    Clean attentive staff excellent care

I placed my dad here for a short-term stay and was very pleased. The facility is clean, well-kept (not brand-new), always staffed, and feels safe. Administration, nurses, and aides are attentive, compassionate, and responsive; PT/OT were excellent and the chef makes great meals. Lots of activities, veteran-friendly, with nice amenities like in-room phones and free cable-overall very good care.

    Summary of reviews

Feedback about Colony Center for Health & Rehabilitation is mixed, with clear strengths in staff, therapy and dining but persistent operational concerns related to maintenance and safety. Many families and short-term residents praised the caregiving team, therapy services and administration for responsiveness; at the same time, other accounts describe lapses in facility upkeep and supervision that families should consider during a tour and intake conversations.

Care and staff: A substantial portion of evaluations highlight attentive, compassionate nurses and nurse aides, and an engaged administrative team and head nurses who are responsive to family concerns. Rehabilitation services โ€” both physical and occupational therapy โ€” are frequently cited as effective, particularly for short-term stays. However, there are also descriptions that indicate variability in clinical care quality and concerns about staff conduct and responsiveness in certain situations. Reports of safety incidents and at least one injury suggest gaps in supervision or transfer-safety practices that merit review with facility leadership.

Dining and activities: Dining receives generally positive comments; a dedicated chef and pleasing meals were noted. Recreational programming and veteran-focused activities are described as engaging, and amenities such as semi-private rooms, in-room telephones and cable are seen as conveniences that families appreciate. The facility is used by veterans and offers activities tailored to that population.

Facilities and management: Many visitors find the location clean and well maintained, and administration is often described as communicative and helpful. Offsetting that, several comments identify sanitation and odor concerns in certain areas, toilets and other basic amenities in disrepair, and rooms that do not always match advertised conditions. Communication and incident-response consistency are uneven: while some families praise quick, clear communication, others report delays or unsatisfactory follow-up after incidents. Prospective residents and families should verify current room condition, ask for details about maintenance schedules, supervision protocols, staffing levels and incident-reporting procedures, and consider an in-person visit during different times of day to assess consistency.

About Colony Center for Health & Rehabilitation

Colony Center for Health & Rehabilitation sits in Abington, MA, and has 92 certified beds, and it's run as a for-profit company by Chaim Broyde and Azriel Lieberman since December 2023, and it's known for covering a lot of care needs, offering assisted living, skilled nursing, memory care, and a full set of rehab services, so you'd see folks here getting help for all sorts of needs, with experienced caregivers trying to support people living with chronic pain or needing help after injury or illness, and as far as rooms go, they use studio layouts for residents. The nursing staff reports a turnover rate of 38.4%, with an average of 3.20 nurse hours provided per resident per day, which is around the state average, and they provide round-the-clock nursing, respiratory therapy, and a dedicated medical rehab unit that tries to get people back on their feet, plus you'll spot extra amenities aimed at safety and comfort, with efforts to tailor care plans for each person so you don't see a one-size-fits-all approach here.

The community holds a 4-star rating from the Centers for Medicare & Medicaid Services, and it's got a 7.1 out of 10 average review, making it the highest-rated place in town, but you should know there have been deficiencies and fines-Colony Center received a $9,561 fine after a July 15, 2024 complaint investigation found problems with pain management and falling short of professional care standards, and inspectors have cited the center for 42 total deficiencies, including three related to stopping the spread of infections, so it's important to weigh these records alongside their wellness programs like Sepsis Smart and Fight the Flu Campaign. While the place aims to be thorough-with respite care, help for end-of-life needs, memory support, and fall prevention-it's had issues that have caused actual harm, though inspectors did not say it reached the level of immediate jeopardy to residents' lives, so there's some comfort but also reason to ask questions.

People often mention the care team's compassion and friendliness, and there are supports for families, like respite care programs if you're a caregiver who needs a short break, and folks getting rehab services here can work toward regaining independence if they want to go home, but you do need to stay aware of the mixed report cards when you consider this place for long-term care.
