    Compassionate staff, clean facility, recommended

I moved my loved one here and have been very pleased. The staff - nurses, CNAs, therapists and admin - are compassionate, professional and attentive; the building is clean, meals are well-prepared, and the therapy and activities really helped his recovery. I feel reassured by the caring community and would recommend it, with only minor room for improvement.

    Summary of reviews

The reviews present a mixed picture of care at this facility, with clear strengths in rehabilitation, activities, and many front-line caregivers alongside recurring operational challenges. Therapy services—particularly physical and occupational therapy—are frequently cited as a high point, with families and residents noting measurable progress and well-structured rehabilitation plans. Activity programming, including music sessions and scheduled events, is often described as engaging and supportive of resident well-being. Many reviewers also praise specific staff members, CNAs, and administrative team members for compassionate, attentive interactions that contribute to a warm community atmosphere.

At the same time, a number of consistent operational weaknesses emerge. Staffing levels and training appear uneven, producing variable experiences with nursing care, timeliness of responses, and professionalism. Medication administration and refill processes are a recurring concern, including delays and difficulties coordinating refills with outpatient providers. Communication breakdowns with families and resident physicians are reported several times; these include difficulty reaching staff by phone, lack of timely discharge paperwork transfer, and unclear updates during care transitions.

Facility maintenance and cleanliness are described inconsistently: some areas and common spaces are noted as clean and pleasant, while other comments highlight room-level maintenance and sanitation concerns and missed housekeeping tasks. Related safety and security issues include allegations of missing personal items and weak property-controls, as well as reports of resident-to-resident disturbances and staff not always addressing behavioral incidents promptly. Food service receives mixed feedback—some reviewers compliment meal quality and dietary accommodations, whereas others describe inconsistent meal temperature, limited menu accuracy, and problems adhering to special-diet needs.

Management and administrative performance is similarly mixed. Several reviewers commend supportive social-work and administrative staff and an involved Director of Nursing, but others describe unresponsive front-desk service, billing and Medicaid-authorization difficulties, and abrupt or poorly communicated discharges. There are also isolated but notable operational concerns: an infection-control event was mentioned, and some families raised questions about transportation safety. Prospective residents and families should weigh the facility’s strengths in rehabilitation and community programming against these operational variability issues. When considering this facility, ask specific questions about current staffing levels and training, medication and refill procedures, property-security measures, meal accommodations, discharge and communication protocols, and recent infection-control practices to get a current snapshot of performance.

About University Park Rehab & Healthcare

University Park Rehabilitation and Healthcare Center sits in Fort Wayne, Indiana, and has 104 certified beds with both private and semi-private rooms, all Medicare and Medicaid certified, and people know it by other names like University Park Nursing Center or Covenant Care Indiana, Inc. The center's owned by Henry County Memorial Hospital, with management since November 2020 by University Park Rehabilitation and Healthcare Center, LLC, and general manager Mr. David Desfenbaugh runs the place. This spot has had 34 documented deficiency reports, mostly about resident rights, care planning, and quality of life, from recent inspections, and it's drawn complaints that pointed to similar concerns. The nurse turnover rate is 72.5%, which is higher than Indiana's average of 48.5%, and the staffing level is a bit low at 2.94 nurse hours per resident per day, while elsewhere in the state it's 3.7 on average, which some families might want to think about. The staff includes admissions coordinators, social workers, licensed nurses, certified nursing aides, and business office managers. The facility belongs to Castle Healthcare and has a hands-on administration team that's known to be courteous, and they're committed to keeping the place clean and treating guests with genuine hometown care. It's not a BBB Accredited Business.

The building offers 24-hour nursing and custodial care that covers help with bathing, grooming, getting dressed, and eating, and meals and snacks come from menus approved by a registered medical nutritionist and prepared by a certified dietary manager. University Park provides both long-term care and full-service rehabilitation services, including specialized programs for stroke recovery, cardiac rehabilitation, orthopedic rehabilitation, and respite care, and there's also a discharge concierge to help people when they go home. The community has a TV lounge and a therapy room, and organized activities like baking, bingo, crafts, religious services, entertainment groups, and visits from school groups. There's a resource center for people wanting to know more, and a virtual tour's available on their website, universityparkhc.com. The facility tries to create a warm environment with amenities that help meet different care needs, and people say the staff aims to deliver skilled nursing care to support residents in living their best life, even though there have been recent reports showing where they could do better.
