    Game-changing rehab with caring staff

My dad's stroke rehab was a game changer - the therapy team got him walking and the nurses and aides were genuinely caring. Clean, modern rooms and beautiful, home-like grounds, with a welcoming admin team and excellent Alzheimer's care - I highly recommend.

    Summary of reviews

Reviews for AVENTURA at Humility House present a strongly polarized picture. Many families and patients describe excellent clinical and rehabilitative care โ€” particularly praising the nursing staff, certified nursing assistants, and the physical/occupational therapy teams โ€” and report meaningful functional gains such as successful stroke rehabilitation. The facility's physical environment also draws consistent praise: clean, modern interiors, attractive grounds, and well-furnished resident rooms contribute to a home-like atmosphere that families value.

At the same time, a substantial set of operational concerns appears across the record. Themes include inconsistent staffing levels that correlate with delays in personal care and toileting assistance, apparent gaps in medication administration and documentation, and fall-prevention shortcomings. Several reviewers describe failures of timely supervision that contributed to falls or unmet basic needs. Billing and invoicing processes are frequently cited as error-prone and slow to resolve, and front-desk or telephone responsiveness is identified as an area of weakness.

Staff conduct and management responsiveness form another key dividing line. Positive accounts describe a warm, family-like administrative team that is welcoming to families and vendors and that goes above and beyond for residents. Contrasting accounts describe dismissive or disrespectful interactions, accusations of discriminatory or exclusionary conduct, and a perception that financial priorities sometimes supersede clinical needs. Reviewers also indicate that a change in ownership preceded an operational decline for some families, and some have considered or pursued external complaints.

Dining and activity offerings are mixed: the facility is noted for religious services and occasional music programming, but several reviewers find activities limited overall and report inconsistent meal temperature and quality. Safety and supervision practices โ€” including the use or communication around locked or segregated units โ€” were raised as concerns by some families and warrant direct clarification during a tour.

Taken together, the reviews indicate pockets of high-quality clinical care and an attractive physical environment alongside recurrent operational and management issues. Prospective residents and families would benefit from targeted questions during a visit: current staffing ratios and how shift coverage is managed, medication-administration protocols and documentation, fall-prevention and supervision procedures, examples of recreational programming beyond religious services, billing practices and avenues for dispute resolution, and the facility's policies on restricted units and family communication. Reviewing recent inspection reports and speaking with current resident families can also help validate whether the positive clinical strengths are consistent and whether the operational concerns have been addressed.

    About AVENTURA at Humility House

AVENTURA at Humility House sits at 755 Ohltown Road in Youngstown, Ohio, and works as part of Mercy Health Humility House Senior Living, focusing on both short-term rehabilitation and long-term care for seniors, where people recovering from illness or injury can stay for a short while or settle in for longer if their needs are greater, and with 70 certified skilled nursing beds and assisted living suites, the place is equipped for those needing help with basic daily things as well as those needing more complex medical care, with licensed nurses and aides always present, day and night, and nurses prepared to help with things like managing diabetes, heart conditions, and even tougher health needs, such as respiratory care with oxygen, CPAP, BiPAP, or a nebulizer. The facility features a Skilled Nursing Facility and a Physical Medicine and Rehabilitation Clinic, with specialized programs in neurological and stroke recovery, memory care, and locked dementia care units for Alzheimer's or other dementias, and in times when infection control is needed, they have a separate Temporary Quarantine Unit for unvaccinated residents and can care for those who test positive for COVID-19, plus they make COVID-19 vaccination available onsite. The facility can arrange IV therapy, IV hydration, assistance with medications, bathing, dressing, meals, and other kinds of personal support as needs arise, and their round-the-clock nursing comes from registered nurses, licensed practical nurses, and nurse aides, which is supported by daily physical, occupational, and speech therapies for those working to regain their abilities after an illness or injury, so those needing help getting stronger have that option too. Residents can take part in various activities that are scheduled each day, with some led by the community, others run by residents themselves, and folks have access to common spaces like a garden, an outdoor area, a dining room with restaurant-style meals where special diets can be accommodated, a small library, and a chapel onsite for spiritual needs, and every resident room comes with cable TV, phone, WiFi, housekeeping, linen service, and is fully furnished with a kitchenette for small meals or snacks if wanted, plus a 24-hour call system if someone needs help in a hurry. The community provides meals and can meet dietary restrictions, including for those with diabetes, and has a wheelchair-accessible van for trips to medical appointments or dialysis, making it easier for residents to get where they need to go. The place has strong points in its advanced therapies and holistic care and is rated four stars by CMS for quality measures, but it has some areas where there've been problems, such as a nurse turnover rate that's higher than the average for Ohio, and reports of deficiencies from inspections-35 in total-including 4 related to infection control, with recent findings covering proper pressure ulcer care, administration shortcomings about informing residents on their rights regarding arbitration agreements, and gaps in developing full care plans for some residents, reflecting some real challenges they're still working to improve. Managerial control is shared by Moishe Kaszirer and Mordechai Scharf since early 2022, and the nursing hours per resident each day come in just a bit below the average for the state, so that's something to consider. Staff mainly speak English, and some may speak other languages as well. At this time, they're not open to new admissions but can be contacted for future availability. While the facility offers many services and features, those wanting to know more should look at the public inspection reports and compare their information with other options in the area to make the best decision for their needs.
