    Compassionate, clean, responsive senior care

I placed my mom here and have been very pleased - staff are warm, attentive, and quick to respond, with compassionate nurses, aides, and excellent therapists. The facility is very clean, well-maintained, secure, and welcoming, and communication has been clear and helpful. Overall I feel confident in the care and would recommend this place.

    Summary of reviews

Landmark of Plaquemine is described by reviewers as a generally clean, well-kept facility with a rehabilitation focus and many staff who are perceived as caring, professional, and attentive. Positive comments frequently reference helpful reception and front-desk staff, prompt maintenance, supportive social work and hospice interactions, and a welcoming, family-like atmosphere supported by on-site amenities such as a chapel, salon, and facility events. Several families highlighted strong rehabilitation therapy services and named individual caregivers who provided notable support during post-surgical or hospice care.

Care quality is mixed but leans positive for therapy-focused stays. Many reviewers praised nurses and aides for compassion and hands-on assistance, and therapy programs were called excellent by multiple families. At the same time, recurring concerns center on inconsistent responsiveness to resident needs — particularly delays with call-bell response or reachability — and uneven clinical follow-up in some situations. There are also instances of disorganized discharge planning and limited clarity around therapy documentation and goals, which suggests variability in care coordination and transition processes.

Staff and management impressions vary. Numerous reviews commend individual staff members and social-work support for clear communication and advocacy. Conversely, others described dismissive interactions or slow responses from certain staff and felt that some administrative issues were not adequately addressed. This pattern indicates generally capable frontline staff with occasional lapses in consistent managerial follow-through or communication practices.

Dining receives mixed feedback. Several families enjoyed the meals and described them as tasty and well‑presented, while others reported issues with meal temperature, presentation, and adherence to prescribed diets. This points to an operational inconsistency in kitchen service and diet management that prospective residents should clarify during a tour or care-planning meeting.

Facilities and activities are often cited as strengths: the building is described as clean, organized, and pleasant, with community events (for example, holiday or cultural celebrations) that contribute to resident engagement. At the same time, parts of the physical plant are described as dated, with longer walking distances and some accessibility challenges; some rooms may benefit from modernization to improve resident comfort and convenience.

Notable patterns for families considering this facility are the combination of compassionate, therapy-oriented care and a well-maintained campus, paired with variability in responsiveness, dietary consistency, and discharge/therapy communication. For prospective residents and families, recommended tour questions include specific call-bell response expectations, diet accommodation and meal-service timing, therapy goals and documentation practices, and discharge-planning procedures to ensure expectations align with facility operations.
