    Compassionate staff got mom home

I had a very positive experience - the therapists and nursing staff were compassionate and competent and got my mom walking and home quickly. The building was clean, meals and laundry were good, and daily activities (Bingo, Po-Ke-No, men's group) kept residents engaged. Social work (Katrina) and the team communicated constantly and handled family-focused discharge planning with care.

    Summary of reviews

The reviews for Crest Manor Living & Rehab Center present a mixture of clearly stated strengths and recurring operational weaknesses. The facility's rehabilitation services (physical and occupational therapy) receive consistent praise for clinical effectiveness and helping residents return home. Several reviewers singled out individual clinicians, therapy teams, and physician assistants as competent and responsive. Likewise, many families described unit-level staff—particularly certain CNAs and aides—and the social work team as compassionate, helpful, and instrumental in discharge planning and insurance navigation.

At the same time, a persistent theme is understaffing and the operational consequences that follow. Reviewers frequently describe long waits for assistance with toileting and hygiene, slow call-bell responses, and limited night supervision. Those staffing limitations are linked to sanitation concerns in common areas and resident rooms, inconsistent bathing schedules and incontinence-care delays, and observations of maintenance issues. Management and ownership instability, including staff turnover and reports of a decline in culture after ownership changes, is described as exacerbating these problems.

Dining and housekeeping impressions are mixed. Some families praised the laundry, food-delivery logistics, and periods of good meals; others described inconsistent food quality, limited menu choices, and issues with meal presentation or incorrect tray deliveries. The activities program is a relative strength: reviewers noted regular, varied offerings (bingo, groups, card games and afternoon programming) that support engagement and social connection for residents.

Facility condition and safety are additional areas of concern. Multiple reviewers cited aging infrastructure—plumbing problems, elevators and heat failures, stuck doors/drawers, peeling surfaces—and general deferred maintenance that affects resident comfort and access. There are also recurring statements about roommate compatibility and behavioral-management processes that can lead to a stressful environment for some residents. A number of reviewers raised serious clinical safety concerns (medication-management errors, feeding-safety lapses, wound-care issues) and filed complaints with regulators; these individual allegations merit careful follow-up by prospective families.

In summary, Crest Manor appears to offer strong rehabilitative care and several dedicated frontline staff and social work professionals who can positively influence outcomes. However, persistent operational weaknesses—notably staffing shortages, inconsistent clinical documentation and medication practices, maintenance deficits, variable dining quality, and uneven family communication—are important considerations. Prospective residents and families should weigh the facility's therapy and social-work strengths against the reported limitations in staffing, maintenance, and clinical oversight; when possible, arrange detailed, current conversations with unit leadership, observe staff responsiveness in person, and confirm policies on staffing levels, medication safety, roommate placement, and escalation/complaint procedures before placement.

About Crest Manor Living & Rehab Center

Crest Manor Living & Rehab Center sits on Pittsford-Palmyra Road in Fairport, New York, and has served the community since 1955, though the business was incorporated in 2005. The facility works as a privately held corporation under the supervision of Mr. A. John Bartholomew II, President and CEO. Crest Manor runs as a smaller, skilled nursing facility, with a staff of about 51 to 200 employees, and provides a range of care options like long-term residential care, short-term and subacute rehab, respite care, hospice or comfort care, and observation and clinical care. They specialize in nursing home care, rehabilitation, and senior living, focusing on each person's needs with a family-like approach where everyone knows each other by name, and you can see the staff and residents interacting like old friends who've known one another for years. There's an emphasis on warmth and respect, and people say the home-like setting helps residents feel comfortable and supported, especially since the center is on the smaller side, making it easier for staff to offer individualized attention and really keep track of everybody's well-being.

Crest Manor offers skilled nursing services, rehabilitation, as well as specialized care for those needing help for a shorter period, like after a hospital stay, or for those seeking comfort in their final stage of life through hospice. The center has amenities and activities common in long-term care, with a dedicated Activities team organizing things like birthday parties and community events to help residents stay engaged and happy. There's a broad spectrum of activities and the staff take care to make each person feel like part of the Crest Manor family, including opportunities for family members to stay involved through programs and tours. Care is tailored, whether someone's there for long-term living, short-term rehabilitation, hospice care, or just needs a place for respite, and there's particular attention paid to vulnerable community members who might need more help along the way.

Crest Manor isn't BBB accredited, but it holds an A+ rating from the Better Business Bureau and is a trusted name for care in the Fairport and Rochester area. Licensing, bonding, and registration may apply, and anyone who needs more details about these requirements or wants to see pictures of the center can check the photo gallery or other resources on the center's website. The staff advocate for residents' needs, working as a united team under a clear mission: to provide the highest level of personalized, compassionate care in a warm, welcoming environment.
