The place is fresh-smelling, residents seem happy, and the hardworking team goes above and beyond with thoughtful treats and heartfelt dedication. Overall a great experience with clear improvements over time.

    Summary of reviews

The reviews reflect a facility with markedly mixed experiences: several families describe dedicated, compassionate caregivers and strong onsite leadership, while others report significant operational and interpersonal shortcomings. Positive comments emphasize individualized attention, staff members who go beyond routine duties to provide treats and special moments, and visible improvements over time under engaged leadership. These accounts point to occasions where residents appeared well-dressed and cared for, and where families felt supported by particular nurses and administrators.

At the same time, persistent themes suggest operational instability. Many reviewers raised concerns about inconsistent staffing levels and extended response times to call lights and requests for assistance. These delays are tied to an overall pattern of variability in day-to-day care delivery, including uneven personal-care and grooming practices. Several accounts also describe a decline in perceived quality following infection-control events, highlighting how crisis-period policies and staffing pressures can affect continuity of care.

Communication and management practices are another recurring area of concern. Families described difficulty obtaining timely, transparent updates about clinical changes and end-of-life situations, and some expressed frustration with the tone and responsiveness of staff and leadership. There are also serious, individual claims — including allegations of missing personal items — that raise questions about accountability and safeguards for residents’ belongings.

Facility upkeep and dining drew mixed feedback. Positive notes mention a lack of pervasive odors and staff attention to resident appearance, but other reports reference maintenance shortfalls such as damaged fixtures and sanitation concerns in some areas. Food-service quality was characterized as variable, with some families dissatisfied with meal reliability and offerings.

Overall, the pattern in these reviews is polarizing: many families have had positive, appreciative experiences tied to specific staff and leaders, while others have encountered operational breakdowns that affected responsiveness, cleanliness, dining, and communication. Prospective residents and families should weigh the facility’s strengths in compassionate caregiving and engaged leadership against reported variability in staffing, maintenance, and transparency. If considering this center, visit at different times of day, ask about staffing ratios and recent corrective actions, and inquire about policies for family communication, visitation flexibility, and protection of personal property.

About RiverView Manor - Pleasant Valley

Aspire of Pleasant Valley Nursing & Rehabilitation Center, also called RiverView Manor, sits at 17990 S Spencer Rd in Pleasant Valley, Iowa, and it's a place that does everything from short-term rehab to long-term nursing care, respite care, and assisted living, and you'll find they help seniors with health problems like Alzheimer's, heart issues, or depression, along with those living with mental illness, emotional disabilities, or intellectual disabilities, and they seem to have a knack for individualized care, since every person gets their own plan based on what they need and what works for them. The center has memory care for folks with dementia, and specialized programs like Pathways to Active Living (PALs) to make sure residents stay engaged with physical, mental, emotional, and spiritual wellness, and there's always something going on, whether it's gardening, bus trips, or shopping, and you'll even find devotional activities and group outings on the schedule.

The staff includes registered nurses, LPNs, CNAs, and specialists who handle wound care, medication, diabetes, and tracheostomy care, and they're around 24 hours a day, seven days a week, so there's always help if someone needs it. The building isn't in a hospital or part of a continuing care community, but it does take Medicare and Medicaid, and it's got 44 beds, with room enough most days for about 37 residents, and those who live here are mostly around 81 years old. The community can take care of people coming straight from a hospital with tough medical needs, and for people who need rehab after a stroke, surgery, or injury, they offer physical, occupational, and speech therapy as needed, working to help folks get stronger, more independent, and comfortable with daily activities, whether they're staying a few weeks or a few years.

RiverView Manor has amenities like home-cooked meals, Wi-Fi, private and semi-private rooms (some with pet visits), comfy indoor spots to gather, and even designated smoking areas. Booster shot rates could be higher, but vaccination rates against COVID-19 are strong among residents and decent among staff. Ratings are mixed: long-term and quality measures come in at four stars, but staffing and health inspections only earn one star, so it's not perfect, and there's work to do. Still, there's a focus on safety-sprinklers throughout the building-and daily living support like help with bathing, dressing, and medication, so people aren't left on their own. There's a wide range of specialized rehab and addiction treatment centers under the same company, including programs for veterans, LGBTQ+ folks, teens, and those needing luxury or holistic settings, as well as centers for dual diagnoses and specific behavioral issues.

You'll also find that RiverView Manor has residential councils, common spaces for socializing, high-speed Wi-Fi, and the staff organizes transportation for medical appointments or errands. Meals always aim to be nutritious, with snacks available, and both acute and chronic conditions like diabetes, CHF, COPD, cancer, and stroke see dedicated support from trained healthcare workers. Consulting services for dental, vision, psych, podiatry, and respiratory needs can be brought in, and there's restorative care for ongoing rehab, as well as memory-enhancing activities for individuals with dementia. RiverView Manor is a for-profit LLC, owned by Riverview SNF Operator LLC, and managed by the RiverView Manor name, with both Medicare and Medicaid participation and oversight from WPS - Iowa. Visiting is open 24 hours each day, and there are always people available to help families and residents with questions or needs, so families don't have to worry about finding someone to ask.

While the building and services have their strengths-like four-star long-term and quality measure ratings-there are areas where staff and inspection ratings are lower than some might like, but the team here stays focused on making life as safe, comfortable, and supportive as possible for anyone who calls this center home.
