    Attentive, clean, warm skilled nursing

I'm very pleased with the care my mom receives - attentive, responsive nurses and staff who treat residents like family and are quick to meet needs. The community is clean, home-like and well kept, with a pleasant smell, nice dining area, fresh flowers, daily activities (cards, bingo, manicures, field trips) and many volunteers keeping residents engaged. Leadership is strong, residents are respected and safe, and I'd recommend this warm, skilled nursing home to families seeking peace of mind.

    Summary of reviews

Overall impression: Reviews for Columbia Manor Health & Rehabilitation are strongly mixed. Many families and long-term residents describe a warm, home-like atmosphere with engaged staff, meaningful activities, and evidence of committed clinical care in some cases. Conversely, a significant portion of feedback identifies consistent operational weaknesses that affect everyday care and resident comfort. Prospective families should weigh the facility’s social and community strengths against recurring concerns about basic care processes and environmental standards.

Care quality and clinical concerns: Clinical experiences described range from attentive skilled nursing and timely medical intervention to lapses in routine clinical care. Positive comments highlight responsive medical attention and skilled interventions that stabilized residents. However, recurring themes include inconsistent medication administration, delays in clinical follow-up, gaps in wound care and infection prevention, and at least one account linking an infection to hospitalization and surgery. In addition, reviewers raise concerns about incontinence-care delays and insufficient toileting assistance as an operational pattern rather than isolated incidents. Transfer and resident-movement practices were also noted as an area of weakness that can affect safety.

Staffing, conduct, and management: Many reviewers praise staff who create a family-like environment and provide compassionate support. That said, multiple accounts identify inconsistent staffing levels, particularly on night shifts, and delayed responses to resident needs. Concerns about staff conduct and professionalism appear repeatedly (including cellphone use during duties and lapses in bedside assistance), and there are serious allegations regarding on-duty substance use by staff and administrative irregularities. Leadership and management receive mixed feedback — some reviewers specifically commend social services and nursing leadership, while others describe inconsistency in oversight and follow-through. These mixed perceptions suggest variability in supervisory effectiveness across shifts and units.

Dining, activities, and environment: The facility offers a robust activity program with cards, crafts, outings, manicures, and bingo; volunteers and a busy social calendar are frequently cited as strengths that support resident engagement. Dining experiences are mixed: some reviewers find meals nutritious and adequate, while others describe inconsistent food quality, cold meals, and insufficient feeding assistance for residents who need help. Facilities-related feedback is similarly mixed — several families mention clean, pleasant common areas and an orderly appearance, while other accounts raise sanitation concerns in resident rooms and kitchen areas, as well as odor concerns and pest-control worries. Accessibility is generally reasonable, though some reviewers note that social and dining spaces are distant from certain rooms and that mobility access can be challenging in parts of the building.

Notable patterns and recommendations: The pattern across reviews is one of contrast — strong pockets of compassionate, engaged caregiving and an active resident community exist alongside recurring operational problems that directly affect daily living and clinical safety. Families considering this facility should ask targeted questions about current staffing ratios, infection-control procedures, wound-care protocols, call-response times, and how the facility addresses administrative and billing concerns. Visiting at multiple times of day (including evenings and weekends), meeting charge nurses, and reviewing recent inspection or complaint history can help clarify whether the facility’s positive attributes are consistent and whether the operational gaps have been addressed.

About Columbia Manor Health & Rehabilitation

Columbia Manor Health & Rehabilitation sits at 2012 E Nifong Blvd in Columbia, Missouri, and offers a mix of care options, so folks who need nursing care, memory care for Alzheimer's or dementia, rehabilitation, or even home healthcare can all get help here, and usually you'll see about 37 residents here in a facility that's got 52 certified beds. The team at Columbia Manor focuses on being compassionate, and they try to build care plans that fit whatever each resident really needs, which can range from therapy for recovery, long-term support, counseling, beauty care, or even just a pleasant place to talk with someone in a comfortable common area or out in the landscaped garden areas. Staff are known to be patient and kind, and the facility's got a reputation for taking the time to nurture both physical and emotional well-being, plus there's a focus on making the meals both healthy and tasty.

Columbia Manor is affiliated with the University of Missouri's health network and keeps active ties with local hospital systems and skilled nursing partners, so there's often access to telehealth, emergency care, lab work, and regular visits from providers, which can make the transition from hospital to recovery smoother. They try to help with all the logistics too, like billing, insurance, financial assistance, and planning for what happens after a hospital stay, making things as straightforward as possible, and their website has easy ways for families to download brochures, send messages, and get in touch with local advisors who can give free advice on next steps. There's a network of partner facilities with names like Columbia Post-Acute, Lenior, The Bluffs, and TigerPlace, and you can find them through affiliated sites.

As for inspections, Columbia Manor does have a history you can review, with reports showing 26 deficiencies, including some noted for infection control and resident rights, as well as staffing turnover higher than the state average, and nurse staffing per resident a bit lower than the state numbers, but it's worth mentioning they keep updated on infection control protocols as federal regulations require. The facility's been recognized with several awards for senior living and activities, and there have been recent renovations to update rooms and shared spaces, aiming to make the atmosphere more inviting. Residents have choices in activities for both socializing and wellness-there's a strong emphasis on community engagement, and the surroundings are kept homelike and comfortable to help folks feel at ease and keep their independence as much as possible.

Columbia Manor is managed as a for-profit LLC and is part of Reach LTC, but it's also got ties to non-profit programs through its health partners, which gives it a broad network for both medical and nonmedical services, trying to reach people who are looking for anything from a senior apartment to advanced care for chronic conditions. While there aren't many public reviews right now, and the inspection reports are available for anyone who wants to look, the administration seems open to families sharing feedback and questions online, and they remain focused on helping residents and their families feel supported during what can be a hard and confusing part of life's journey.
