    Warm family-like care with professionalism

I chose this community for my mother and I'm very pleased-the caring, home-like staff (Mary, Jacqueline, Jennifer and the whole team) are warm, professional and genuinely resident-focused, creating joyful moments like hair-styling entertainment and an 80s country concert. Leadership is compassionate and hands-on, the facility is clean and welcoming, and I confidently recommend this family-like place.

    Summary of reviews

Reviews for Mar-Ka Nursing & Rehabilitation present a mixed picture. Many families and staff describe genuinely caring, relationship-based caregiving and a small, home-like atmosphere where residents receive attentive, personal attention from CNAs, therapists, and certain administrators. Positive accounts specifically cite engaging activities (hair styling events, concerts), a responsive therapy team, maintenance staff who address issues, and moments of staff appreciation and morale that contribute to a pleasant environment for residents.

At the same time, there are persistent operational concerns. Several accounts describe inconsistent staffing levels that contribute to delays in responding to nurse call lights and longer waits for assistance. These responsiveness problems are tied to broader care-quality worries, including examples of falls and injuries noted by families and frustrations with timeliness of incontinence and personal care. Reviewers also raise sanitation and pest-control concerns and cite odor issues in some common areas, indicating uneven environmental standards across the building.

Facility condition and logistics are recurring themes. The physical plant is described as dated, with deferred maintenance, broken items, water leaks, and occasional safety hazards affecting resident comfort and safety. Dining feedback is mixed: some residents and families praise the food quality and friendly dining staff, while others identify inconsistent meal timing and delays between residents being seated and receiving meals. Personal-item management is another operational gap โ€” lost clothing and devices and disputes over replacement or reimbursement were mentioned.

Feedback about management and staff conduct is polarized. Several reviewers praise specific leaders (including an engaged director of nursing and other administrators) for clear processes and compassionate oversight. Conversely, other reviews describe unprofessional communication, concerns about the tone of staff interactions, and instances where families felt ignored by phone or during follow-up. A number of families reported filing complaints with regulators; there were also allegations regarding internal rating practices. Rehabilitation services receive both positive and negative evaluations, suggesting variability in therapy quality or expectations.

For prospective residents and families, the pattern suggests that individual unit conditions and specific staff leadership substantially affect experience. Recommended topics to address during a visit or inquiry include: current staffing ratios and typical response times to call lights, recent sanitation and pest-control measures, recent maintenance or capital-improvement plans for the building, meal-service schedules and menu samples, rehabilitation program structure and typical outcomes, procedures for tracking and replacing personal items, and recent state inspection results or corrective actions. These focused questions can help clarify whether the facilityโ€™s strengths align with a prospective residentโ€™s needs and whether operational concerns have been addressed.

    About Mar-Ka Nursing & Rehabilitation

Mar-Ka Nursing & Rehabilitation sits at 201 S 10th St in Mascoutah, Illinois, right in the 62258 area, and the building's been there since 1969, so it's got some history there and it's part of a Continuing Care Retirement Community. This place works with local schools like Mascoutah Elementary, Middle, and High Schools, and you'll find a peaceful setting with outdoor spaces, gardens, and walking paths, and a walk score of 52, which means you can get to a few places on foot but might need a ride for others, and they do offer transportation and parking for residents and visitors.

The facility has 76 beds, and as of June 2025, 37 are open, and they're classified as a Skilled Nursing Facility for people who need 24-hour nursing and personal care or those recovering after a hospital stay, illness, or surgery, with physical, occupational, and speech therapy, and there's also specialized help for memory care, dementia, or Alzheimer's. Residents who need help with bathing, dressing, transfers, medication, or have trouble getting around can expect 24-hour supervision, non-ambulatory care, and assistance with all daily tasks, and there's a 12-16 hour nursing care provision each day.

There's a focus on comfort, so residents get private, furnished rooms with kitchenettes, cable TV, Wi-Fi, air conditioning, a private bathroom, and a 24-hour call system, and the building also has cozy community spaces, a library, game and activity rooms, a spa/wellness area, fitness room, arts room, movie theater, and community dining room. A professional chef prepares meals including special diets like diabetic or allergy-sensitive options, and dining happens all day, not just at set times, and they handle housekeeping, laundry, and move-in coordination, so things stay simple for everyone.

Programs go beyond just care-Mar-Ka sets up movie nights, music, arts, and outdoor activities, with resident and family councils for raising concerns or sharing ideas about care, and community involvement is encouraged with access to local parks and trails. The grounds outside are well-kept, and the environment stays calm and supportive so residents can feel comfortable and independent, and activities also support social connections, wellness, and recreation.

Mar-Ka takes Medicare and Medicaid, and works as a for-profit corporation, offering a mix of intermediate and skilled care, medication help, and assistance with daily life, and there's a reputation for compassionate support, though there are no published reviews so far. The facility makes room for both short-term rehab and longer-term residential care, with a strong focus on safety and security for everyone, especially for those with complex health needs.
