    Caring clean facility exceeded expectations

I placed my mom at Essex for a two-year stay and was consistently impressed - clean, bright facility with a pleasant vanilla/sugar-cookie scent. Nursing, rehab (PT) and activities (music, concerts, gym, courtyard, lots of options) exceeded expectations, the food was good, and staff treated her like family. Jamie in social services and admin Sabrina were outstanding and responsive during discharge and medication coordination; I highly recommend this caring, well-run place.

    Summary of reviews

Reviewer feedback for Essex Nursing and Rehabilitation Center is strongly mixed, with a clear split between highly positive experiences—particularly for short-term rehabilitation—and serious concerns about some aspects of long-term clinical care and facility operations. Many families praised the rehabilitation program, physical therapy resources, and discharge coordination; those commenters highlighted responsive social services staff, helpful transition assistance, and progress in mobility. Several reviewers also noted clean, bright common areas, a range of activities (music programs, concerts, bingo, holiday events), an accessible courtyard and pond, and amenities such as a beauty salon and meeting rooms that support family involvement.

At the same time, a recurring cluster of clinical concerns appears across reviews. These include inconsistent medication administration and monitoring, delays or gaps in wound and infection management, and reports of weight loss and missed clinical interventions for some residents. Several families described slow staff response times and delays in attending to basic care needs; these issues contributed to perceptions of unreliable day-to-day clinical oversight. There are also repeated mentions of supervision and safety lapses within memory-care areas, which raised safety and privacy worries for residents and families.

Staffing and management-related themes are prominent. Positive accounts emphasize caring, long-tenured front-line staff and specific personnel (social services staff named positively) who facilitated good experiences. Conversely, other reviewers described short staffing, variable professionalism, and communication breakdowns with administration—particularly around notification of incidents, changes in condition, and end-of-life events. Some reviewers tied a decline in perceived quality to leadership or ownership changes; others cited inconsistent enforcement of procedures and a need for more active administrative oversight.

Dining and amenities receive mixed ratings: several families praised meal variety and a pleasant dining environment, while others reported inconsistent meal quality, heavy carbohydrate–focused menus, and reduced holiday observances. Facility features are similarly dual-natured: many found rooms and common spaces clean and comfortable, while some reported concerns about mattresses, towels/soap availability, and odor or sanitation issues in specific areas.

Finally, there are serious procedural and safety concerns that merit attention: allegations of theft and controlled-substance diversion were raised by reviewers, along with reports of unclear billing or unexpected charges for supplies. Environmental and equipment safety risks (for example, hazards near nursing stations or problematic equipment) and gaps in clinical incident response and family communication were also noted. Prospective residents and families should weigh the strong rehabilitation and activity offerings against these operational concerns, consider a site visit, ask for documentation of clinical protocols (medication administration, wound care, memory‑care supervision), and verify how the facility handles incident reporting, billing transparency, and staff training/turnover before making placement decisions.

The center provides long-term care, assisted living, and independent senior living, with all services falling under the Long Term Care category and accepting Medicaid or Medi-Cal for financial help. It supports people who need high acuity care, and offers both inpatient and outpatient therapy through Signature Healthcare of Spencer County, with services like physical, occupational, and speech therapy on-site to support different needs. The staff help with wound care, care for stroke, cardiac, pulmonary, or neurological conditions, and create personalized care plans using teams that focus on mind, body, and spirit. The center works to connect the physical, social, emotional, and spiritual sides of care for each resident, and welcomes adults of different ages who need help with health or well-being. The skilled nursing team is available 24 hours a day for both short-term stays and long-term care, supporting recovery from illness or surgery or helping with chronic conditions. The facility's interdisciplinary approach blends medical and social care, creates smooth transitions between home, hospital, and the nursing center, and encourages family connections. Safety is important here, so they have access control, air cleaning with HEPA scrubbers, electrostatic cleaning, bipolar ionization, and regular screening and mask policies to limit COVID-19 and other infections, with advice from university epidemiology experts. Essex Nursing and Rehabilitation Center has earned a 5-star quality measures rating from CMS.
