    Very pleased with Centennial Gardens

I placed my loved one at Centennial Gardens and have been very pleased. The staff are caring, dependable, and genuinely motivated to put smiles on residents' faces. Nursing, rehab and activities are excellent, the front desk is friendly, and recent ownership improvements make the facility inviting and comfortable. Staff respond quickly to family questions - I would recommend Centennial Gardens for short or long stays.

    Summary of reviews

Centennial Gardens presents a polarized picture across reviewer accounts. Positive feedback centers on the direct caregiving and rehabilitation side of operations: many families describe compassionate nurses and aides, effective therapy that enabled successful discharges, active programming, and a generally welcoming atmosphere at the front desk and common areas. Recent interior improvements and an inviting environment for both short- and long-term stays are also cited as strengths, and several comments highlight strong teamwork between leadership and direct-care staff.

At the same time, a number of operational concerns recur. Staffing inconsistency and elevated turnover are frequently mentioned and are linked to uneven care responsiveness. Specific care-process themes include incontinence-care delays and broader sanitation concerns, as well as complaints about the timeliness of staff response to resident needs. Communication gaps with families — including delayed notifications after significant events and difficulty reaching front-desk staff by phone — emerge as a persistent issue that affects family confidence.

Facility-level environment and programming are mixed. Positive items include engaging activities and rehabilitation outcomes, but reviewers also describe variability in room amenities (missing TVs, phones, or radios) and noise or behavioral-management challenges on certain units that families found disruptive. Dining was not a common focus in the available comments; there is limited direct feedback on food quality or meal service continuity.

Management and institutional culture are areas of clear contention. Some reviewers praise visible leadership involvement and improvements under new ownership, while others report poor treatment of long-tenured staff, inconsistent oversight, and concerns about motive and transparency. Serious claims surfaced around alleged financial misconduct and tip/theft irregularities; these are significant if substantiated and warrant follow-up through appropriate channels. Additionally, questions about the authenticity of posted reviews and reputation-management practices were raised, which complicates assessment of the facility’s overall performance.

For prospective residents and families, the overall pattern suggests strengths in rehab services, compassionate direct caregivers, and a welcoming atmosphere in parts of the facility, contrasted with uneven operational reliability — particularly around staffing, communication, sanitation-related care, and management practices. Those considering Centennial Gardens should visit in person, ask about current staffing ratios and turnover, request clarification on incident- and end-of-life notification policies, inspect room amenities, and seek documentation or regulatory records regarding the more serious financial or conduct-related allegations before making placement decisions.

About Centennial Gardens for Nursing and Rehabilitation

Centennial Gardens for Nursing and Rehabilitation sits in a quiet part of Crystal, Minnesota, and has been serving the area since 1984, offering skilled nursing care, assisted living, memory care for Alzheimer's and dementia, and rehabilitation services like physical, occupational, and speech therapy for adults with different needs. The facility holds 115 certified beds, accepts Medicare and Medicaid, and provides respite care for families that need temporary support, which really helps out caregivers who need a break. The rooms come fully furnished with private bathrooms, kitchenettes, cable TV, telephones, air conditioning, and Wi-Fi, which makes it easier for residents to settle in, and everyone can join in on daily activities, movie nights, music programs, and both community-sponsored and resident-run events, all meant to help seniors stay social and active.

People living there or staying for short-term care can spend time in the fitness room, community dining area, wellness center, library, gaming room, beauty salon, or outside in the garden and walking paths, where there's always something going on or a quiet spot to rest, and Centennial Gardens offers scheduled transportation for non-medical needs too. Staff is there around the clock for support and supervision, including a 24-hour call system, and helpers provide assistance with daily activities like bathing, dressing, medication management, and transfers, plus medical care for those recovering after an illness, surgery, or hospital stay. There's a focus on each person's health and safety, with attention to both emotional and physical well-being, and a commitment to keeping everything clean and secure, including a sprinkler system for fire safety.

Families looking for care options get help from staff in finding the right fit for each resident's needs and financial situation since options are customizable, and though no resident reviews are published to date, tools are available online to help families search for information and ratings. Centennial Gardens has had two facility incidents, three substantiated complaints, and total fines of $8,125 in recent years, with recent ownership changes and some challenges reflected in ratings-a 1 out of 5 health rating, a 2 out of 5 staff rating, a 3 out of 5 for quality, and an overall MediCare.gov rating of 1 out of 5. There's a staff of English speakers, a transitional care unit, and a therapeutic recreation program, along with resident and family councils. The facility doesn't publish prices but will give details upon request. With a peaceful neighborhood setting, Centennial Gardens focuses on helping seniors feel comfortable, supported, and connected to family, even though the rankings and ratings do show there's work to do.
