    Clean professional staff, fast recovery

I placed my loved one here for rehab and was impressed - the facility is spotless, bright, single-story with wide corridors and large windows, and the rooms and common areas feel homey. The nursing, CNAs and therapists (Casey, Megan and others) were professional, kind and instrumental in a fast recovery; admissions and social services (Margaret, Shannan) and the welcoming administrator were knowledgeable and responsive. Activities, good dining options and attentive staff made us feel like family - I'd definitely recommend this place.

    Summary of reviews

The Pearl of Elk Grove elicits strongly mixed experiences. Across reviews there is a clear pattern of strong rehabilitative services and many staff members who are described as compassionate, skilled, and helpful. Physical and occupational therapy receive consistent praise for effectiveness and individualized programming. Admissions, social-work coordination, and certain nursing staff members are frequently singled out for proactive communication and assistance. The facility's common areas, outdoor spaces, and many patient rooms are described as bright, clean, and well-maintained, supporting a comfortable short-term rehab environment.

At the same time, operational weaknesses are repeatedly identified. Staffing consistency is a principal concern: many accounts point to understaffing, high turnover, and reliance on agency personnel that can disrupt continuity of care. Call-light response times — particularly overnight — and delays in attending to routine personal-care needs are recurring themes. Related patterns include inconsistent bathing and incontinence-care processes, gaps in medication and treatment follow-through, and occasional maintenance or equipment failures that affect daily comfort and safety.

Dining and activities present a mixed picture. The facility offers restaurant-style service and has professional dietary staff; several reviews describe appealing meals and social dining opportunities. However, other accounts describe late or cold meal delivery, inconsistent menu quality, and scheduling problems. Activities and therapy-based programming are a relative strength: reviewers frequently note engaging group and one-on-one options and a rehab-focused schedule that supports recovery goals.

Management and communication show variability by case and by shift. Multiple families praise specific administrators, therapists, and social-service staff for responsiveness and advocacy, but others describe inadequate family communication, pressure around discharge planning, billing or scheduling errors, and inconsistent handling of clinical concerns. A small number of reviews include allegations of serious clinical lapses, including a severe pressure injury requiring surgical intervention; those accounts raise questions about wound-prevention protocols and clinical oversight and warrant follow-up with facility leadership and regulators if substantiated.

Facility condition and logistics are generally positive in many areas — clean common spaces, ample natural light, usable outdoor areas, and a rehabilitation-oriented layout — but reviewers also describe occasional sanitation or odor concerns, non-functioning equipment (TVs, ice machines), and limited storage or cramped layouts in some rooms. Prospective residents and families considering this facility should weigh its demonstrated strengths in rehab and the many compassionate staff members against the documented operational inconsistencies. Recommended pre-admission questions include current staffing ratios by shift, night-shift response metrics, wound-prevention and incontinence-care protocols, meal-service timing and temperature controls, use of agency staff, and the facility's process for family communication and discharge planning.

About The Pearl of Elk Grove

The Pearl of Elk Grove is a skilled nursing and senior care facility in Elk Grove Village, Illinois, serving about 149 residents with 190 certified beds, and it accepts both Medicare and Medicaid. The facility is owned by Elk Grove Holding Company, LLC, and has indirect owners through several trusts and companies, but there's no public information on who directly manages or oversees daily operations or the employees. There's a memory care community inside the building, and the facility covers different care options including long-term skilled nursing, short-term post-hospital rehabilitation, respite care, independent and assisted living, and specialized rehab like orthopedic, pulmonary, cardiac, and wound care, so there are services for people with specific disabilities or diseases as well as seniors aged 55 and older.

The building itself is fairly new, built within the last year, and offers private and semi-private rooms, though many rooms are small and cramped, with some TVs not working and closet designs that only let one resident at a time use them, plus some run-down furnishings and limited closet space. There's no specific area for visitors to sit and wait during privacy-required procedures, which some families should know. The Pearl of Elk Grove gives access to kitchens or kitchenettes, cable TV, telephones, air conditioning, WiFi, washers, dryers, and all rooms are furnished. There's always a 24-hour call system and 12-16 hour nurse supervision. The nurse turnover rate is very high at 99.2%, much higher than the state average, and there's a recorded pattern of deficiencies and care violations-including failure to meet residents' preferences and needs, and problems with pharmacy services-so it's currently labeled as a Special Focus Facility Candidate due to serious quality concerns.

Despite these challenges, the staff offers personal care with bathing, dressing, toileting, mobility help, wheelchair assistance, medication management, occupational therapy, wound care, podiatry, physical therapy, and skilled nursing care. Dining services include a professional chef, special meals for diets like diabetes or allergies, and all-day availability in a restaurant-style space. The community helps keep residents active and social with activities like music, arts and crafts, fitness and wellness programs, educational events, resident-led groups, movie nights, walking paths, a garden, a library, and business and activity rooms. There's also transportation, guest parking, outdoor programs, family and resident councils, safety and handicap features, laundry, dry cleaning, property maintenance, a salon/barbershop, a spa/sauna, and security features including sprinkler systems. Housekeeping, concierge, and family support services are available, but you'll find the closets small and the rooms a tight fit, which matters for some.

Prospective residents and families can tour the facility to see if it feels right, and the Pearl focuses on comfort and well-being, with community-sponsored events, and a patient-centered approach to rehab and skilled care across several other Chicagoland locations. There's an ongoing commitment to improvements, evident by organized family and resident councils and a focus on reporting concerns, but the history of care violations remains documented in public inspection and complaint reports, tallying up to 75 deficiencies including those related to infection prevention. The facility promotes community comfort with emergency alert systems, move-in coordination, scheduled daily activities, a dining room, outdoor spaces, and memory care support. Overall, the Pearl of Elk Grove provides a range of care services and amenities while currently working through regulatory oversight and past care issues that families may want to consider.
