    Compassionate rehab restored husband's mobility

I'm very satisfied with my loved one's stay - the staff were consistently compassionate, professional, and communicative, and the rehab team helped my husband walk again. The facility is clean, modern, and welcoming, with helpful front desk/security and coordinators who keep you in the loop; they even support home dialysis. I'd definitely recommend this place for post-acute care.

    Summary of reviews

The review set presents a mixed picture of Four Seasons Nursing & Rehabilitation Center. Positive remarks focus on the facilityโ€™s hospitality functions โ€” welcoming security and front-desk interactions, an attractive lobby, and generally clean, renovated units โ€” and a visible strength in rehabilitation services. Several families and visitors praised physical therapy, occupational therapy and speech services for measurable mobility and functional gains. Multiple comments also highlight compassionate individual caregivers and CNAs, helpful coordinators who provide direct contact information, dialysis support including training for home hemodialysis, and an active recreation program with music, prayer and social activities.

At the same time, there are recurrent operational concerns that prospective families should weigh. Staffing consistency and coverage emerge as a central theme: reviewers describe uneven nurse/CNA availability across shifts and floors. That inconsistency is linked to a range of clinical process gaps cited in the reviews โ€” lapses in medication administration and monitoring, missed or delayed assistance with activities of daily living (including incontinence care and repositioning), and safety-monitoring shortcomings. There are also several notes about infection-control issues and concerns following a residentโ€™s death; in addition, there are allegations of improper medication practices and other regulatory concerns that merit review of state inspection records and facility responses.

Dining and environmental reports are similarly mixed. Many reviewers report timely meal service and satisfying menu items, while others describe small portions, cold meals, or inconsistent food quality. Cleanliness is praised for much of the building โ€” especially public areas and some units โ€” but reviewers also describe sanitation and odor inconsistencies on particular floors and raised pest-control concerns. The physical environment therefore appears variable by unit: some floors are recently renovated and well maintained, while others feel older or less well cared for.

Staff behavior and management communication show wide variance. Numerous accounts describe warm, helpful, and communicative staff members, effective case managers, and prompt resolution of problems. Conversely, other accounts document poor family communication, unreturned calls, abrupt or unhelpful social-work interactions, rushed or poorly explained discharges, and limited escalation when families seek updates. That unevenness suggests variability in leadership presence or unit-level management practices.

Notable patterns for decision-making: the facility has evident strengths in rehabilitation programming, certain clinical staff, and hospitality functions, but it also demonstrates variability in core nursing processes, cleanliness by unit, and family communication. Prospective residents and families should conduct repeated in-person visits at different times of day and on multiple floors, ask for current staffing ratios and weekend coverage, review recent state inspection and infection-control reports, request written medication-administration and discharge procedures, and identify named contacts for clinical updates. When touring, ask about repositioning protocols, incontinence-care procedures, meal schedules/portions, pest control, and how care concerns are escalated and resolved.

About Four Seasons Nursing & Rehabilitation Center

Four Seasons Nursing & Rehabilitation Center sits at 1555 Rockaway Parkway in Brooklyn, NY and takes care of people needing skilled nursing and rehabilitation. People come here for help with stroke, heart problems, wound care, and breathing support, and the place is led by a board-certified pulmonologist who oversees ventilator and respiratory services. The ventilator unit, which has run for over 20 years, is New York State Certified and has a strong record for helping people come off ventilators. Staff help with all kinds of medical needs, using individualized care plans. On each floor, there are residential lounges, and rooms are single or double, each with a private bathroom and shower, along with individual climate controls, and they've been recently updated.

There's always a licensed nurse close by, and there's a high nurse-to-resident ratio, so people usually don't have to wait long for help. Physical, occupational, and speech therapists work with residents, focusing on recovery from surgeries or illnesses, and offering chronic disease management, pain management, medication checks, and post-operative care. Most residents use wheelchairs and need full care with daily tasks. Group and individual activities are part of daily life, along with health classes, religious services, holiday parties, and a beauty parlor/barber service on site to help with grooming.

Residents can join special events, and the center offers support for a variety of lifestyle needs. Meals come from New York State registered dietitians and can be eaten in the dining room, which tries to offer culturally diverse and gourmet options, but sometimes food comes out cold, which people have criticized. Wi-Fi and phone service are free, and every month, the center updates its online provider directory. English is spoken by staff, but it's not clear if other languages are supported. Four Seasons is currently not taking new patients.
