownership and financial practices. The facility appears capable of delivering strong, compassionate care in many casesโ€”particularly for rehabilitation and activity programmingโ€”but the variability described across shifts and units, combined with the serious administrative allegations, justifies careful verification before making placement decisions.

    About Integrity Healthcare of Carbondale

Integrity Healthcare of Carbondale sits in a quiet residential neighborhood, making it easy for families and visitors to find, and folks can come and go as needed since the doors are open every day, all hours, which really helps people keep in touch and feel at home. The place has 131 beds and runs as a for-profit corporation, managed by Integrity Healthcare Communities LLC, and it holds a proper Skilled Nursing Facility license, so it's checked and inspected regularly, earning a strong A-minus for inspections with no severe issues noted, and its nursing staff has a B-plus grade, which means the care is above average. Reviews land around 3.2 out of 5 with 31 people weighing in, and historically the overall grade sits at a B-minus, so most would say the place keeps a respectable standard over time.

The staff here really focus on keeping the care personal, saying that quality means doing things right even when nobody's watching, and the feeling around the halls often comes off as a kind of extended family, especially since the staff treat the residents as the most important folks in the building. They take care of all sorts of health needs, with skilled nursing services going all day and night, plus there's help for memory issues like Alzheimer's and other dementia types, along with behavioral health support for those who need it. If a person needs help recovering from a stroke, wound care, IV antibiotics, or any kind of rehab-physical, occupational, or speech therapy-they'll get it, and they'll also find counseling on nutrition and other daily needs since the approach here leans towards looking after the whole person, not just the medical needs.

Integrity Healthcare of Carbondale welcomes people on Medicare and Medicaid, and also helps folks looking into long-term care insurance or searching for caregiving resources, and once inside, every resident gets a care plan tailored for them, plus help with medications, end-of-life and palliative support, and memory care if needed. There's a steady calendar of social activities, with outings, recreational programs, in-house church services, and even entertainment, so boredom doesn't really last long. People who want something more independent or need less help can choose different care settings, from full nursing home stays to assisted or independent living arrangements. There aren't formal resident or family councils, and while amenities aren't specifically listed, ongoing renovations are putting in new furnishings, lighting, and other touches to brighten the place up. The goal is to keep things safe and compliant, to help residents feel at peace, and to foster a setting where everyone feels like a valued part of the community, no matter where they come from or what they need.
