    Caring staff clean facility comfortable

I'm impressed-my mother settled quickly and we feel comfortable leaving her here. Staff from nursing to kitchen are loving and attentive; Mona McCubbins and Jasmine made the transition smooth and celebrated my mom's individuality. The facility is clean, safe, offers good meals, effective therapy, plenty of activities (we even enjoyed Rocko the pony at a festival) and monthly family brunches. I'd happily move in here when I need extra care.

    Summary of reviews

Franciscan Health Care Center elicits a wide range of experiences. Many families and residents praise the facility for strong rehabilitation services, a comprehensive clinical team, and a warm, family-like atmosphere. Therapy departments (PT/OT/ST) are frequently described as knowledgeable and effective, and short-term rehab outcomes are often positive. Numerous comments highlight friendly, attentive staff members, useful amenities (spacious apartments with kitchenettes, patios, emergency call systems), and active programming including music, bingo, seasonal events and family brunches. On-site features such as a chapel, beauty salon, courtyards, and pet-friendly policies are cited as quality-of-life strengths.

Care quality shows two contrasting patterns. Positive reports emphasize compassionate nursing aides, prompt CNAs, thorough therapists, and social workers who facilitate smooth transitions. At the same time, a substantive set of accounts describes operational inconsistencies: variable responsiveness across shifts (particularly nights and weekends), gaps in clinical coverage on weekends, and concerns about medication-management and wound/skin care practices. A small number of serious allegations—including medication-management concerns and worries raised following a resident's death—appear in the record and suggest the need for close review of clinical safeguards and escalation protocols.

Dining and activities are generally described as meaningful contributors to resident life; many enjoyed three meals a day, themed brunches, live music and varied programming. However, meal quality and adherence to diet restrictions are reported inconsistently: some family members praised the kitchen and menu variety, while others noted cold meals, delayed service, or diet orders not followed. Activity offerings and family events are a recurrent positive, supporting social engagement.

Facility condition and amenities receive mixed but often favorable comments. Several reviewers describe clean, well-maintained rooms and communal spaces, and many appreciate the apartment layouts and outdoor spaces. Conversely, there are recurring notes about maintenance variability—older bathrooms, plumbing issues, and noisy rooms near ambulance entry—that suggest physical upkeep is uneven in parts of the building.

Communication, management and operations are another mixed area. Individual staff members and social workers receive strong endorsements for helpfulness and responsiveness, but reviewers also describe leadership availability problems, administrative turnover, rushed or poorly communicated discharges, and billing-contact issues. Scheduling challenges—difficulty arranging follow-up appointments or weekend therapy—surface repeatedly and can affect continuity of care.

Overall pattern: the center offers robust rehabilitation services, committed front-line staff, and a range of amenities that many families find comforting and restorative. At the same time, there is notable variability in how consistently those strengths are delivered across shifts and departments. Prospective residents and families should consider an in-person tour, ask specific questions about weekend staffing and therapy coverage, review medication and wound-care protocols, and confirm discharge, billing and infection-control procedures before making placement decisions.

    About Franciscan Health Care Center

Franciscan Health Care Center sits in South Louisville, Kentucky, as part of Trilogy Health Services, where seniors find support and comfort in their daily lives. This place has private suites-each with its own shower and either a kitchen or kitchenette-that let folks keep their independence, but also provides help when someone needs it. Residents can enjoy chef-prepared meals for breakfast, lunch, and dinner in a private dining room or join others in indoor and outdoor common areas, like the courtyard and a resident garden, and if someone's got a pet, they're welcome, too. Housekeeping and laundry services help keep things tidy, and the facility covers utilities and cable TV, along with wireless internet, so people can stay connected without worry. Seniors who need extra support get personalized care, including medication management and 24/7 nursing, and those who require skilled nursing care, rehab, or on-site dialysis services find it here, all under one roof. Adults living with Alzheimer's disease or memory loss get specialized support, and there's daily programming and Life Enrichment activities to keep everyone engaged. Residents can get help with daily needs, and short-term stays through respite care are available for those needing a temporary pause from home. Transportation services, handicap parking, and a 24-hour emergency alert system add peace of mind. The center features both long-term and short-term care options, along with independent living, assisted living, and even therapy services to help folks keep as active and independent as possible. The grounds include spaces for outdoor relaxation, and there's a chapel on-site where Mass is offered regularly, which some folks find comforting. Veterans find community here, with over 1,400 living in Trilogy Communities. This place maintains simple house rules during the week, though things are a bit more relaxed on weekends when management isn't present. The center's reputation has eased many concerns families have about assisted living and nursing homes, and it's not far from local names like the Belle of Louisville and Churchill Downs, which might mean something to some families in the area. There's on-site salon services and a private dining room for special gatherings, and residents get help with their daily routines if needed, making this community a steady place for seniors as their needs change.
