    Compassionate staff, excellent rehab, welcoming

I had a very positive experience at Bria of Mascoutah - the nursing, CNAs and therapy team (especially Stephanie T. and Amy S.) were kind, attentive and treated residents like family. The facility is clean, welcoming, with good food, meaningful activities, strong rehab and compassionate end-of-life care; staff consistently went above and beyond and communicated well. I recommend it overall - a few small improvements would make it outstanding.

    Summary of reviews

The reviews for BRIA of Mascoutah describe a facility with sharply divergent experiences: many families and visitors praise the direct-care staff and therapy teams, while others report significant operational shortcomings that affected resident wellbeing. Care-related praise centers on compassionate, attentive nurses and CNAs, therapists who run a capable rehab program, and staff who provide individualized, one-on-one attention in a smaller, family-style setting. Several accounts highlight proactive communication from specific staff members during transitions, helpful admissions assistance, meaningful activities and holiday events, and supportive end-of-life care in particular cases.

At the same time, recurring concerns point to systemic issues that are not limited to isolated incidents. Multiple reviews indicate sanitation and odor control problems in some areas, along with inconsistencies in personal-care and wound-management processes. There are reports of infection-related outcomes and concerns about clinical oversight; reviewers described medication administration gaps, missed or delayed treatments, and canceled therapy appointments, which together suggest opportunities to strengthen clinical protocols and scheduling reliability. Some families also noted falls and inadequate assistance with eating or hydration, which align with the broader pattern of variable clinical attention.

Staffing and operational consistency emerge as central themes. While many staff members are praised as caring and hardworking, reviewers describe variability across shifts and personnel โ€” including perceptions of undertraining or inexperienced workers, long shift-change delays, and slow call-button response times. These factors appear to contribute to uneven day-to-day care quality. Environmental and safety issues were also raised: narrow, cluttered corridors, unsecured equipment or wiring, and cramped shared rooms were cited as potential hazards that could impede safe mobility and response.

Dining and activities are generally viewed positively when available: several reviewers enjoyed the food and appreciated planned social events, small celebrations, and active therapy calendars. The facilityโ€™s smaller scale supports a family-style atmosphere for many residents, fostering close staffโ€“resident relationships in multiple accounts.

Management and communication present mixed impressions. Some families experienced proactive updates and helpful transitions from named staff members, while others described poor communication about emergencies, insurance lapses, and the absence of designated emergency contacts. A subset of reviewers raised serious concerns โ€” including allegations about clinical misconduct and discriminatory behavior โ€” which indicate areas where leadership should ensure transparent investigation and remedial action.

In summary, BRIA of Mascoutah appears to offer strong, personalized care and an engaging rehab and activities program for many residents, supported by several highly regarded staff members. However, persistent operational weaknesses โ€” sanitation control, wound and infection management, medication and therapy scheduling reliability, staffing consistency, and environmental safety โ€” are reported often enough to warrant attention. Prospective residents and families should weigh the facilityโ€™s therapy and small-facility strengths against these variability concerns, ask specific questions about clinical oversight, staffing patterns, infection-control protocols, emergency communication procedures, and room/environment safety during tours, and request references from recent families when possible.

    About BRIA of Mascoutah

BRIA of Mascoutah sits on 901 N 10th St in Mascoutah, Illinois and acts as both a nursing home and senior living apartment community, and folks who stay here get skilled nursing care with 24-hour support including therapy like physical, occupational, and speech if needed, and sometimes people go there just to recover between hospital and going back to their homes, so lots of services tie in with things like home care, hospice, and referrals too, but one big thing to know is there have been inspection reports mentioning some problems about resident rights and safety, and the nurse turnover rate runs pretty high at about 78.9%, with nurses giving 3.99 hours of care per resident each day, and out of the 55 certified beds, there are usually around 50 residents living there each day. BRIA of Mascoutah connects to Bria Health Services as an affiliate, and its ownership is split between Avrum Weinfeld, Daniel Weiss, Natan Weiss, and Efriam Weinfeld. The place works as a for-profit partnership and has a Medicare page that anyone can view for more details. The property offers different kinds of rentals, including apartments, townhomes, condos, and even single-family homes with features like washers and dryers, air conditioning, fitness centers, pools, patios, and even pet-friendly policies, plus you'll find apartments with things like waterfront views, barbecue areas, and doormen, though there aren't a lot of details about specific outdoor spaces or parking, but it is pet friendly and includes some listings for luxury or multiple-bedroom housing. BRIA of Mascoutah has social media and messaging features to help residents connect with others and share content online, and it also allows for account management and online interaction, which makes it a bit unique. The facility follows local laws that protect LGBTQ individuals in housing and other areas. The community gives access to major medical centers in St. Louis and holds social activities for residents, hoping to keep things safe and supportive, though there's not much detail about exact floors, entrances, or parking situations. Staff offer a mix of care, including adult care home, assisted living, independent living, memory care, and skilled nursing, though it's not always clear which services are day-to-day. Some amenities and services are outlined, but some details are missing or aren't well-explained, and so it's important for anyone interested to look up inspection reports and reach out directly if they want more specific information.
