    Caring staff, clean facility, recommended

I placed my dad here and have been very pleased: the staff are caring, professional and attentive, the administrator truly goes the extra mile, and the building is clean, pleasantly decorated and family-friendly. Meals are tasty and well presented, rehab helped him get stronger, residents seem happy and comfortable, and parking/convenience make visits easy. I highly recommend this facility - overall excellent with just a couple minor things that could be tightened up.

    Summary of reviews

Overall impression: The reviews present a mixed picture. Many families and residents praise the facility for compassionate caregivers, strong short-term rehabilitation results, and a clean, pleasantly decorated environment with themed common areas and convenient parking. Several accounts highlight administrators and nursing staff who go beyond expectations, a family-friendly atmosphere, and food that is generally well presented and enjoyed by residents.

Care and staffing: Care quality appears uneven. Positive experiences emphasize attentive, professional staff and around-the-clock coverage that supported successful discharges from rehab. At the same time, a number of accounts describe operational challenges consistent with understaffing โ€” delayed responses to clinical events, inconsistent bathing and personal-care schedules, and variable attention to residents' needs. These patterns suggest staffing levels and allocation are key determinants of individual experience.

Clinical management and communication: Some families expressed concern about medication effects (grogginess or increased confusion) and asked questions about medication management practices. There are also repeated concerns about how clinical incidents are handled and communicated to families โ€” including delays in notifying next of kin after events and inconsistent responsiveness from management. These issues point to gaps in incident-response protocols and family communication processes.

Dining and housekeeping: Dining is frequently cited as a strength in taste and presentation, and some residents reported good appetite and satisfaction. However, there is variability in meal consistency and texture for certain items. Housekeeping and dรฉcor are commonly described as clean and pleasant, though other accounts raised sanitation concerns and problems with laundry or personal belongings handling, indicating inconsistent operational performance in those areas.

Facilities and activities: The facility's interiors and themed decor are generally viewed positively, contributing to a home-like feel for many residents. Parking and access are noted as practical conveniences. Residents and families described a warm social environment and staff who help create a comfortable atmosphere.

Management and notable patterns: Reviews reflect a split view of management. Several reviewers praised administrators for being engaged and supportive, while others described delays in responsiveness or a perceived decline in oversight and operational standards over time. There are also isolated but serious allegations related to asset security and concerns following a resident's death that some families felt were not handled transparently. These patterns suggest variability in leadership responsiveness and the need for stronger, consistent policies around incident response, property controls, and family communication.

Recommended focus areas for prospective families and the facility: Prospective families should weigh the facility's strengths in compassionate caregiving, rehab outcomes, and pleasant environment against the potential for operational variability driven by staffing and management practices. For the provider, prioritizing staffing stability, clearer incident-response and family-notification protocols, tighter personal-property controls, and consistent dining and personal-care processes would address the most commonly cited weaknesses and reduce experience variability.

    About Brooke Haven Healthcare

Brooke Haven Healthcare sits at 1410 Kentucky Avenue in West Plains, Missouri, and folks will find it's a family-owned place where seniors can get different kinds of care like skilled nursing, short-term rehab, long-term care, hospice, memory care, and even palliative care, so you see they've got their bases covered whether someone's staying for a little while or needs a lasting home and what folks seem to notice is that residents can pick between private rooms, with eight set aside, or share semi-private rooms if they want that, and everyone can use the comfortable living spaces or go outside when weather allows to visit or relax. The facility's licensed, Medicare and Medicaid certified, and has a dedicated post-acute rehab center-there's a special entrance and nursing station for folks coming in after a hospital stay and looking to recover before heading home, and that center also provides outpatient rehab if someone only needs help for a short time without moving in, and they don't forget about things like physical, occupational, speech, or even respiratory therapies, since the rehab specialists spend a lot of time with residents depending on what's needed. Brooke Haven keeps 24-hour licensed nursing staff on-site too, and they've got a physician who leads care plans and checks on everyone like clockwork, plus a dietitian to give advice about meals and nutrition, and if someone needs medications, the staff works with an outside pharmacy that sorts and packages everything to keep it all straight. People will see hair care and manicures offered at the on-site beauty and barber salon too, plus activities like arts and crafts, group birthday parties, and all kinds of social or exercise programs so residents and families can stay busy and social if they want, which sometimes makes a big difference in how connected folks feel. They offer transportation for appointments and errands that can be set up ahead of time. The laundry's handled for everybody, and there's an active resident action council, letting folks speak up about their wants and needs, which can make things feel a bit more like home. The facility also supports companion care and respite stays if caregivers outside the facility need a break, and they seem to focus on treating everyone as an individual, involving families, and giving attention to community health with a whole range of services under one roof. Holly Osgood is the administrator, so there's a point person leading all the staff and services, and Brooke Haven shares info on Facebook for folks looking to keep in touch with what's happening or find more details about their programs.
