    Compassionate staff and welcoming facility

I'm very pleased with this facility. Staff are consistently friendly, compassionate, and attentive - nurses and caregivers (notably Celia, Amanda and Juanita) provided excellent wound care, prompt medical attention, and empathetic end-of-life support. Communication is helpful and informative, the building is clean and welcoming with nice common areas, garden visits, activities, a beauty shop and regular updates. I feel secure and would be comfortable leaving a family member here.

    Summary of reviews

Community Springs elicits a mix of strongly positive experiences and a small set of serious concerns. Many reviewers emphasize compassionate, attentive caregiving and good clinical coordination between nurses and physicians. Specific clinical strengths cited include competent wound care, prompt interventions in acute situations, and empathetic end-of-life support. Several families described transparent communication practices — an open-door approach, newsletters, calendars and photo updates — that support family involvement and oversight.

Staff and management are frequent points of praise: reviewers often describe caring, supportive direct caregivers and engaged leadership who facilitate hospital transfers or other higher-level interventions when needed. That said, there is a pattern of variability in staff availability and training. Multiple comments describe inconsistent nursing coverage and delays responding to clinical events; a subset of reviewers raised serious allegations about medication and care oversight. These are significant concerns that prospective families should clarify with the facility and with relevant oversight authorities.

Programming and mealtime experiences are generally a strength. The facility offers an active activities program, garden and outdoor-visit access, a mother–daughter program, and an on-site beauty salon. Meal variety was described as adequate by several reviewers, and many residents appear to be engaged and satisfied with social offerings. However, there are occasional notes about meal-service reliability and assistance during meals, which suggest variability in how dining and feeding support are provided.

Facilities and upkeep are presented as mixed but leaning positive: reviewers note ongoing remodeling, bathroom upgrades, larger rooms for long-term residents, and welcoming common spaces such as a lobby, dining area and back patio. Conversely, some reviews raise cleanliness and odor concerns in particular areas (floors, dusting, common spaces), indicating uneven sanitation standards across the building.

Overall pattern: a facility that delivers strong, resident-focused care for many families, with active programming and visible investments in the physical plant, but with operational inconsistencies that merit scrutiny. Prospective residents and their families should tour the unit, observe staffing levels and cleanliness, ask for current staffing ratios and incident-response policies, review medication-management procedures, and request documentation about infection-control and complaint history. These steps can help verify whether the facility’s strengths align with an individual resident’s needs and whether the noted operational weaknesses have been addressed.

    About Community Springs Healthcare Facility

Community Springs Healthcare Facility sits at 400 E. Hospital Rd. in El Dorado Springs, Missouri, and has 120 certified beds with about 72 residents staying daily, though the facility recently noted only 105 beds are available and isn't taking new residents right now. The staff, led by Administrator Vautrin Cadle, helps people with skilled nursing care, memory care needs, and rehab services, all in one place, and they serve both long-term and short-term residents, plus those needing special help after a hospital stay, illness, or surgery. There's a 15-bed high-functioning memory care unit that opened in February 2024, focusing on residents with Alzheimer's and dementia, and the place has a caring setup for memory support, with policies honoring residents' rights and steps to support resident self-determination.

The nurses and aides at Community Springs give 24-hour supervision, help with personal care like bathing and dressing, and manage medicines, but their average nurse staffing hours per resident per day (3.31) is a little under the Missouri state average, and nurse turnover is slightly higher than most places too. Residents get care planning by a team of health professionals, and they set up a full care plan within seven days of assessment, with regular reviews and changes as needed. As a not-for-profit, the facility does take Medicare and Medicaid insurance and is part of the Missouri Health Care Association. English is spoken by the staff.

The place is comfortable, with private and semi-private rooms, private bathrooms, kitchenettes, air conditioning, cable TV, Wi-Fi, and high-speed internet. There's a spa with jetted tubs and private showers for relaxation, a full-service salon for grooming, a library, gardens, walking paths, outdoor courtyards, and a unique bird aviary. Residents enjoy group activities like music, bingo, arts and crafts, movies, fitness, gardening, and luncheon outings, with both resident-run and community-sponsored options. The facility even has a community theater, arts room, fitness and wellness programs, game room, and a special "Care Finder" program, though details on the last one aren't clear.

Meals are served all day in a dining room, are cooked by professional chefs, and use menus balanced by a registered dietitian; residents have restaurant-style dining, with choices that respect allergies and dietary needs. The staff provides laundry and housekeeping, concierge service, help with moving in, and scheduled transportation, plus there's guest parking. Security covers the place day and night, including a 24-hour alert system, overnight lockdown from 9 p.m. to 6 a.m., and 24/7 visiting hours in a secure environment.

Community Springs Healthcare Facility is a CMS five-star location, though like most places, they've had inspection reports showing some deficiencies, including items tied to care planning, resident rights, and infection control, but they have stated policies to prevent infections and respond to incidents. The number of certified beds is 120, and while the facility holds many amenities tied to comfort and simple pleasures, the staff is always available around the clock, especially for those with more serious medical needs and memory challenges.
