    Caring staff, clean remodeled facility

I'm really impressed with the top-notch, caring staff - friendly, helpful, and truly attentive to residents. The newly remodeled facility is clean, cozy, and fresh-smelling, and both long-term care and rehab-to-home stays were excellent; overall a great place with only a few minor things I'd tweak.

    Summary of reviews

Daybreak Nursing Center elicits strongly mixed impressions from families. Several reviewers praise the facilityโ€™s physical environment and day-to-day caregiving: the building is described as recently remodeled and well maintained, with a clean, odor-controlled interior and a cozy, home-like atmosphere. Multiple accounts highlight staff who are caring, friendly, and helpful, and the facility appears to provide both long-term care and short-term acute rehab-to-home services that some families found effective for transitions back to home.

At the same time, there are persistent operational concerns that prospective residents and families should weigh carefully. The most prominent pattern is inconsistent staff behavior and professionalism โ€” some families describe attentive, respectful interactions while others describe poor communication and disrespectful treatment of family members. Linked to this are reports suggesting gaps in medication administration and in arranging medical equipment or hospital-level supports during transitions; these suggest the facilityโ€™s clinical coordination and discharge-planning processes may be uneven.

Several reviews raise difficulties with the facilityโ€™s incident response and communication practices. Families described delays or inadequacies in how clinical events were handled, and in some cases expressed concerns about documentation and reporting processes. One category of reviewer statements rises to the level of serious legal concern; there are allegations of criminal-level staff conduct that families say are under investigation. These are significant and warrant direct inquiry with the facility and with regulators when evaluating placement.

Facility-level logistics also appear variable. Some reviewers flagged room-cleanliness inconsistencies and limited room size that restrict family presence or use of personal equipment; others noted a lack of on-site accommodations for family members during stays. Activity and engagement impressions were generally positive where mentioned โ€” reviewers used terms like โ€œfunโ€ and โ€œcaringโ€ to describe staff-resident interactions โ€” but activity programming detail is limited in the feedback.

In short, Daybreak presents a combination of clear strengths in physical environment and, in many instances, compassionate frontline staff, alongside important operational weaknesses in clinical coordination, incident response, documentation, and family communication. Prospective families should tour the unit, ask for documentation about ongoing investigations or corrective actions, request current staffing ratios and medication-management protocols, review discharge and equipment-arrangement procedures, and observe care interactions to assess consistency before making placement decisions.

Care at Daybreak Nursing Center covers a lot, with skilled nursing, memory care, long-term care, rehab-to-home programs, assisted living, bariatric care, respite care, hospice care, and special care services for people recovering from illness or needing support with daily activities, and staff includes certified nursing assistants, licensed practical nurses, registered nurses, as well as physicians assistants and nurse practitioners who know about family practice medicine and geriatric psychiatric issues, and they also have a consulting physician for dementia and psychiatric disorders with 24-hour physician oversight and nursing care, and this center provides physical, occupational, and speech therapy, as well as programs for sensory stimulation, pain management, and healthy weight loss supported by a Registered Dietitian, and you'll find clinical lab services, IV therapy, wound care, and a full team for medical and personal care needs, while you get family services and referrals to dementia support groups.

There's plenty of social space and a good list of events, while residents can run their own activities too. The center employs a Certified Nursing Assistant training program on-site, and supports community-sponsored activities, plus offers communication amenities with telephones and Wi-Fi.

Problems have shown up in some inspections, especially around resident rights, quality of life, and pharmacy matters, and managerial control comes from Forvis Mazars LLP, Paradigm Rehab Services LLC, Paradigm Senior Management LLC, and certain individuals, so folks should know there's a high nurse turnover rate of 61.5%, which is over the state average, and nursing staff hours per resident come out to 2.75 each day, which is below state average. Daybreak accepts Medicare, Medicaid, private insurance, and private pay, and the facility is family-owned and locally operated, offering round-the-clock support, skilled services, and specialized programs for Alzheimer's and dementia, plus a focus on safety and resident comfort as best as possible in a setting that tries to be home-like.
