    Compassionate care, strong therapy, satisfied

I stayed two weeks at Life Care after surgery and received very good, compassionate care - strong PT/OT that helped me regain strength and return home, attentive nurses and aides, and supportive social workers and admissions staff who went the extra mile. My room was cleaned daily, the facility felt welcoming and tidy, and the meals (meatloaf and breakfast stood out) were satisfying. I'm grateful to the caring professionals who made my short stay comfortable and would recommend this center.

    Summary of reviews

Life Care Center of Sullivan elicits strongly mixed feedback. Many families and residents praise the facility for its compassionate caregiving, strong therapy team, and helpful admissions staff. Physical and occupational therapy are frequently highlighted as effective and outcome-focused, with several accounts of successful discharges and measurable functional improvement. Housekeeping and the general cleanliness of private rooms are often commended, and the atmosphere is described as welcoming with a robust activities program and occasional pet visits that contribute positively to residents’ well-being.

At the same time, there are recurring operational concerns that prospective residents and families should evaluate. Clinical responsiveness and monitoring are inconsistent across shifts: reviewers describe delays in assistance, delayed clinical attention for changes in condition, and wound-care/fall-prevention shortcomings. Medication-administration and reconciliation problems are another notable theme, alongside communication breakdowns between admissions/front-office staff and nursing. Staffing levels appear to vary by time and day, with particular gaps mentioned on weekends and off shifts, which correlates with a subset of complaints about slower response times and limited supervision.

Dining and rehabilitation services receive both praise and critique. Several reviewers enjoyed meals, especially breakfast items, while others described variability in meal taste and texture (including puréed options). Therapy is frequently identified as a strength, but some families encountered limited therapy hours or small gym space that constrained access. Facility condition is similarly mixed: many found rooms clean and comfortable, whereas others pointed to aging infrastructure issues such as HVAC concerns, occasional temperature or plumbing problems, and ongoing flooring updates. There are also isolated but serious concerns about environmental sanitation (including HVAC mold concerns) and lapses in personal-property handling.

Management and communication show positive examples alongside problematic patterns. Admissions and some administrators receive praise for being helpful and involved; several staff members are singled out for going above and beyond. Conversely, families describe gaps in orientation, unclear handoffs, and instances of leadership deflection when clinical concerns arise. Billing and cost transparency are a recurring operational issue, particularly the financial burden after Medicare coverage ends.

Overall, the facility demonstrates clear strengths in therapy, caregiving compassion, and cleanliness in many units, but there are consistent themes that indicate variability in clinical oversight, staffing stability, medication control, and facility maintenance. Prospective residents should arrange a tour, request current staffing and therapy schedules, review medication-safety and wound/fall-prevention protocols, and ask about recent environmental maintenance and billing policies to get a current operational picture before admission.
