    Caring clean professional pet-friendly facility

I placed my mom at Grantsbrook and am very grateful-staff are consistently friendly, caring, and attentive, with excellent nurses, aides and a helpful therapy team. The facility is clean, the food is good, the outdoor garden and home-like atmosphere made her stay pleasant (and pet-friendly), and admissions/discharge were smooth. I'd recommend Grantsbrook to families seeking professional, compassionate care.

    Summary of reviews

Reviewer feedback for Grantsbrook Rehabilitation & Nursing Center is sharply mixed, with clear strengths in therapy and certain caregiving relationships but repeated operational weaknesses that affect day-to-day resident experience. Many families and residents praised individual staff members, particularly rehabilitative therapists and several nurses and aides who provided attentive, compassionate support. Positive accounts highlight successful short-term rehab outcomes, a welcoming, familyโ€‘like atmosphere, pleasant common areas and an outdoor garden, and an accommodating pet policy. The facilityโ€™s Medicare 5-star rating and reports of thorough therapy care align with these positive impressions.

At the same time, a substantial number of reviews describe systemic problems that appear to be operational rather than isolated. Staffing inconsistencies and turnover were a frequent theme; families reported delays in responding to call systems, missed personal-care activities (bathing, repositioning), and inconsistent attention to continence care. Those staffing gaps are linked in several accounts to delayed medication administration and pharmacy logistics problems. While some individual caregivers were singled out for praise, reviewers commonly characterized the overall staffing picture as unreliable.

Dining and nutrition experiences vary widely. Some residents enjoyed meals and found food satisfying, while others described cold or unidentifiable dishes and raised concerns about feeding safety and portion/texture management. Housekeeping and sanitation controls also showed variability: reviewers noted cleanliness in some common areas but cited inconsistent room and bathroom maintenance, odor concerns in shared spaces, and pest-control issues in certain units. These facility-maintenance shortcomings are compounded by reports that the building is dated, rooms can be small and shared, and the physical plant would benefit from updates.

Management and communication emerged as another recurrent area of concern. Families reported difficulty obtaining timely responses from administrators and social-work staff, and some described unsatisfactory complaint-resolution experiences and perceived lapses in professional conduct during interactions with leadership. A few reviews raised serious individual claims, including documentation and privacy issues; these were characterized here as allegations and suggest that prospective families should ask for clarification about recordโ€‘keeping and privacy policies. Additionally, some reviewers noted that the facility may not be equipped to meet complex hospice or intensive endโ€‘ofโ€‘life needs.

In summary, Grantsbrook offers clear strengths in rehabilitative therapy and in individual staff who create a friendly, home-like environment for residents. However, persistent operational weaknesses โ€” notably inconsistent staffing, housekeeping and sanitation controls, dining variability, medication and discharge coordination gaps, and management/communication shortfalls โ€” have produced uneven experiences. Prospective residents and families should plan an in-person visit, inspect room cleanliness and shared facilities, ask specific questions about staffing ratios, medication and feeding protocols, hospice capability, pest-control measures, and the facilityโ€™s process for handling complaints and documentation/privacy concerns before making placement decisions.

About Grantsbrook Rehabilitation & Nursing Center

Grantsbrook Rehabilitation & Nursing Center, also called Grantsbrook Nursing and Rehabilitation Center, sits at 290 Keel Rd in Grantsboro, NC, and has 96 skilled nursing beds for long-term and short-term care. The center's an older facility but keeps a clean, orderly environment with friendly aides who help make the place welcoming. Grantsbrook works with epidemiology experts from universities and the community to follow good safety standards, using cold plasma therapy, regular electrostatic cleaning, HEPA air scrubbers, needlepoint bipolar ionization, and MERV 13 filters, along with mask requirements and health screenings when needed. The staff follow CDC and state guidelines to help keep everyone safe. There's a well-kept restaurant-style dining room with personal table service, customized meal plans for special diets, and meals provided each day. Residents can visit accessible courtyards, enjoy lounge and activity areas, and take part in daily events, offsite activities, and devotional programs.

The facility has a beauty salon for hair and nail services, on-site dental care, and personal services like daily housekeeping, laundry, trash removal, and weekly deep cleaning. People can stop by nourishment stations or the resident store as needed. Living spaces include both semi-private and private pay rooms. Residents have assistance with daily living activities, personalized care plans built by an interdisciplinary care team, and incontinence care if required. The staff delivers care that tries to support each person's dignity, independence, and interests, using a philosophy focused on respect, kindness, and well-being. Nursing care, non-ambulatory care, diabetic support, IV therapy, wound care, and specialized services for heart, lung, stroke, or other chronic conditions are provided, with licensed nurses and therapy staff available at all hours. The therapy gym has advanced equipment for rehab after surgery or hospital stays, and the center works hard to promote healing and help prevent re-hospitalizations.

Amenities include cable, secured Wi-Fi, and telephone service in the price, and translation is available for non-English speakers since some staff speak other languages. The building has the newest access control systems to help keep the area secure. There's a focus on celebrating residents' accomplishments, fostering community, and trying to exceed expectations when possible. Grantsbrook maintains a 5-star quality measures rating from the Centers for Medicare & Medicaid Services. At this time, they aren't taking new patients, and interested families need to call about openings. The administrator is Jim Burrell.
