    Compassionate responsive staff, excellent therapies

I felt right at home - the staff were compassionate, responsive, and genuinely welcoming, and Mrs. Young, the social worker, was a true standout. The facility is very clean, offers excellent therapy/rehab and dialysis support, tasty meals, and lively activities (storytelling, singing, patio), so I'd definitely recommend it.

    Summary of reviews

Feedback for Signature HealthCARE at Summerfield Rehab & Wellness Center is highly polarized: many accounts highlight strong rehabilitation services, compassionate aides, and engaged social work, while an overlapping set of accounts raises substantive concerns about operational consistency and safety. Prospective residents and families should be aware that experiences appear to vary widely by unit, shift and time period; both strongly positive and strongly negative experiences are described.

Care quality and clinical processes are a mixed picture. Physical and occupational therapy teams receive frequent praise for effectiveness and focus on returning residents to the community. At the same time, reviewers describe inconsistent nursing responsiveness, delayed or irregular medication administration, and gaps in personal-care routines (bathing, skin treatments and toileting assistance). Several reviews note incidents that suggest weaknesses in transfer and fall-prevention practices and in follow-through on care plans. A small number of accounts reference hospital readmissions and concerns following a resident's death; such serious outcomes were described alongside broader themes of delayed response and documentation lapses.

Staffing and staff conduct show considerable variability. Many family members and residents commend compassionate aides, helpful nurses, and standout clinicians — including social work and therapy staff who are named as particularly effective. Conversely, there are substantial concerns about staffing consistency, particularly reliance on agency nurses, uneven training and competency, and lower supervision on weekends and evenings. Communication tone and timeliness also vary: some families describe proactive administrative engagement, while others cite unreturned calls, lost paperwork, and limited incident reporting to families or regulators.

Dining and activities are likewise inconsistent. Several reviewers praise creative, well-presented and varied meal options and active programming (storytelling, music, patios, social cafeterias and daily events). Other accounts describe interruptions in meal service, temperature/quality problems, and missing mealtime supports. Activities and social programming are generally cited as a strength when staffing and leadership are stable.

Facilities and housekeeping show uneven performance. Positive comments reference a clean, fresh environment and strong housekeeping in parts of the facility. Countervailing concerns include maintenance issues (pipe bursts, flooding, broken equipment), laundry management problems, and sanitation or odor concerns in specific areas. Infection-control and hand-hygiene practices are identified as an area needing improvement in some accounts.

Management and oversight present a split narrative. Some reviews credit recent leadership changes and active administrators with rapid improvements and good communication; others describe leadership turnover, inconsistent oversight after ownership change, and even serious administrative allegations including financial and regulatory concerns. These contrasting descriptions suggest that leadership stability and transparency materially affect resident experience.

Overall pattern: strengths center on rehabilitation therapy, committed individual staff members, social work engagement, and community-oriented activities. Recurrent operational weaknesses include staffing consistency, clinical-response timeliness, medication and documentation practices, sanitation and laundry controls, and variable management follow-through. Families considering this facility should weigh the strong therapy and social supports against the documented variability in nursing continuity, safety processes, and facility operations; when possible, verify current staffing patterns, leadership stability, and recent state survey or inspection results during the decision process.
