    Attentive compassionate staff and services

I'm very satisfied with the care my loved one receives - staff are attentive, compassionate, and management and case managers are responsive. The facility is clean and well-run with strong nursing and rehab services (on-site PT/OT, doctors, 24-hour respiratory therapy and specialized units), diverse meals including Korean and Indian options, and engaging activities. I'm grateful for the caring team and would recommend this facility.

    Summary of reviews

The available reviews present a strongly polarized picture of A. Holly Patterson Extended Care. Many families and former residents describe effective rehabilitation services, attentive bedside care from particular nurses and therapists, and a well-equipped therapy department that supported measurable functional gains. Positive comments emphasize on-site medical capabilities (including physicians and 24-hour respiratory support), specialized clinical units, a variety of meal choices with dietitian involvement, organized recreational programming, and culturally tailored services such as a Korean unit. Several reviewers also noted improvements under recent leadership and instances of responsive management and good family communication.

At the same time, a substantial set of concerns recurs across reviews and suggests operational inconsistency. Staffing levels and coverage are described as variable, with corresponding delays in attending to resident needs and call-bell response times. Families raised issues with medication timing and pain management, and some flagged gaps in clinical oversight and physician continuity. Safety practices related to transfers and fall prevention were questioned in several accounts. There are also repeated references to sanitation and odor concerns in particular units and to pest-control concerns in a few reports, which indicate uneven housekeeping or environmental maintenance in parts of the facility.

Communication and administrative processes are another mixed area. Some reviewers found social work and admissions staff helpful, while others described slow or unresponsive social work communication, inconsistent family updates, and difficulties obtaining timely documentation. Complaints about lost or mishandled personal items, billing discrepancies, and therapy charges without clear documentation point to weaknesses in property controls and record-keeping. A small number of serious allegationsโ€”including theft and problematic staff conductโ€”appear in the record; these merit direct inquiry and, where appropriate, escalation to oversight channels.

Dining and activities receive both praise and criticism. Positive accounts highlight diverse menus, good food-service infrastructure, and engaging recreation programs and religious services. Negative feedback centers on meal-delivery timing, food-temperature control, and limited activity engagement on some units. The facility's physical plant and amenities are described positively by many reviewers (clean common areas, large gym, well-maintained grounds), while others report unit-level maintenance needs and inconsistent cleanliness.

For prospective residents and families: this facility demonstrates capacity for high-quality rehabilitative care and has distinct strengths in therapy, on-site medical services, and certain specialty units. However, persistent patterns of inconsistent staffing, communication gaps, environmental maintenance variability, and administrative control issues suggest the need for careful due diligence. Recommended actions before placement include: asking for current staffing ratios by shift and unit, requesting recent quality- and infection-control reports, clarifying medication administration and pain-management protocols, documenting how personal property is protected, confirming physician coverage and therapy documentation practices, and discussing visitation and grievance procedures. Visiting in person, observing specific units at different times of day, and obtaining written answers to these operational questions will help families assess whether the facilityโ€™s strengths align with a prospective residentโ€™s clinical and safety needs.

About A. Holly Patterson Extended Care

    A. Holly Patterson Extended Care sits on Jerusalem Avenue in Uniondale, New York, and provides long-term medical care for seniors with serious health needs who can't be looked after at home but don't need to be in a hospital, and it's got medical staff on duty at all hours, so someone's always there to help. This place is a Skilled Nursing Facility that offers skilled medical help with a full range of therapies like physical, occupational, and speech therapy, plus pain management, rehabilitation, and even round-the-clock respiratory care, which means they have nurses and respiratory specialists always around under a doctor's guidance if someone needs breathing support. The facility works with Nassau University Medical Center and has services from Loving Care Health Management. They have private and semi-private rooms, and everyone gets three cooked meals every day, plus housekeeping and laundry are taken care of.

They've set up specialty divisions for Korean, Chinese, and Indian residents, with special diets and cultural programs so people find food and activities that feel familiar, and there are specific contact numbers for those divisions too. There's a 20-bed unit that focuses on care for residents with HIV/AIDS, led by an infectious diseases specialist, and in addition to hospice and end-of-life care, they've got on-site hemodialysis available for those who need it. The medical services stand out because they offer on-site clinics where residents get to see specialists in things like rehabilitation, radiology, psychiatric and psychological care, dental, eye care, podiatry, heart conditions, and neurological testing, so most needs get handled without having to leave the building.

There's a big focus on keeping people active and entertained, with a team of recreation therapists, including music and art therapists, planning activities, field trips, cultural events, holiday celebrations, adapted sports, and even game rooms with things like video games, pool tables, a big TV, and basketball, all watched over by staff. Social workers help with planning for discharge and offer support, and there's an ombudsman to help with any concerns or advocacy needs. Religious services are available through interfaith chaplaincy, while beauty and barber shop services and the volunteer-run Holly Shop give a bit of normalcy and comfort, and residents can use on-site bank services too.

They have a wide range of staff-registered nurses, licensed practical nurses, board-certified doctors, social workers, dieticians, rehabilitation and respiratory therapists, activity specialists, and pastoral care-so different needs are covered from medical care to daily living help and companionship. This place is recognized nationally as a model for skilled nursing care in a holistic environment, meaning they try to look after people's health, mind, and social well-being, not just the medical side of things. There's plenty of parking for visitors, and the facility accepts many forms of payment, including Medicare, Medicaid, private insurance, HMO, Medicaid Pending, and private pay.
