    Rochester Presbyterian Home (Creekstone) sits at 20 Crossing Creek Drive in Fairport, NY, and has a quiet, home-like feel with four ranch-style residences, each with 14 private bedrooms and bathrooms, shared living rooms, open kitchens, and dining areas where Elders gather and eat together, and there are screened front porches and a secure courtyard with raised gardens, walking paths, and room for sunshine, where folks can relax or spend time with friends, family, and even pets like dogs, cats, birds, or fish, because pets are welcome and sometimes visiting school kids drop by to brighten things up. This is a memory care community with secured supportive housing for people with memory loss, early-to-mid stage dementia, or Alzheimer's, and it has an enhanced care area for persons with more advanced dementia who need extra staff support. The place does offer respite care at all residences, which means Elders can stay from two to six weeks after an illness or when caregivers need a break, or they'll even come out to do adult in-home respite care in the Fairport area.

    The staff get special training in cultural competency, diversity, equity, and inclusion, and many speak Spanish, with Spanish translations on the website, so all Elders can feel comfortable here. Creekstone follows the Eden Alternative® philosophy, which means they work hard to make things feel like home, encourage Elders to help out and make choices about daily life, keep the days lively with activities, pets, and plants, and focus on removing loneliness, helplessness, and boredom for everyone living there. Elders join an "Eden family" with a focus on joy, dignity, and growth, so residents are cared for as individuals, not just as patients, and family or friends are always welcome to drop by or spend time together.

    In each residence, there are Residence Guides, like the administrator and two nurse/care-managers, who help plan and oversee health care, and then there are Shahbazim, the main care staff, who assist with personal care, meals, house chores, and connecting Elders to daily life, and Engagement Guides plan activities to help with physical, social, and spiritual well-being, and nurses are around 24 hours a day for extra safety. People can personalize their bedrooms with favorite chairs or decorations, and the community encourages independence and keeps security up with secure entry and emergency response systems throughout the homes. There are also several amenities, including a residents lounge, sunrooms, private parking, and spaces for hobbies or social gatherings, and Creekstone offers three meals a day in a communal setting with special diets accommodated. They partner with outside agencies to provide hospice care when needed, and the model of care tries to flatten hierarchy, letting staff closest with Elders make key decisions while supporting independence.
