    Attentive staff and welcoming facility

I placed my loved one at Maple Grove Wellness & Rehabilitation and have been very pleased - the staff and CNAs are attentive and caring, management and therapy are outstanding, and the building feels clean, welcoming and family-like with plenty of activities. Admissions and desk staff were helpful, meals have improved, and I would happily recommend this facility to others.

    Summary of reviews

Reviews for Cori Manor Nursing & Rehab (now operating as Maple Grove Wellness & Rehabilitation under new ownership) present a markedly mixed picture. A consistent strength identified across accounts is the direct-care team: many families describe nursing staff, aides, and therapists as compassionate, engaged, and effectiveโ€”especially the therapy department and the social work team, which are credited with proactive coordination, discharge planning and hospital follow-up. Admissions and front-office personnel are also frequently described as supportive and comforting during transitions. At the same time, reviewers describe operational and environmental problems that have materially affected resident experience.

Care and staffing patterns show a split between individualized praise and systemic gaps. Positive notes highlight staff who include residents in care decisions, assist with transportation and dialysis coordination, and maintain warm relationships with families. Conversely, other accounts emphasize inconsistent responsiveness to calls for assistance, delays in attending to residents, and concerns about staff supervision and professional conduct (including smoking and tone). Several reviewers specifically raised concerns about staffing levels at night and gaps in clinical follow-through and therapy scheduling that affected timely care and rehabilitation progress.

Dining and therapy present contrasting impressions. The rehabilitation/therapy program is repeatedly described as a strength, with reports of meaningful progress and successful discharges. Meal service quality appears inconsistent: some reviews note varied options and acceptable meals, while others report poorly prepared or unpalatable food and unreliable meal experiences. Activity programming was not extensively described in the reviews provided, so conclusions about recreational offerings are limited.

Facility condition, maintenance, and safety are significant recurring themes. Multiple reviewers reported visible maintenance issues, environmental safety hazards, overcrowded room arrangements, and areas in need of structural repair and updating. Sanitation and odor concerns in rooms and common areas, as well as incontinence-care management issues, are frequently cited. Pest-control concerns and worries about personal property security and the handling of residentsโ€™ belongings were also mentioned. Maintenance staff receive occasional positive mentions for responsiveness, but overall the physical plant appears to require substantial investment.

Management and administration show signs of transition and attempts at improvement. Several reviews reference new ownership, name change, and outreach from leadership with apologies and stated commitments to higher-quality care, staff stability, and community engagement. At the same time, there are persistent comments about billing and admission disputes and inconsistent administrative follow-through. Notable patterns are the polarization of experiencesโ€”many families praise individual staff and therapy outcomes, while others describe service-delivery and environmental problems that they consider safety risks. Prospective residents and families should schedule an in-person tour, ask for current staffing ratios (including night coverage), review sanitation and pest-control protocols, clarify therapy schedules and expected rehabilitation goals, and confirm billing/insurance procedures before admission.

About Cori Manor Nursing & Rehab - Fenton

Cori Manor Healthcare & Rehabilitation Center is a large, for-profit facility with 144 beds that specializes in both short-term rehabilitation and long-term care services. The center participates in both Medicare and Medicaid programs, making it accessible to a broad range of individuals who require either temporary recovery support or ongoing healthcare and daily living assistance. The facility is not part of a continuing care retirement community, and its structure and offerings are tailored to individuals who need nursing home care rather than an integrated continuum of services.

The center provides care for individuals recovering from hospital stays due to events like stroke, heart attack, infection, or accidental injury. Nursing staff at Cori Manor Healthcare & Rehabilitation Center deliver an average of 2 hours and 38 minutes of staffing per resident each day, supporting residents with both medical needsโ€”such as administering medicationโ€”and non-medical support, including help with daily activities like eating, dressing, and using the bathroom. The staff structure sees consistent weekend and weekday coverage, and the care home is noted for maintaining excellent nurse staffing levels during weekends.

A significant focus at Cori Manor Healthcare & Rehabilitation Center lies in fostering resident ability to maintain independence for as long as possible; approximately 79.6% of its residents are able to continue self-care activities without assistance. Immunization efforts are robust, with nearly nine out of ten residents receiving the annual influenza vaccination.

Inspectors from the Centers for Medicare and Medicaid Services conduct regular health, fire, and safety evaluations at Cori Manor Healthcare & Rehabilitation Center. Findings from recent inspections highlight various areas for improvement, such as timely care planning, accurate resident assessments, and ensuring treatment aligns with individual preferences and goals. The center has also faced citations regarding documentation practices, sufficient communication with residents and their representatives, and the maintenance of a safe, clean, and comfortable environment. Additionally, areas for growth have included ensuring appropriate nurse staffing, maintaining proper infection prevention programs, and providing dedicated rooms for dining and activities with adequate lighting and airflow.

The center emphasizes ongoing education and training for nurse aides, particularly in dementia care and abuse prevention. Food services are required to adhere to professional standards in sourcing, preparation, and service, ensuring both safety and quality. Cori Manor Healthcare & Rehabilitation Center aims to set up structured processes for quality assessment and improvement, involving multidisciplinary teams and regular review meetings. Attention to resident rights, individualized advance directives, and appeals processes in cases of discharge or transfer are also essential elements of the care provided.

Cori Manor Healthcare & Rehabilitation Center combines the resources of a large skilled nursing facility with efforts to create a homelike and supportive environment. Through its nursing staff, support services, and ongoing improvement efforts, the center seeks to address both the clinical and daily living needs of its residents, with particular attention to safety, health, and comfort.
