    Negligent, abusive care; avoid facility

    I lived here and endured negligent, abusive care - repeated delays and outright withholding of pain medication, rough handling that caused bruises and bedsores, and staff locking doors and locking me in my room at night. Communication was a disaster; caregivers ignored tips and protocols, a resident died, and state/DIA investigations and lawsuits later uncovered serious issues. I do not recommend this facility.

They emphasize a family-for-life approach and try to keep things feeling home-like with daily activities, life enrichment programming, and visits from community members performing music or other talents, and there's even an "adopt a grandparent" program with local fourth graders who come in to spend time with residents, creating connections across generations. There's a special focus on communication, too, as they've invested in iPads for residents and families to use when face-to-face visits aren't possible, and they made sure to keep families updated, especially during the pandemic, about changes and events in the building. The care covers medical, psychological, nutrition, and spiritual needs, and the staff has been noted by families for being respectful and helpful with 24-hour nursing for day-to-day and special needs, as well as terminal and pain management care for hospice patients, and short-term rehab and long-term care both focus on helping people keep as much strength and ability as they can. The administrator Gwen Larsen has received recognition and the place itself holds a 2021 Governor's Award for Quality Care, but there are some realities to consider, as inspectors have found 25 deficiencies in total, including infection control and dietary issues like failing to have proper programs and food sources, and the most recent 2024 inspection reported 15 deficiencies, one tied to infection and two involving infection overall, so while the team does a lot of things for the residents, there are some areas still needing work. The nurse turnover rate is high at 53.1%, above the state average, and nurse staffing hours are below average, sitting at 3.47 hours per resident each day compared to the statewide 3.8, though the facility's got a full array of clinical, behavioral, and recreational programs to round out the experience, with respite, day programs, and home health monitoring all in the mix. As of now, the facility isn't accepting new patients, but for those already living there, there's support for independence, daily routines, and health, with organized activity plans, specialty care, and a general focus on making life meaningful and connected while addressing individual circumstances and care needs.
