    Spotless, attentive, compassionate memory care

My mom has been here five years and I'm so grateful - the facility is spotless, bright and home-like with spacious rooms, a pleasant dining area and courtyard. The staff are kind, attentive and truly go above and beyond in memory care and daily nursing, catch issues quickly, keep our family informed, and eased our worries. Activities are engaging, the food is good, and overall the care is excellent with only minor room for improvement.

    Summary of reviews

The reviews present a mixed but instructive picture of Wayne County Nursing Home and Rehabilitation Center and Outpatient Therapy. Many families describe strong, compassionate caregiving: nursing staff who are attentive, personable, and willing to go beyond basic duties; activity staff who create engagement and accommodate celebrations; and a generally home-like environment with clean, modern spaces, a pleasant dining area, and an accessible courtyard. Several long-term residents and families attribute peace of mind to the staff’s approachability and individualized attention, and the facility’s memory-care programming and dementia accommodations receive positive mention.

Operationally, reviewers highlight variability in experience. While clinical responsiveness is praised in many accounts, others describe occasions when care quality was inconsistent—notably across different shifts or after management changes. Concerns expressed point to gaps in clinical oversight and medication management, as well as issues with skin and injury surveillance. Some families also describe inconsistent assistance with activities of daily living and mealtimes, along with episodes that suggest delays in responding to resident needs.

Communication and administrative processes are another recurring theme. Positive comments note informative updates and staff who call residents by name, but contrasting accounts describe difficulty reaching reception or rehab staff by phone and frustration with coordination. These communication shortfalls appear linked in some cases to staffing instability and changes in management, which reviewers say have affected morale and the continuity of care on certain units.

For prospective residents and families, the facility shows clear strengths in staff warmth, activity programming, a clean and pleasant physical environment, and memory-care offerings. At the same time, variability in staffing, clinical oversight, responsiveness, and administrative communication are important operational weaknesses to explore during a tour. Recommended questions include current staffing levels and turnover, nurse oversight and medication-review processes, wound- and skin-care protocols, how call-response times are measured and tracked, and examples of how management addresses family communication and reception/rehab coordination. Observing a mealtime and speaking with unit-level staff can help assess consistency of day-to-day care.
