    Inconsistent care despite caring staff

I had a mixed but mostly negative stay. The building and first-floor areas are clean, and a few staff were phenomenal (Social Services Lisa, charge nurse Rebecca, Sarah Brayton and some CNAs) - friendly, responsive, and helpful; food was often good and some residents did well in therapy. But staffing is inconsistent and short, management unhelpful, and therapy time/scheduling unreliable. I experienced delayed meds (including insulin/antibiotics), privacy breaches, hygiene neglect (soiled/unchanged linens, lost laundry), safety issues, and rooms in disrepair. In short: great people at times, but serious care, supervision, and leadership failures - would not recommend without extreme caution.

About Sarah S. Brayton Nursing Center

Sarah S. Brayton Nursing Center sits in Fall River, Massachusetts, and has served seniors since 1993, with 183 beds spread out over two floors, and you'll find that it's a privately held place giving both post-acute and extended care. The center provides skilled nursing and rehabilitation services, where staff like nurses, therapists, and a dedicated Executive Director work together, and there are healthcare options for both short and long-term stays. The facility offers care for those who need memory support, with environments meant to reduce confusion and help seniors who have Alzheimer's or other types of dementia, and also supplies care for those needing support with daily activities or 24-hour nursing. People can come for short-term respite or even end-of-life care, with a big focus on safety and health programs, like fall prevention, the Fight the Flu Campaign, or the Sepsis Smart initiative.

You'll see the in-house rehab department ready to help with physical, occupational, and speech therapy, and the menu team plans three balanced meals every day, served in a dining area that's earned praise for both taste and good nutrition, so you'll always find something to eat that's cooked fresh. The place prides itself on keeping residents comfortable through small things like air conditioning, cable TV, big dayrooms for everyone, on-site beauty salon, musical entertainment, weekly dog-therapy, and special dining events, along with newspaper delivery and laundry services.

There's staff for social services, too, who help arrange support for residents, and they offer transportation and parking, which makes it easier for visitors and outings. The nursing team provides round-the-clock medical attention, and there's an individualized care planning process so each person's needs are looked at by all the clinical departments, and for those wanting to get home, short-term care is set up to help them regain skills and return to their community. The community holds training for Resident Care Aide certification, runs seminars for staff, and keeps programs in place to cut back on unnecessary antipsychotic use. Since its 2022 partnership with Best Care Services, the center keeps working on changes in care culture and supports care transition programs for easy moves between different settings. Residents can count on things like musical events, recreational activities, and weekly visits from therapy dogs, so there's always something to do. All in all, Sarah S. Brayton Nursing Center gives a wide range of care and services in a stable, caring setting where staff focus on each resident's needs.
