    Excellent rehab care, friendly staff

I stayed at Golden Living Centers for rehab and had a very good experience - outstanding OT/PT team, amazing nurses and CNAs, and friendly, responsive staff who made it feel like home. The facility is clean and spacious with private rooms, lots of activities, and supportive administration; my therapy helped me regain independence, so I'd recommend it.

    Summary of reviews

Overall impression: Reviews present a mixed picture. Many families and residents describe strong rehabilitation outcomes, attentive therapists, private updated rooms, and a welcoming, home-like atmosphere. At the same time, other reviewers raise serious operational concerns that suggest variability in day-to-day care and supervision.

Care quality and clinical responsiveness: The facility appears to deliver effective rehabilitative care for a large number of residents; occupational and physical therapy teams receive repeated praise for helping residents regain independence. Conversely, there are descriptions of significant clinical lapses in some cases — including delays in medication administration and delayed escalation of medical events — that point to gaps in clinical response protocols and oversight. A small set of reports reference serious post-incident concerns and involvement of external authorities; these are individual but noteworthy and underline the importance of probing the facility's incident-response and escalation procedures.

Staffing, conduct, and culture: Staffing is described as uneven. Many accounts highlight compassionate, resident-focused caregivers, long-tenured employees, and staff members who go above and beyond. Other accounts describe unprofessional or rude interactions and safety-related conduct concerns. This mix suggests variability in staff training, supervision, and continuity; families should ask about staff-to-resident ratios, turnover, and behavioral expectations when evaluating the facility.

Dining and daily living services: Several reviewers appreciate home-style meals (especially within memory care), multiple dining rooms, and regular housekeeping. However, others describe inconsistent meal quality and instances where families relied on external food sources. Questions about meal planning, special-diet accommodation, and consistency of food-service staffing would be appropriate for prospective residents.

Activities and social environment: The facility offers a broad activity program — bingo, karaoke, movies, church services, prom/family events, and outdoor seating areas — and many residents report being engaged and respected. Those seeking an active social calendar will likely find appropriate options, and administrators appear to support family-oriented events.

Facilities and cleanliness: Physical updates and private rooms are frequently noted positively (newly updated rooms, Hall of Heroes, spacious dining areas, decks). At the same time, there are multiple mentions of sanitation issues, pest-control concerns, and localized odor problems. These comments suggest that while much of the facility is well maintained, maintenance and environmental controls may be inconsistent across units or shifts.

Management and communication: Feedback on management is mixed. Some reviewers praise responsive administration and helpful admissions staff; others report difficulty getting satisfactory responses to complaints and inconsistent reception experiences. This suggests variable follow-through on grievances and a need for clearer family-communication processes.

Notable patterns and takeaways: The strongest, consistent positives relate to rehabilitation services, individualized caregiving by specific staff members, and facility amenities. The most significant concerns are operational: variability in staff professionalism and continuity, gaps in clinical response and medication handling, inconsistencies in personal-care practices, and intermittent environmental/sanitation issues. Prospective residents and families should conduct an in-person visit, request information on staffing ratios and turnover, review medication and incident-escalation protocols, and ask about pest-control and meal-service practices to assess how well the facility’s strengths align with their needs.

About Brickyard Healthcare - Brookview Care Center

Brickyard Healthcare - Brookview Care Center sits less than five minutes from Methodist Hospital in Indianapolis, IN, where folks get skilled nursing care and a range of services in a facility with 136 certified beds, though on most days about 76 residents live there. Brookview Care Center's owned by Hendricks County Hospital and managed by Brookview Operating LLC since 2012, and the place runs under the Brickyard Healthcare name. The building's had recent renovations which folks say made the layout easier to get around and the common areas feel more comfortable, plus there are outdoor areas where residents can relax or visit with family. There's a Memory Care community for people with Alzheimer's or dementia that includes trained staff, a secure setting, and an outdoor area with safety alerts for wandering; folks can get private suites and amenities like a barber and beauty shop, daily activities, church services, arts and crafts, Wi-Fi, cable TV, family lounges, phone service, laundry, and snacks. Staff help with personal care and offer 24/7 nursing, physical, occupational, and speech therapy, outpatient therapy, short-term rehabilitation, post-acute rehab, and long-term care. Brookview also provides specialized dementia care, dialysis, stroke care, diabetic care, pain management, cognitive training, pulmonary care, VA contract services, hospice, and palliative care. Therapy includes recovery care to help folks regain strength after a hospital stay, and the nurse turnover rate-at 31.7%-sits lower than the state average. The nurse staffing ratio stands at 3.65 nurse hours per resident per day, which is just a bit lower than the Indiana state average. The facility's led by Executive Director Patti Aldridge, who's spent years in long-term care.

Inspection reports show Brookview Care Center's had 36 deficiencies, including some with infection control and notifications about transfers, though so far none caused actual harm but did bring potential for more than minimal harm, and there were also resident rights concerns related to dignity and self-determination. Daily living and recreation get attention here, with activities and support meant to fit each resident's needs, but some inspection reports do note complaints and infection controls that need ongoing monitoring. The staff focuses on being compassionate in their work, aiming for recovery and well-being, and the facility's always working to improve based on inspection findings. People who stay at Brookview get options for skilled nursing, memory care, outpatient therapy, and VA contract services, while also having access to both social and practical amenities like family rooms, linens, housekeeping, and transportation. The building's managed by experienced healthcare professionals who try to maintain a welcoming atmosphere and provide care that's both tailored and practical, keeping things straightforward while covering a wide range of needs for seniors and people with serious health conditions.
