    Spotless facility, compassionate attentive staff

I placed my loved one here and am very pleased with the beautiful, spotless facility, compassionate and attentive nursing/aide staff, and excellent therapy that helped with recovery. Meals are good, rooms and bathrooms are cleaned daily, administration is responsive, and it truly feels like a second family - I recommend it for short-term rehab or long-term care.

    Summary of reviews

Tallmadge Health and Rehab elicits strongly mixed responses. Many reviewers describe a clean, modern facility with compassionate caregivers, effective rehabilitation services, and a welcoming community atmosphere. Therapy programs (physical, occupational and speech) receive frequent praise for restoring mobility and independence; several families also highlighted strong hospice and end-of-life support. The facility’s appearance, private-room options, daily housekeeping when it occurs, and an engaged kitchen staff that provides customizable meals were cited as clear strengths.

Care quality and staffing are the principal axes of variability. Positive accounts emphasize attentive, dedicated nursing and aide staff who communicate with families and follow up when concerns arise. Conversely, a notable pattern of understaffing, frequent turnover, and long wait times for assistance is described across reviews. Those operational issues are associated with delays responding to call lights, inconsistent bathing and toileting assistance schedules, and interruptions in therapy or scheduled care. Reviewers also raised clinical concerns including delayed medication or feeding administration and gaps in wound-care follow-up; these concerns point to opportunities to strengthen clinical protocols and oversight.

Dining and activities show mixed impressions. The kitchen is frequently praised for hot meals and improved offerings, and many residents enjoy customizable choices and regular menu communication. At the same time, several reviewers expressed that menu options were not consistently adapted for specialized diets such as diabetes. Activity programming and on-site therapies are generally viewed favorably, but transportation and off-site outing capacity appear limited, constraining opportunities for larger-group excursions.

Facility-level operations and management present a contrasting picture. Some families describe responsive administrators who address problems and follow through; others report weak HR responsiveness, leadership instability, and inconsistent enforcement of professional standards. Housekeeping and supply management are uneven: while many describe a tidy, organized environment, other accounts note sanitation concerns in specific rooms or supply shortages (e.g., linens, washcloths). There are also periodic mentions of personal-item accountability issues and pest-control concerns in isolated units.

Overall, Tallmadge offers many of the elements families seek—modern facilities, strong rehabilitation services, and caring staff—but recurring operational weaknesses (staffing consistency, clinical-administration processes, sanitation controls, and leadership responsiveness) create variability in resident experience. Prospective residents and family members should consider visiting during multiple shifts, asking for current staffing ratios, clarifying medication- and wound-care protocols, reviewing housekeeping and pest-control procedures, confirming dietary accommodations for special needs, and requesting recent examples of management follow-through on concerns to evaluate whether the facility’s strengths align with their priorities.

Tallmadge Health and Rehab sits at 619 Northwest Ave in Tallmadge, OH, near Tallmadge Circle and 6 Corners, so you're close to parks, shopping, and restaurants, which can be helpful for visitors and residents. The center opened in 2021 and has 90 certified beds, usually caring for about 82 people each day, and it's part of the Saber Healthcare Group. The facility is a for-profit operation and focuses on both short-term senior rehabilitation and long-term, skilled nursing care, providing services like physical, occupational, and speech therapy, plus a wide range of nursing care and daily living support, such as medication management, help with bathing, dressing, transfers, and non-ambulatory care.

People living at Tallmadge Health and Rehab have access to amenities like cable TV, kitchenettes, private bathrooms, air conditioning, phones, Wi-Fi, and an emergency alert system in their rooms, while common areas offer a movie theater, game room, library, fitness room, outdoor patios with scenic views and walking paths, and community spaces for daily activities. The place holds arts rooms, business rooms, a wellness and sauna room, and fitness and music programs, plus animal therapy, book reading, coloring, and chair-based exercise groups, trying to make daily life comfortable and engaging. Licensed therapy staff run programs for pulmonary rehab, respiratory therapy, cardiac care, post-surgical care, IV therapy, wound care, and more, and there's a special Pre-registration Program for those planning elective surgery who'll need rehab. Tallmadge Health and Rehab also offers on-site home dialysis through Dialyze Direct, supporting residents who need regular dialysis, and uses the Progressive Approach to Home (PATH), trying to help residents recover safely and go home when possible.

Inspection reports have shown the facility has 42 documented deficiencies, including several around infection control, with seven infection-related violations, and issues related to how food is prepared and served-not always meeting professional standards, and sometimes missing the mark on making sure food is safe, appetizing, and at the right temperatures. Staff are trained to handle a range of medical needs, with programs for physical and mental well-being, stroke recovery, powdery and congestive heart failure care, and special therapies for residents with respiratory issues or people who've had lung transplants.

Residents get information and support for both themselves and their families, and the team works alongside physicians to create rehab and care plans that suit each person's needs, whether someone's there for a short stay to get back on their feet or for longer-term nursing care, assisted living, memory care, or personal care. Tallmadge Health and Rehab takes Medicare and Medicaid and aims for an at-home atmosphere with scheduled activities and holiday events for everyone living there.
