    Spotless facility, excellent caring staff

I stayed here for rehab and was very pleased - the facility was spotless, meals were excellent, and nursing and therapy were attentive and effective. Staff treated me like family with respectful, compassionate care and clear communication; I'd happily recommend this place for quality, dignified short- or long-term care.

    Summary of reviews

Overall impression: Reviewers convey a predominantly positive experience with Elizabethtown Healthcare & Rehabilitation Center, with frequent praise for the direct-care workforce and rehabilitation program. Nursing staff, certified nursing assistants, therapists, and ancillary teams (housekeeping, dietary, transport) are repeatedly described as compassionate, attentive, and supportive. Many families highlighted effective rehabilitation outcomes, strong wound-care skills, proactive discharge planning, and sensitive end-of-life support. The facilityโ€™s housekeeping and dining services also receive consistent commendation for cleanliness and meal quality, contributing to a family-like, welcoming environment.

Care quality and clinical operations: Clinical care is frequently characterized as diligent and caring, with specific strengths in PT/OT engagement, wound care, and hospice coordination. At the same time, reviews identify operational gaps that merit attention. Several accounts suggest weaknesses in clinical monitoring and incident response, along with discontinuities in medication management and physician oversight. These concerns point to potential variability in how the facility handles higher-acuity needs, emergent equipment availability, and medication continuity. Prospective families should ask about current protocols for monitoring, emergency equipment checks, medication reconciliation, and on-call physician coverage.

Staff, communication, and culture: The staff culture is a clear assetโ€”many reviewers describe respectful, warm interactions and a strong sense of community. Social work and administrative personnel are noted for being responsive and helpful. However, there are intermittent comments about inconsistent professionalism and communication tone from some staff members. A small number of reviews raise questions about leadership responsiveness to critical feedback and transparency around complaints or negative experiences. These observations suggest variability in interpersonal interactions rather than a uniformly negative management pattern.

Dining, activities, and environment: Dining services are widely praised for variety and quality, with several reviewers calling the food โ€˜โ€˜restaurant-quality.โ€™โ€™ Housekeeping is appreciated for keeping rooms and common areas clean. The activity program, including games and spiritual offerings, is viewed as engaging and supportive of resident well-being. Facility condition is generally acceptable but described by some as dated; renovation needs are noted alongside positive comments about cleanliness. Operational issues identified include entrance smoking control and limited in-room communication equipment, which may affect convenience and perceived safety.

Management and notable patterns: The dominant pattern across reviews is strong direct-care performance and effective rehabilitation outcomes, balanced against occasional operational and clinical-process weaknesses. Praise centers on front-line staff and therapy teams; concerns cluster around systems of clinical oversight, medication continuity, equipment availability, and certain aspects of staff conduct and feedback handling. For families considering this facility, recommended due diligence includes asking about current staffing levels, protocols for high-acuity monitoring (including enteral/respiratory equipment availability), medication reconciliation practices, smoke/entrance policies, plans for infrastructure upgrades, and how the facility handles and documents safety incidents and family complaints.

About Elizabethtown Healthcare & Rehabilitation Center

Elizabethtown Healthcare & Rehabilitation Center sits at 208 Mercer Mill Road in Elizabethtown, NC, just a little ways from the town center, and it's been run by Long Term Care Management Services LLC since February 2020, so you've got managers like Lori Barrow and Joseph V Calcutt who've been there for a while overseeing things, and this is a for-profit facility that's part of Liberty Senior Living, so you'll find a focus on skilled nursing care and a full range of services going from short-term rehabilitation after hospital stays all the way to long-term living and hospice care for folks needing comfort at the end of life. The building has 94 certified beds, and the average census is about 85 residents, so it stays pretty full, though right now they aren't taking new patients but they welcome questions about future openings, and you'll see that English is the language spoken by staff.

They've got round-the-clock nursing care with registered nurses, LPNs, and certified nursing assistants, and the nurse turnover rate is at 39.3%, which is higher than you might hope, so staff changes are common, but every resident gets an average of 3.26 nursing hours per day, according to their recent numbers. There's a clinical team that makes person-centered care plans to help residents regain skills and try to avoid hospital readmissions-plus they offer full-time physician coverage and specialists for things like wound care, podiatry, and geriatric psychiatry. Rehabilitation is a big part of what goes on here, with physical, occupational, and speech therapy happening in a therapy gym outfitted with an Activities of Daily Living Suite to practice everyday skills, and therapy can help after surgeries, strokes, or other health events, with extras like electrical stimulation to help joint movement and muscle strength.

The care at Elizabethtown Healthcare & Rehabilitation Center covers a lot of needs, from help with daily tasks like bathing and dressing to medication management and reminders, IV therapy, wound care, cardiopulmonary support, incontinence management, fall prevention, and supportive services like housekeeping, laundry, and transportation for appointments or errands. Rooms come in private or semi-private setups with their own cable and complimentary WiFi, and there are in-room dining options for convenience. Residents also can spend time in community spaces like outdoor patios, quiet lounges, and activity areas for social gatherings, plus there's a full-service salon and programs designed to keep both body and mind active.

While the facility aims for comfort and a safe environment, it's important to note that their inspection report from September 26, 2024, shows two deficiencies, one about not fully honoring residents' rights to a safe, clean, homelike environment, and another about not having complete policies in place to help prevent abuse, neglect, or theft, so they've got work to do there. Still, Elizabethtown Healthcare & Rehabilitation Center offers a place where folks with complex nursing or rehabilitation needs can find a structured, person-focused approach, with features like outdoor spaces, indoor lounges, and programs for ongoing wellness.
