    Professional, attentive staff; excellent rehabilitation

I'm impressed with the clean, welcoming facility and round-the-clock, professional care - nurses and doctors are informative, staff know residents well, and rehab helped my mom improve and smile again. Communication is timely, meals and activities are good, and overall it's a caring, safety-conscious place that meets our needs.

    Summary of reviews

The reviews for Accordius Health of Salisbury present a strongly mixed picture: several families describe positive clinical outcomes and attentive rehabilitation services, while a substantial number raise operational and care-delivery concerns. Strengths most consistently mentioned include effective physical and occupational therapy, successful post-operative rehabilitation in many cases, and instances of compassionate, professional nursing and caregiving staff. Multiple reviewers praised friendly front-desk personnel, clear family communication, and improvements tied to management or ownership changes.

Care quality appears uneven. Positive accounts describe timely post-surgical attention, encouragement from therapy staff, and measurable functional gains. Contrasting accounts document lapses in routine personal care and responsiveness โ€” for example, inconsistent bathing or mealtime assistance and delays in addressing incontinence needs. Medication administration and pain-management practices are another area of variability: some families report timely treatment, while others describe delays or inconsistencies. There are also several more serious allegations concerning medication handling and administrative conduct; those items warrant independent verification with regulatory sources.

Staffing and staff conduct are recurrent themes. Reviewers describe dedicated, caring caregivers and skilled therapists alongside reports of staff shortages, high turnover, and inconsistent staff behavior across shifts. Instances of poor communication or unhelpful responses from certain nursing assistants and managers are contrasted by other reports of attentive, professional clinicians. This variability suggests experience may depend heavily on unit assignment and shift staffing levels.

Dining, activities, and ancillary services are similarly inconsistent. Some residents and families report appetizing, hot meals and meaningful group activities or social therapy; others describe missed or delayed meal delivery, limited activity assistance, and variability in the availability of programs. Laundry and linen management, housekeeping, and routine maintenance also appear uneven: several reviewers noted delays in linen turnover, peeling paint, broken furnishings, and sanitation or odor concerns in specific areas. A subset of comments raises pest-control concerns and housekeeping gaps that affect perceived cleanliness.

Management and safety patterns emerge as mixed: a number of reviewers observed tangible improvements after leadership changes and praised clear communication and better systems, while others reported unresponsiveness from management and regional leadership, unresolved follow-up, and difficulties obtaining timely answers about transfers or incidents. There are also reports of property-security shortcomings (missing personal items) and gaps in emergency-response coordination, including issues with hospital transfers; these point to potential process weaknesses rather than isolated events.

Overall, the facility appears to deliver strong rehabilitation and positive clinical outcomes for some residents, particularly when therapy and staffing are stable, but also exhibits operational weaknesses that can materially affect day-to-day resident experience. Prospective residents and families should consider an on-site visit to evaluate current unit cleanliness and maintenance, ask detailed questions about staffing ratios and turnover, review medication and emergency-transfer policies, inquire about laundry and property-security procedures, and request recent state inspection and corrective-action documentation to corroborate improvements cited in positive reviews.
