    Attentive staff despite occasional lapses

I'm very pleased with the clean, lively, professional facility - wonderful management and caring staff who communicate quickly with family, set up visiting areas, run activities and arrange outings. I did experience medication administration and turning/care lapses that led to hospital readmissions, but overall the team is attentive and comforting. I highly recommend this place - five stars.

    Summary review

Overall sentiment in the reviews of Prairie View Skilled Nursing is strongly positive, with multiple reviewers emphasizing the quality of the staff, management, and the general atmosphere. Words and phrases such as "caring," "comforting," "professional," "clean," "lively," and "well oiled machine" appear repeatedly, and several reviewers explicitly state they would highly recommend the facility. Reviewers commonly note that interactions with staff are warm and that management is described as "wonderful," which contributes to an overall impression of competence and a supportive environment.

Staffing and family communication are consistent strengths. Reviews repeatedly highlight that staff are caring and quick to call families, indicating proactive communication practices. Family-facing logistics are also praised: reviewers note designated settings for visits and organized outings with family members, which suggests the facility prioritizes family involvement and social engagement. These elements, together with statements about good family communication, point to strong relational and operational practices that benefit residents and their loved ones.

Resident life and facilities receive positive comments as well. Multiple reviewers mention resident activities and games, and describe the facility as lively—suggesting an active recreational program and an environment that supports engagement. The facility is also described as clean and professionally run, which reinforces the perception of an orderly, well-maintained setting that is conducive to resident comfort and dignity.

Despite the strong positive themes, there are notable clinical care concerns raised in the reviews that should not be overlooked. Several summaries reference medication administration issues and instances of improper care such as failure to turn residents appropriately; these concerns are tied to subsequent hospital readmissions in at least some accounts. While these negative points appear less frequent than the positive commentary, they are serious because they relate to resident safety and the quality of hands-on clinical care.

In synthesis, reviews portray Prairie View Skilled Nursing as a well-managed, caring facility that excels at family communication, staff responsiveness, cleanliness, and resident activities. However, there is a recurring, albeit less common, pattern of clinical lapses—specifically around medication administration and routine care tasks like turning—that have led to hospital readmissions for some residents. Prospective families and stakeholders should weigh the facility's strong relational and operational qualities against these reported clinical concerns, and consider asking targeted questions about medication management, turning/skin care protocols, and readmission prevention when evaluating the facility further.

    About Prairie View Skilled Nursing

Prairie View Skilled Nursing sits out in the country in Bloomfield, Missouri, with sixty certified beds and usually about forty-two residents living there each day, so it's not too crowded, and you'll find private and semi-private rooms, some with their own bathrooms, all set up with hotel-style furnishings, new floors, and modern touches that still feel traditional in the dining and living areas, and there's even a quiet covered patio where families can visit. They've got a library, free Wi-Fi, cable TV, and a bunch of daily activities like exercise classes, religious services, and special programs, plus chaplain visits if that's something you like, and there's always something happening between entertainment and group events. If you need to get to appointments or outings, they do provide transportation, and housekeeping takes care of cleaning, so you won't have to worry about that.

Their care covers a lot, from skilled nursing and rehab-to-home programs to long-term care, hospice, assisted living, respite, and even bariatric care, and they handle things like IV therapy, wound care, pain management, and pharmacy services, with clinical lab work done on-site. There's always a nurse or doctor around, since they keep staff on duty twenty-four hours a day, every day, using registered, licensed, certified, and restorative nurse assistants, and all residents get help with daily tasks like bathing, eating, or managing their medicine, plus they provide comprehensive rehabilitation like physical, occupational, and speech therapy, with a big therapy room for that. They offer balanced, home-cooked meals from their dietary staff, and there are always call buttons in the rooms that connect right to the nursing station.

The facility has changed hands recently and now has management from Paradigm Senior Management and Forvis Mazars, with direct owners Angela Jones and Benjamin Sells. Inspection reports show they've had sixteen deficiencies, which include two related to infection control and others tied to care practices like helping residents with bladder and bowel needs, infection prevention, and plans to protect range of motion-nothing caused actual harm, but reports say these issues could lead to more than minimal harm if not fixed. Federal inspectors have cited Prairie View for infection control and for making sure services meet professional standards. They offer recovery-focused rooms for people coming from the hospital, subject to what's open, and all in all, while they're dedicated to a full range of senior care and modern comforts, it's worth knowing where improvements have been needed and what services and management you'll find if you're looking at Prairie View Skilled Nursing.
