    Clean facility, excellent personalized care

I placed my grandmother at West Liberty and have been very pleased - a clean, updated small facility with excellent rehab and truly personalized, family-like care. The staff went above and beyond (special thanks to Anna Cantrell, Rebecca Lane and Angie Ratliff); they were compassionate, knowledgeable and attentive, and I would recommend this facility for thoughtful, resident-focused care.

    Summary of reviews

Overall sentiment: The review summaries for West Liberty Nursing & Rehabilitation are overwhelmingly positive. Reviewers repeatedly emphasize compassionate, attentive care and express strong satisfaction with the facility. There is a consistent pattern of high praise for nursing staff, nurse aides, and specific employees (notably Anna Cantrell and Rebecca Lane). Multiple reviewers state they would recommend the facility and describe the experience as the best decision for their loved one.

Care quality and staff performance: The dominant theme across reviews is the quality and character of the staff. Descriptors such as "compassionate," "caring," "knowledgeable," "supportive," and "hardworking" recur throughout. Reviewers report that staff treat residents like family, provide personalized attention, and often go above and beyond routine duties (examples include holding a resident's hand through processes, making residents feel at home, and family members receiving personal-level communication). Nurses and nurse aides receive particular acclaim; phrases like "exceptional nursing staff," "top-notch staff," and "attentive nurse aide" appear multiple times. Specific staff members are named and praised, indicating memorable, high-impact interactions that stood out to families.

Rehabilitation, clinical care, and outcomes: Reviews frequently describe West Liberty as a great rehab facility and recommend it for rehabilitation needs. Commenters note attentive rehab staff and strong clinical outcomes, suggesting effective therapy and recovery support. The clinical environment is portrayed as high-quality and resident-focused, with staff willing to be contacted outside regular hours and showing readiness to meet residents' needs. Several reviews state that clinical and supportive care "exceed expectations," reinforcing a perception of reliable therapeutic services.

Facility environment and safety: Reviewers describe the facility as clean, updated, and safe for residents. The building is noted as modern or refreshed, contributing to a positive first impression. The facility's small, rural setting is mentioned, often framed positively — fostering a close-knit, family-like atmosphere where staff and residents form strong relationships. Safety and respectfulness are explicitly noted, with families feeling comfortable leaving loved ones in the facility's care.

Family experience, communication, and accessibility: Communication and family interactions are strong recurring themes. Families report personalized attention to them as well as to residents, with staff described as courteous, understanding, and hospitable. Several reviews highlight staff accessibility, including willingness to be contacted at home, and praise for staff who provide emotional support through difficult times. This level of personal engagement strengthens families' trust and contributes to the frequent recommendations and expressions of gratitude.

Management, culture, and staffing implications: The reviews imply an organizational culture that supports compassionate caregiving; statements like "awesome place to work" and "best staff" suggest positive morale among employees. Some reviewers even assert staff deserve raises, indicating recognition of staff effort and potential under-compensation concerns from the community perspective. There are no consistent complaints about management or operational issues in the provided summaries.

Notable absences and potential limitations: The reviews provided focus heavily on staff, clinical care, and facility condition but do not offer detail about certain operational aspects such as dining quality, activity programming, transportation, admission logistics, or billing and insurance experiences. The facility's small, rural nature is mentioned but not framed as a negative in these summaries — however, potential residents should consider location-related factors (e.g., travel for visitors, local specialist availability) separately since reviews do not address them. Importantly, no substantive negative patterns or recurring complaints appear in the supplied summaries.

Conclusion: Based on these review summaries, West Liberty Nursing & Rehabilitation is viewed very positively by families and likely provides high-quality, compassionate, and personalized care in a clean and updated environment. Strengths are particularly pronounced in nursing and rehab services, staff engagement, and family-centered communication. The lack of reported negative issues in these summaries is notable, though prospective residents or families may want to seek additional information on aspects not covered here (dining, activities, costs, and access to specialized outpatient services) to get a complete picture before deciding.

About West Liberty Nursing & Rehabilitation

West Liberty Nursing & Rehabilitation sits in a small, rural town in Kentucky and has room for up to 48 people, though the average number of residents tends to be around 43 most days, and you'll find their building at 774 Liberty Rd, West Liberty, KY 41472, where they focus on both short-term and long-term care and offer memory care, respite care, hospice, and outpatient therapy all under one roof, with a specially-trained care team that includes nurses on duty around the clock. They'll help with daily living tasks, medication management, IV therapy, and diabetes management, and if you need speech therapy, occupational therapy, or balance training, they offer those, too, along with care plans aimed at helping each person reclaim their independence as much as possible, and they can arrange services like mental health consultations, x-rays, lab work, podiatry, and private home care. West Liberty Nursing & Rehabilitation, which is affiliated with David Marx through ownership and is managed as a for-profit corporation, has social care as a main focus since it's part of The Social Care Network, and all staff speak English. The rooms have high-speed internet and can support smartphones and tablets, so there's help with technology if you need it. The facility has seen several inspection deficiencies, including those related to infection control, care planning, resident rights, and accident hazards, with a standard inspection as recent as January 25, 2023, showing five deficiencies, and reports list a total of 16 deficiencies over time, with two specifically involving infection concerns and a nurse turnover rate of 77.8%, noticeably higher than the state average, along with nurse staffing levels below Kentucky's average as well, so while there are plenty of services, it's a good idea to look over the inspection reports and weigh the pros and cons if you're considering care here, though right now they're not accepting new patients at this time.
