    Caring attentive staff, excellent rehab

I placed my mom at Rosewalk Indianapolis and was very pleased - the staff were caring, attentive, and friendly, the therapists were excellent, and the facility felt clean and homey. Communication and compassionate care gave us real peace of mind; I would recommend it for quality rehab and recovery.

    Summary of reviews

Reviews of Rosewalk present a highly mixed picture, with clear strengths in rehabilitation services and notable, recurring operational weaknesses. Many families and patients praised the therapy teams — physical, occupational, and speech therapists — for effective clinical skill, individualized plans, and outcomes that supported recovery and safe discharge. Several reviews describe staff who created a family-like atmosphere, engaged residents in activities, and provided compassionate bedside care. The facility’s physical plant is frequently described as modern and clean in common areas, and its location and reception staff are often cited as conveniences for families.

At the same time, a substantial portion of reviews document variability in day-to-day care quality tied to staffing and operational systems. Chronic understaffing, particularly on night shifts, and slow responses to call lights and care requests are recurring themes. Reviewers also highlighted process gaps around medication administration and pain-management, as well as inconsistent catheter and incontinence-care practices. These issues, when they occur, were associated with delayed symptom control and avoidable distress for residents and families. Transportation coordination and assistance with external appointments emerged as an operational weak point, with several accounts of missed or poorly executed transfers.

Facility-level management and administrative processes are another area of concern. Multiple reviews point to communication breakdowns between staff and families, delays or errors in billing and paperwork (including proxy/POA documentation), and restrictive or unevenly applied visitation policies. Some reviewers described high staff turnover and a culture they perceived as focused on financial metrics rather than clinical continuity. There are also repeated sanitation and pest-control concerns reported for certain units or rooms, which contrast with other reviewers’ statements about clean, odor-free spaces. A few reviews referenced regulatory attention and serious allegations, including theft and other financial concerns; these items suggest the value of checking for current state inspection reports and any outstanding complaints.

Dining and activities receive mixed comments: some residents found meal service accommodating and social programs engaging, while others experienced scheduling conflicts with religious observances or perceived inconsistency in meal delivery. Activity programming and social engagement were often described positively when staffing levels supported them. Overall, the facility appears capable of providing high-quality rehabilitation and compassionate care, but the experience seems highly dependent on staffing, shift, and unit. Prospective residents and families should verify current staffing ratios, observe cleanliness and pest-control practices during a tour, review the facility’s medication-safety and escalation procedures, ask about transportation protocols, and request recent state inspection and complaint history to ensure the level of consistency they require.

    About Rosewalk

Rosewalk, found in Indianapolis, Indiana, is a nursing home community connected with ASCCare.com and stays open all day and night, every day of the week, which means there's always help if someone needs it, and they've specially arranged wheelchair services for those in hospice care, which can matter a lot when someone becomes less mobile near the end. The staff there, trained in nursing and senior care, focuses on both short-term things like recovery after surgery or stroke and on long-term needs for people who need daily help and can't really be cared for at home anymore, and they'll also take time to look at each new resident for hospice needs before accepting them, since that's a serious change and it's important to get it right. You'll find they offer many healthcare options from memory care and skilled nursing to rehabilitation, palliative care, and respite stays, and these services come with a strong team approach where doctors, nurses, aides, therapists, and social workers all pitch in to help keep people as comfortable and independent as possible. The building itself is modern and has a warm, family feel, and there's always a push to make people feel respected and cared for, especially when someone is dealing with memory issues, since their memory care programs use safety alerts, secured outdoor areas, and staff trained to handle Alzheimer's and dementia, all while focusing on daily activities that touch on social, physical, emotional, and intellectual wellness to help residents stay active and connected. The rehabilitation programs help with walking again, rebuilding strength, and managing risks like falls or pressure wounds, and there's even outpatient therapy that uses proven strategies so people can work on health and mobility even if they don't stay at Rosewalk. The partnership with hospice teams means when someone needs comfort or spiritual and bereavement help, Rosewalk brings in medical supplies, guidance, and support services so no one feels alone, and family caregivers can use respite care for short breaks or if they need to travel or recover from illness themselves. Their team believes in a personalized plan for every resident, taking a close look at what each person needs, and with social services, mobility programs, and a strong memory care philosophy, Rosewalk tries to cover as many senior care needs as possible while providing a calm, stable, and respectful setting for people in Indianapolis.
