    Outstanding rehab and compassionate staff

I'm very pleased with our experience - the rehab and therapy team were outstanding and Lori R. (licensed social worker), Mallory, Marissa and others went above and beyond arranging equipment, home care and insurance help. The facility is clean, homelike with good food and a friendly, respectful staff whose responsiveness and kindness made my mom comfortable during a difficult time. Overall a very good, well-run place I'd recommend for rehab or long-term care.

    Summary of reviews

Overall impression: Reviews for Aventura at Assumption Village are mixed, with a clear split between strong clinical/rehabilitation experiences and recurring operational concerns. Many families and former patients praise the therapy department, individual caregivers, and the social-work team for effective discharge planning and hands-on coordination. At the same time, a pattern of staffing instability, communication breakdowns, and process gaps is described by other families, producing inconsistent resident experiences.

Care quality: The facility demonstrates significant strengths in short-term rehabilitation: reviewers cite compassionate therapists, measurable therapy gains, and above-average rehab outcomes. Social-work support (frequently named) is repeatedly highlighted for arranging home health and equipment. Conversely, there are substantive concerns about clinical process reliability โ€” examples include missed or poorly documented medication orders, therapy removal without family notification, and coordination failures with external treatments such as dialysis. Several accounts also indicate delays in attending to toileting and personal-care needs and instances where transfer safety procedures appeared inadequate. These reports point to variability in clinical oversight and protocol adherence.

Staff and workforce issues: Many reviewers describe direct-care staff as kind, respectful, and willing to go above and beyond; admissions and front-office staff are also praised in numerous comments. However, recurring themes include high staff turnover, low morale following a change in operator, and uneven professionalism at entry points. Some families report difficulty reaching managers, unreturned phone calls, or nurses who are not visibly present on the unit. Staff shortages and workload pressures appear to contribute to slower response times and inconsistent day-to-day care.

Dining, activities, and amenities: Opinions on dining are mixed. Several reviewers appreciated homemade-style meals and responsive dietary staff, while others described small portions and meals that did not match posted menus. Activity programming and resident engagement receive positive mention โ€” events, pet visits, and individualized celebrations were noted as meaningful to residents and families. Salon services and some nonclinical amenities were identified as inconsistent in quality.

Facilities and management: The physical environment is described as clean and homelike by many families, particularly during periods of stable leadership. Several reviewers link declines in service quality to recent ownership and management changes; others note improvements after a newly engaged administrator took a hands-on approach. Financial and administrative concerns appear repeatedly โ€” examples include disputed charges, billing for periods when a resident was not present, and deposit policies. Families are also concerned about personal-item handling and record accuracy.

Notable patterns and guidance: The most consistent positive signal is the strength of the rehabilitation program and individual caregivers who provide attentive, empathetic support. The most consistent operational risks are staffing instability, communication failures with families, and lapses in procedural consistency (medication, transfers, external-service coordination). Prospective residents and families should clarify current staffing ratios, medication-administration safeguards, transfer-training and lift protocols, call-bell response targets, billing practices, and how the facility communicates clinical changes to families. Visiting during meal service, observing therapy sessions, and speaking directly with the social-worker or therapy lead can help confirm whether the facilityโ€™s strengths align with a specific residentโ€™s needs.

    About Aventura at Assumption Village

Aventura at Assumption Village sits at 9800 Market Street in North Lima, OH 44452, offering skilled nursing and rehabilitation services with memory care and locked dementia care units, and while it isn't accepting new patients right now, folks can still reach out to find out when spots open up. The campus has assisted living suites and seventy skilled nursing beds, and all together there's a bed count of 150, so it's big enough to meet a lot of needs without being overwhelming, and you'll find a wide range of services for seniors needing daily help or round-the-clock care. The facility's staffed by licensed nurses and certified aides twenty-four hours a day, seven days a week, and some staff speak languages besides English, so many residents find someone who understands them, and the staff get described as helpful by folks who've been there. The clinical care includes things like respiratory care, cardiac management, diabetes management, pain management, IV therapy and hydration, wound care, and even specialized therapies like O2, nebulizer, CPAP, and BiPAP, plus post-acute rehab with physical, occupational, and speech therapy every day, which helps people get back on their feet after illness or surgery.

Memory care is available for those with dementia, and hospice services are offered for residents needing comfort at the end of life. For people needing help with daily activities, there's assistance with bathing, dressing, and eating, and there's incontinence and diabetic care, and the facility can help with medication management and supplements for feeding when needed. Housekeeping and laundry are done by staff, and meals come prepared fresh and served in a restaurant-style dining room, making it easy for residents who like company at meals. There's a chapel for spiritual services for people of many faiths, and devotional activities take place on-site for those who want them. Common areas-both indoors and out-are open for gathering, and activities take place both at the facility and out in the community, aiming to keep people socially engaged and entertained.

Residents can use a wheelchair-accessible van for trips to doctors' appointments or dialysis, which helps when someone can't drive or has trouble with transportation. The campus is described as beautiful, with the Assumption Village location in North Lima and another site, Humility House, in Austintown, which may make visits more pleasant for family and friends. Suites come fully furnished and comfortable, and residents can pick flexible schedules that work for them. Amenities include TV, Internet, and phone service at no extra cost, so people can stay connected, and both in-person and virtual visits are supported for families who live nearby or farther away. There's even a studio apartment setup where residents can practice daily living activities so they can build up strength or relearn skills in a safe space.

Aventura at Assumption Village can provide quarantine units for people who don't want a COVID-19 vaccine, and the vaccine is available on-site for those who do, so the place is equipped to adapt to the needs and choices of its residents. This facility holds a four-star quality measure rating from CMS, which speaks to the standards in place. The staff provide care for people who need help with walking or non-ambulatory care, and the flexible care model lets someone move between assisted living and higher nursing care as needs change. Wellness programs like music therapy and spiritual services are included, and the facility offers respite and long-term care, plus extra attention for stroke and neurological recovery. Aventura at Assumption Village isn't fancy, it doesn't promise everything, but it's a solid choice for folks needing skilled nursing, memory care, therapy, and daily living help, all in a steady, easy-to-navigate setting.
