I'm very pleased with the compassionate, professional care my dad receives-staff (including social worker Amber) are welcoming, trustworthy, and attentive, with strong care coordination, regular family updates, and excellent home health, physical, and occupational therapy. The place feels well-run and supportive; my dad is happy.

    Summary of reviews

Overall impression is mixed: reviewers identify several strong clinical and rehabilitative resources alongside recurring operational concerns. Many families and patients praised individual staff members โ€” especially nursing staff, therapists, and a named social worker โ€” for compassionate, attentive care, good therapy outcomes, and proactive help with benefits and care coordination. At the same time, multiple accounts point to systemic issues that appear to affect safety, continuity, and resident experience.

Care quality and clinical safety: The facility offers robust rehabilitative services and home-health support that some families found valuable for recovery and functional improvement. However, a consistent concern is clinical staffing and escalation: reviewers described delays in arranging physician visits, medication errors or delays at discharge, and problems following safe-transfer guidelines. Several accounts referenced falls with resulting injuries and delays in response โ€” indicating potential weaknesses in transfer-safety practices and in how clinical incidents are escalated and managed.

Staff and communication: Staff performance is variable. Positive reports emphasize kind, professional, and advocacy-minded personnel who communicate regularly with families. Negative reports describe understaffing that forces aides to rush tasks, limits opportunity to build relationships, and contributes to inconsistent attention to resident needs. There are also recurring concerns about staff conduct and tone, including incidents of exclusion of family members and confrontational interactions. Communication with external providers and insurers was described as uneven; families noted insurance-related appointment denials and slow coordination that affected access to care.

Dining, activities, and therapies: Reviews provide limited detail on dining and recreational programming. Therapy services (physical and occupational) are frequently singled out as a strength, with positive impressions of staff skill and rehabilitative progress. Some family members observed that therapy or social interactions were occasionally interrupted by administrative tasks, reducing one-on-one engagement.

Facilities and property: Facility condition reports are mixed. Positive comments describe a welcoming atmosphere and pleasant roommate situations. Conversely, some reviews raised sanitation and pest-control concerns in certain areas, along with instances of missing personal items and problems securing residents' belongings. There are also reports of privacy breaches related to personal information handling and documentation practices.

Management and oversight: Management impressions are mixed as well. Several reviewers praised responsive coordinators and described the facility as well-run, while others noted poor oversight of care coordination, insurance navigation, and discharge processes. A serious procedural concern raised by reviewers involves alleged coordinated responses during regulatory interactions, which raises questions about transparency in incident reporting and investigatory processes.

Notable patterns and recommended inquiries: The dominant pattern is one of strong individual caregivers and therapy programs operating within a system strained by staffing shortages, inconsistent processes, and lapses in property/privacy management. Prospective families should ask facility leaders about current staffing ratios, transfer-safety protocols, medication reconciliation and discharge procedures, policies on privacy and personal-property security, pest-control records, and how the facility handles incident reporting and external investigations. Doing so will help clarify whether the facilityโ€™s strengths in therapy and individual caregiver professionalism are supported by reliable, facility-level systems for safety and communication.

    About Tabitha Health Care Services

Tabitha Health Care Services provides senior care all across Nebraska and has been doing so since 1886, running as a nonprofit with a strong focus on helping older adults live with dignity, independence, and joy, and with over 450 employees they manage quite a few locations, with a main campus in Lincoln, regional offices in York and Grand Island, and local campuses in places like Lincoln's Williamsburg neighborhood, Crete, Grand Island, and spots in Nebraska City and York, so they're pretty spread out. Working together with Immanuel, Tabitha offers home health and hospice care in the Omaha area, supporting families through tough times with services like the Journey Hospice House, where people can get pain management and therapies aimed at improving their quality of life, and for those who want to stay at home, they deliver warm lunches and daily safety checks with the Tabitha Meals on Wheels program, as well as skilled nursing care and all sorts of therapy, from physical and occupational to speech-language, plus medical technology support for people who need that extra help.

Seniors here can choose between independent living, assisted living, memory care, long-term communities, rehab, and serious illness support, and there are special programs for people with Alzheimer's or dementia, so folks can get the right support without leaving Nebraska, from very independent places like Sยฒage Living in Lincoln or Prairie Commons in Grand Island-where everything's maintenance free with lots of amenities-to more hands-on communities such as The Gardens in Crete or GracePointe in Lincoln, which offer assisted living with help for everyday needs like bathing, medication, and getting around town. Memory care is available at places like GracePointe and Prairie Commons, and if someone needs frequent help or full-time nursing, Tabitha has traditional and residential-style long-term care at their facilities in Lincoln, Crete, and Grand Island, so there's always somewhere to go as needs change.

Tabitha supports caregivers, too, with respite care so people can take a break, and the medical group practice in Lincoln deals with family medicine, has psychiatrists, psychoanalysts, and providers like Dr. Paul Plessman, and it's all set up inside a hospital for easy access to pharmacy and free parking, plus telehealth visits if getting out isn't easy. Tabitha likes to try new things, so they've brought projects like the Green House Project for unique elder living, and offer a full line of services designed just for seniors, including skilled rehab after hospital stays, bereavement support after loss, counseling, general home care, hospital beds, and basically the whole continuum from independent living all the way through hospice. They've won the Nebraska state quality award, so people can know they've kept their standards high, and at the end of the day, Tabitha stands out mainly because they cover nearly every type of care an older person might need as they age in Nebraska.
