    Mom happy compassionate attentive staff

I'm mostly very pleased with Sunset Rehab & Nursing Center. The nurses, CNAs, therapists and front-desk staff were consistently kind, attentive and helpful; the building and grounds are well kept and activities are engaging. My mom is happy and well cared for and I would recommend Sunset to families seeking compassionate, responsive care.

    Summary of reviews

Overall impression is mixed but distinct: the facility receives frequent praise for the compassion and dedication of direct-care staff and its therapy/rehabilitation teams, paired with repeated operational concerns that center on staffing, cleanliness, and systems. Many accounts highlight nurses, CNAs, therapists (notably physical therapy), admissions staff, maintenance, and case management as strengths โ€” family members commonly describe a welcoming front desk, attentive therapy that produces measurable progress, engaging activities, and pleasant outdoor grounds and common spaces.

Care quality and staffing: Clinical care and bedside manner are often characterized positively, with staff described as caring and willing to go above and beyond. However, there is a persistent pattern of staffing shortages and variability, including night-shift constraints and extended shift lengths. Reviewers link those staffing gaps to delays in responding to resident needs, inconsistent bathing and repositioning schedules, and difficulties getting residents out of bed. These operational shortfalls contribute to pressure-injury risk and other care-timing concerns and are the primary source of negative feedback.

Dining, activities, and therapy: The activity program is consistently cited as a strength โ€” scheduled offerings such as Bingo, social hours, manicure time, and arts and crafts are noted as beneficial to morale. Therapy services receive strong, recurrent praise for improving mobility and health outcomes. Feedback on meals is mixed: some families find the food satisfactory and appreciate special meals, while others describe inconsistencies in meal adequacy or timing.

Facilities and housekeeping: The campus and outdoor areas are often described as attractive and well-maintained, with accessible parking and pleasant landscaping. The building itself is noted to be older with smaller rooms and some shared bathrooms. At the same time, multiple comments describe cleanliness issues in specific areas, odor concerns, and lapses in laundry handling or timely linen changes. These sanitation and laundry-process inconsistencies are a recurring operational theme.

Management and operations: Administrative responsiveness receives both positive and negative notes โ€” some families praise an engaged administrator and helpful case workers, while others point to turnover, communication gaps, billing transparency concerns, and system-level failures (for example, room-assignment mix-ups and lost personal items). Reviewers also mention state-identified deficiencies that they feel have not been fully resolved, which warrants direct inquiry.

Notable patterns and considerations for families: Prospective residents and families should weigh the facility's evident strengths in therapy, day-to-day compassion from individual staff members, and pleasant outdoor environment against repeated operational weaknessesโ€”principally chronic understaffing, inconsistent housekeeping/laundry processes, and communication or management instability. Recommended pre-admission questions include current staffing ratios (day and night), protocols for incontinence and repositioning, laundry and personal-property protections, medication-management practices, recent state inspection findings and corrective actions, and visitation logistics given the facilityโ€™s remote location.

About Sunset Nursing & Rehab Center

Sunset Nursing & Rehab Center sits at 232 Academy Street in Boonville, New York, and holds 120 certified beds in a smoke-free building, so you'll find it offers long-term care, skilled nursing, and post-acute care for folks who need ongoing help or are recovering from surgery or illness, and it also has respite care for short stays and a Secured Memory Care Unit with specialized programming for people who need that extra support. The place is part of a bigger group that includes The McGuire Group, Absolut Care, and Taconic Health Care, so they're not on their own, and you get activities listed on two separate calendars marked A & B and C, which helps with social time if you want to join in. You'll find skilled nursing care around the clock, rehab services like physical, occupational, and speech therapy, and there are services that cover assisted living and short-term rehab if that's what someone needs. The facility has modern features and amenities designed for comfort, but there's no specific list of what those are, and while they do try to keep things homelike, past inspection reports show 22 deficiencies, including serious areas like safety, cleanliness, and pressure ulcer care, as well as nutrition standards and an infection-related issue, so it's something to keep in mind. They've got a nurse turnover rate of 50.9% and staffing averages about 3.64 nurse hours per resident per day, which is useful to know for families looking at care levels. Sunset is a for-profit center and has received the AHCA/NCAL National Quality Award Program - Bronze Award, showing some recognition, and they say they focus on compassionate care, promoting independence, and offering programs tailored to different needs, with a team that works to give residents meaningful things to do every day. The website, www.sunsetnrc.com, has more general information, but they don't provide a detailed list of amenities or unique programs, and there's no detailed outline of services beyond those required for skilled nursing, memory care, post-acute recovery, and rehabilitation. The center's part of the local community, using programs and resources from its broader health network, and it operates with leadership focused on resident care, but it's best to check recent inspection reports and staff levels if you want a clear picture of daily life there.
