    Caring clean facility and staff

I stayed here for short-term rehab and felt genuinely cared for - the beautiful, clean facility and friendly, attentive staff (including RN Candace, social services Tiffany, billing Carrie and administrator Kristy) made a big difference. Rehab and nursing were excellent, issues were handled promptly, and the community felt family-like, so I'd recommend it for short- or long-term care.

    Summary of reviews

The reviews present a mixed but informative picture of Creekwood Nursing & Rehabilitation. Strengths repeatedly noted include a pleasant, wellโ€‘maintained facility with active resident programming and a familyโ€‘oriented culture. Several contributors highlighted effective shortโ€‘term rehabilitation services, helpful admissions and billing staff, and visible administrative leadership. Specific staff members in clinical and administrative roles were praised for professionalism and for resolving issues efficiently. The dietary manager and aspects of the admissions process also received positive attention, and community events and socialโ€‘services coordination were described as strengths.

Care quality appears uneven. Many reviewers praised compassionate, dedicated caregivers and strong rehabilitation outcomes; others described operational gaps such as delayed bathing and toileting assistance, inconsistent callโ€‘light responsiveness, and unclear assignment or identification of nursing staff. These operational issues suggest variability in dayโ€‘toโ€‘day caregiving practices and documentation. Therapy services are generally viewed as beneficial for returning residents home, though there are reports of inconsistent therapy scheduling and early departures that affected expected treatment completion.

Dining and nutrition show a divide between leadership and execution: the dietary manager is frequently commended, yet reviewers noted inconsistent meal quality, limited menu variety, and occasional lack of effort from kitchen staff. Management and administrative performance also receive mixed feedback. Some reviewers described strong, responsive administrators who address problems and support families; others experienced a lack of followโ€‘through on feedback and slow communication after critical events. Admissions and billing teams were most often singled out for positive, helpful interactions.

Staff culture and professionalism are recurring themes. Many families and residents described caring, attentive staff and a positive team environment. Conversely, there are documented concerns about staff conduct, instances of perceived rudeness, and breaches of confidentiality. These contrasting impressions point to variability in staff training, supervision, and morale. Facility aesthetics and programmingโ€”clean, attractive spaces, resident activities such as fishing events, and a community focusโ€”are reliably cited as positive features.

Overall, Creekwood appears to offer strong rehabilitation services, supportive administrative processes in several areas, and an attractive physical environment. Prospective residents and families should weigh these strengths against recurring operational issues: inconsistent responsiveness to basic care needs, variability in therapy delivery, mealโ€‘service inconsistency, and occasional communication and confidentiality lapses. When evaluating placement, families may wish to ask specific questions about current staffing levels and turnover, callโ€‘light response metrics, therapy schedules, meal options, and procedures for family communication and confidentiality to determine how current operations align with their expectations.

    About Creekwood Nursing & Rehabilitation

Creekwood Nursing & Rehabilitation sits at 107 Boyles Dr in Russellville, Kentucky, with a modern look even though the building is about ten years old, and it offers 104 certified beds though most days about 90 residents are there, and folks come for skilled nursing and rehab services because, well, that's what the place focuses on, and they try to give everyone care that fits what they need, which means each person has a care plan that the staff follow to help with recovery, health, or comfort, depending on the resident. There's a family-focused feeling in the way things get done, and the staff mostly speak English, doing their best to support everyone, partly by tailoring activities and daily routines to fit what each person is able to do or what's best for them, so no one feels left out or overlooked.

Management changed to Clearview Healthcare Management Ky LLC back in January 2020, and Creekwood operates as a for-profit place, with Simcha Hyman and Naftali Zanziper as affiliates, and they do try to run things with heart and attention to detail, given the balance of about 3.34 nurse hours per resident each day, which is a little under the state average of 3.9, yet nurse turnover is lower here at 38.1% while the state average is nearer 48%, so you tend to have familiar faces. The facility has specialized areas for therapy and patient recovery, offering rehabilitation, and plenty of amenities so folks can stay active or just sit in a clean, calm spot with a home-like touch.

State inspectors came out on October 31, 2024, and found four deficiencies-one had to do with infection control, and overall Creekwood's had three infection-related citations, along with issues in nutrition and dietary standards since some food wasn't purchased or served the way the rules require, plus some reported shortcomings in giving treatment and care according to what people want and need. No extra languages are spoken by staff, and there's no fax, but they do have tools for patient profile sharing and updates, including a printable vCard, and they've got a directory that gets updated monthly, although the office hours aren't public and right now they aren't accepting new patients. Creekwood's approach mixes personalized healthcare, activities, and a modern atmosphere, all with the hope of comfort and support, but as with any care facility, it has areas that need ongoing attention and steady improvement.
