    Caring staff, clean rooms, responsive

I placed my parent here and I'm very pleased with the caring, go-above-and-beyond staff, clean odor-free rooms and apartments with nice views, lively activities, and consistently hot, well-prepared meals. Management and maintenance are responsive, the facility feels warm and family-oriented, and the clinical excellence awards seem well deserved - overall a strong choice with a bit of room to improve.

    Summary of reviews

Overall impression: The reviews portray Greenfield Rehabilitation & Health Care Center as a facility with strong frontline caregiving and community life, but with recurring operational issues at the administrative level. Many families and visitors emphasize caring, personable staff who create a family-like, homey atmosphere and who respond attentively in emergencies. At the same time, several reviews describe managerial and billing issues that merit verification by prospective residents and their families.

Care and clinical quality: Reviewers frequently praised direct-care staff for compassion, responsiveness, and a willingness to go beyond basic duties. Rehabilitation and personal-care services received positive mentions, and the facility has been recognized with clinical awards, which supports the perception of competent clinical programs. However, comments about difficulty retaining experienced nurses and nursing leadership suggest uneven continuity of care; reliance on agency personnel is also cited as a factor that can affect day-to-day consistency.

Staff and communication: Staff–resident interactions are a prominent strength: respectful, welcoming transitions, and proactive family communication are commonly noted. Maintenance and support staff receive positive mentions for being helpful. Conversely, several accounts indicate leadership and administrative challenges — including turnover in key leadership roles — which can undermine staff morale and make staffing patterns less predictable.

Dining and activities: Dining is generally described positively — meals are characterized as hot and enjoyable by many reviewers. The activities department receives strong praise for offering engaging programming, frequent highlights, and a lively social environment. These elements contribute to the facility’s reputation for an active, community-oriented daily life.

Facilities and environment: The facility is often described as clean, peaceful, and home-like, with spacious rooms and pleasant views in some units. Reviewers highlight odor-free common areas and attentive facility maintenance. There are, however, intermittent observations about sanitation and cleanliness in parts of the facility; prospective families should confirm current conditions during a tour.

Management and notable patterns: The major recurring concerns relate to administration, billing transparency, and infection-control practices. Multiple reviewers raised questions about financial/billing processes and suspected overcharges, indicating a need for clear explanations of fees and billing procedures. There are also references to inconsistent enforcement of isolation or infection-control protocols, which led at least one family to remove a resident. Taken together with comments about leadership instability and agency staffing, these patterns suggest operational areas where the facility may be improving or vulnerable to fluctuation.

Recommendation: Greenfield appears to offer strong frontline caregiving, a robust activities program, and a comfortable environment for residents who benefit from an engaged staff and social programming. Prospective residents and families should confirm current staffing levels and turnover, review the facility’s billing and financial policies in detail, and inquire about recent sanitation inspections and infection-control protocols during a visit. These checks will help weigh the facility’s clear strengths against the operational concerns noted by reviewers.

About Greenfield Rehabilitation & Health Care Center

Greenfield Rehabilitation & Health Care Center in Greenfield, Iowa, has served seniors for over 30 years, focusing on skilled nursing, physical therapy, and memory care in a supportive setting where compassionate, personalized care is a big part of daily life, and the staff have experience working with a range of resident needs. The facility offers skilled nursing care around the clock, with help available for both short-term recovery and longer stays, and there's always a nurse on hand, though the nurse turnover rate here is higher than the state average, and their nurse hours per resident are about the same as what you'll find at other places in the state. Greenfield Rehabilitation & Health Care Center has 46 certified beds and sees about 32 residents a day, and you'll find services like occupational therapy, speech therapy, palliative and hospice care, and respite care for folks who need a safe place to recover after an injury or surgery.

Residents get personalized care plans, and the team works to help people regain independence if they've lost some of their strength, and there's always attention to physical, emotional, and intellectual health, though the facility has had deficiencies reported in past inspections, including an infection-related issue and about 16 total deficiencies, like care planning and infection control, so families might want to check inspection records. The center is managed by Tutera Senior Living & Health Care LLC since 2019 and is a for-profit corporation, co-owned by Arlene Hinz and Roger Hinz, who each have half ownership.

Amenities here include things like a fitness center, theater room, walking paths, gathering and activity spaces, and beautifully landscaped patios and gardens, and it's pet-friendly, so folks who want to bring their pets can do so, plus there's transportation service at cost, resident parking, and guest parking too, along with the option for family and friends to stay overnight. There are home-style apartments with multiple floor plans, private rooms, wheelchair-accessible showers, kitchens, Wi-Fi, and cable TV, plus restaurant-style dining where dietary needs and guest meals are accommodated, and folks have access to a full-service hair salon and a relaxing whirlpool spa.

The center has a structured program of daily activities, including religious services both onsite and offsite, gardening club, grocery shopping, games, live performances, and day trips, all meant to encourage socializing and a good quality of life. Staff keeps the environment clean and safe, and there's a focus on letting residents age in place, which means helping people stay in familiar surroundings as their needs change. Greenfield Rehabilitation & Health Care Center is known for friendly, compassionate care, but families should also be aware of past citations and a higher nurse turnover when considering the right fit for their loved one.
