    Compassionate staff helped mom recover

I'm very satisfied with the care my mom received. The nurses and therapy team (PT/OT/ST-Kayla and Lynnette stood out) were compassionate, competent, and treated her like family, helping her regain independence; staff went above and beyond with personalized attention, snacks and brain puzzles. The facility is clean, nicely remodeled, meals and social company are enjoyable, and I'd recommend their palliative care.

    Summary of reviews

Comprehensive Rehab and Nursing Center at Williamsville elicits strongly mixed impressions. A clear strength is the facility’s rehabilitation capability: physical, occupational and speech therapy teams receive frequent praise for achieving meaningful functional gains and preparing residents for discharge. Several families also described compassionate hospice and palliative care, and individual caregivers (named by some families) who provided attentive, person-centered assistance. Portions of the facility have been remodeled and some common areas and front-desk staff were described as welcoming, which supports positive experiences for some residents.

At the same time, numerous reviews describe persistent operational shortcomings that affect basic nursing care and resident safety. A common pattern is inconsistent staffing—including heavy reliance on agency personnel—resulting in delayed responses to call bells, missed or late medication administrations, and variable frequency of bathing and toileting assistance. Reviewers cited lapses in routine clinical monitoring (for example, inconsistent vital-sign checks and oxygen management) and concerns about continuity and accuracy of discharge paperwork and prescriptions.

Dining and daily care present another clear area of variability. Families reported unreliable meal delivery, late or cold trays, and irregular feeding assistance that in some cases coincided with weight loss concerns. Activities and therapy programming are present and effective for many residents (puzzles, bingo, social activities), but several reviewers asked for more consistent engagement options and closer alignment of therapy and nursing schedules.

Facility condition and environmental management are recurring themes. While parts of the building have been updated, many reviews raise sanitation concerns, odor concerns in specific wings, maintenance issues (broken furniture, HVAC concerns, roof leaks), and pest-control worries. Security and inventory-control processes also appear inconsistent: multiple reports describe missing personal items and delays in locating belongings, which families experienced as a serious operational weakness.

Management and communication patterns are uneven. Some families praised front-desk staff and found admissions/tours informative, but others described difficulty reaching administrators, unanswered phones, voicemail problems, and slow follow-up on complaints. Reviewers also reported billing errors, discharge-process mismanagement, and restrictive or inconsistently applied visitation policies. A number of reviews referenced filing complaints with regulators or ombudsmen, indicating that concerns have sometimes escalated beyond internal resolution.

Taken together, the reviews portray a facility with notable strengths in rehabilitative care and with individual staff members who provide excellent, compassionate attention. However, these positives coexist with recurring, facility-level weaknesses in staffing reliability, basic nursing and hygiene practices, meal-service continuity, environmental maintenance, clinical monitoring, and administrative responsiveness. Families considering this facility should verify current staffing patterns, infection-control practices, medication-management processes, and security protocols, and seek direct confirmation of the facility’s responses to any regulatory actions or complaints prior to placement.

About Williamsville Rehab and Nursing Center

Comprehensive Rehab & Nursing Center at Williamsville sits in a scenic part of Williamsville, NY, surrounded by a state park and a creek, where daily life includes help from caring nursing staff who're available at any hour for things like bathing, grooming, and toileting, and folks here have on-site rides for shopping trips and appointments so nobody's stuck without a way to get places and the place is part of the Catholic health system, which brings some roots and traditions to the care but you'll also find a full range of activities like pet visits, music, art therapy, and group trips outdoors for people who like staying active or being social, and for those needing extra support, there are assisted living and Alzheimer's care units, along with long-term skilled nursing and a rehab center that helps people regain things like mobility or strength after an illness or injury. The facility can handle different care needs under one roof-including Independent Living, Assisted Living, Memory Care, Home Care, Adult Day Services, Long-Term Care, and Medicare-Certified Home Health and Hospice. With 142 certified beds and about 116 residents staying most days, rooms come in different types depending on what's required, and nurses and therapists work together to build plans that fit each resident, though they've had some issues-inspectors counted 41 deficiencies, including one infection control problem, a deficiency on care planning, one about residents' rights, and another about quality checks, along with 3 infection-related citations, and the latest Medicare inspection from December 6, 2024, shows there's work to be done in preventing infections and making sure care plans get followed. The nurse turnover here is high at 76.5% compared to the New York average of 41.2%, so people may notice new faces more often than at other places, and while nurse hours per resident are tracked, the exact details aren't available, but residents can expect both long-term and short-term stays, a secure unit for those who need extra supervision, and a general focus on respectful, personalized treatment shaped by resident needs. The facility runs as a for-profit limited liability company, and it's tied to Ephram Lahasky, who has an ownership stake, but manager or employee details aren't made public. At the end of the day, the place tries to build a home-like setting where staff listen to residents, follow personal preferences, and the therapists try to use the right methods to help each person as best as they can, although the inspection history shows some areas still need attention.
