    Friendly staff, clean facility, peace

I'm very happy with my choice - the staff (Director Amy included) are friendly, professional and caring, the building is clean and well-kept, and residents are social and engaged. Three meals and snacks, laundry, housekeeping, med management, on-site doctor, PT/OT, haircuts, grocery and bus trips, plus a lively activity calendar give me real peace of mind; they also assisted with veterans benefits and burial-planning suggestions. Rooms are available now and it's about 20 minutes to Lima - I would recommend this community.

    Summary of reviews

Horizons at Canandaigua presents a mixed but distinct profile. Many reviewers praise its clinical and daily-living supports: there is an on-site physician, available physical and occupational therapy, medication management, and nursing-staff communication that families described as reassuring. The community offers three meals plus snacks, diabetic-appropriate menu modifications, on-site haircare, housekeeping and laundry services, and assistance with benefits and financial planning. For residents seeking social engagement, the activity calendar, scheduled outings and bus trips, and a generally social atmosphere were frequently cited as strengths.

Staffing and care quality evoke divergent impressions. Numerous families describe caring, consistent staff and administrators who provide quick responses and continuity of care; those accounts emphasize a family-like culture and attentive routine care. However, a number of reviews raise facility-level concerns about staffing instability and variability in staff conduct and communication tone. Several reviewers described abrupt discharges or refusal to readmit residents after hospital stays, which points to potential gaps in admissions/readmission policies and in continuity of care for residents with increasing acuity. Reviewers also noted falls and mobility decline in some residents, suggesting prospective families should confirm the community’s suitability and protocols for higher-acuity needs.

Dining and activities are generally seen as positive offerings but with inconsistency. The community provides meal service that accommodates dietary restrictions, and some reviewers praised food variety and extras such as wine availability. Contrastingly, others reported cold meals, limited menu variety, and that dietary support could be improved. The activities program and transportation for outings are strengths, though free or on-demand transportation options appear limited.

Facility condition and management impressions are mixed. Many reviewers describe a clean, well-kept building and pleasant common areas; supportive administrative staff and a director who goes above and beyond were singled out positively. At the same time, there are cleanliness issues and pest-control concerns reported in some areas, and complaints about room cleanliness and phone responsiveness. Management consistency is another dividing line: while some families received effective, communicative care meetings and continuity of staff, others described unkept promises, slow call responses, and disputed eviction or move-out practices. Pricing was described as comparable by some and as high relative to the level of services and room types by others; the community provides rooms rather than apartment-style residences, which may affect perceived value.

Notable patterns and practical recommendations: experiences appear polarized — many residents and families are satisfied and feel safe and engaged, while a subset of families reported significant administrative and operational concerns. Prospective residents should schedule an in-person tour, verify written policies on admissions/readmission and discharge, ask about staffing ratios and turnover, review pest-control and cleaning schedules, confirm how dietary needs and meal service temperature/variety are handled, and clarify transportation availability and any additional fees. For individuals with higher medical needs, confirm the community’s capability and willingness to manage escalating care needs and hospital readmissions before committing.

    About Horizons at Canandaigua

Horizons at Canandaigua sits along State Route 21 and offers supportive housing, senior living, and a full set of mental health services for people in the Finger Lakes region who need a little extra support, and with 76 licensed assisted living beds, some private rooms, and some semi-private, there's a bit of choice depending on someone's needs, though studio apartments are also set aside for folks who like a quieter space, and the place does allow some pets and even has a resident cat for company if someone misses having animals around or just likes hearing a purr in the background now and then. People who choose Horizons get access to a wide range of care options, including memory care for those with Alzheimer's or dementia and ongoing monitoring for any health changes, and there are home health aides and registered nurses on staff, with all kinds of help available for daily activities like bathing, dressing, or managing medications, and care staff work closely with residents, their families, doctors, and local agencies to make sure everyone has a plan that meets their needs. The community takes part in New York's Assisted Living Program (ALP), which means Medicaid can cover the cost for eligible people who'd prefer assisted living instead of a nursing home, and this place tries to make things easier by offering all-inclusive pricing, a secure stay guarantee to help with planning, and a bunch of services already built in like laundry, linen cleaning, housekeeping, barber and haircuts, in-house pharmacy, and even religious services for folks who want them. There's a focus on social life, too-regular activities, crafts, outings, exercise, and meals planned by a Registered Dietitian, and the meals always try to use quality ingredients while taking resident's health and taste into account, and people get a chance to relax on outdoor patios or in gardens, chat in community areas, use Wi-Fi, or make use of free transportation to get around for appointments or trips, and everyone can expect 24-hour supervision from trained staff. Horizons is pet-friendly, though there are some rules, and offsite activities are available so life doesn't have to feel closed in, while community dining, on-site parking, daily housekeeping, medication management, and personal care help round out the daily rhythms. Folks who want to stay active can find independent living options, while others needing more help or a break after an illness can arrange for long-term or respite stays. The community's not flashy but it's steady, with committed support, comfort, and a place for people who want their needs met in a familiar way.
