    Wonderful caring staff and facility

I had a wonderful stay - the staff were caring and friendly, nurses and therapists were attentive and excellent, and aides were sweet and efficient. The facility was clean, activities were engaging, and communication was responsive; I was always greeted with smiles. I highly recommend this resident-focused place and would return for future care.

    Summary of reviews

The reviews for The Pinnacle Rehabilitation Center and Nursing Home are markedly polarized, with clear patterns of both strong rehabilitation outcomes and troubling operational inconsistencies. Many families and residents praised the facility's therapy services, noting skilled physical therapists and a supportive rehab environment that produced measurable recovery. Positive comments also highlighted compassionate nursing attention, attentive certified nursing assistants, friendly admissions and frontline staff, and an engaging activities program that contributed to resident well-being. Several accounts described effective communication from staff and management and a generally well-maintained public environment in at least parts of the facility.

Conversely, a number of reviews describe recurring operational weaknesses that affected safety, comfort, and trust. The most consistent concerns relate to delays in clinical responsiveness โ€” including slow response to call lights and delayed attention to wounds or incisions โ€” and to apparent gaps in infection-control and wound-management practices. These patterns were sometimes linked to escalation to higher-acuity care. Reviewers also raised issues around resident dignity and privacy, including lapses in timely assistance for toileting and dressing. Cleanliness and sanitation were described as uneven: while some common areas were noted as clean, other accounts cited odor concerns, overflowing receptacles, and portable sanitation problems.

Dining and daily routine were another area of variability. Several reviewers criticized inconsistent meal timing and food quality, including late dinners and meal-service interruptions. Staffing consistency and supervision appear to be underlying contributors to many negative experiences; reports of staff chatting at the nurses' desk, communication-system outages, and visible smoking near the entrance (with smoke entering the building) point to lapses in enforcement of policy and equipment reliability. Phone or communication outages were also noted, which can amplify delays in response and family communication during those periods.

For prospective residents and families: the facility appears capable of delivering effective rehabilitation and compassionate care in many cases, particularly around therapy and activities. At the same time, there are notable and recurring operational concerns that merit direct inquiry. Recommended steps before placement include observing staff response times, asking for the facility's infection-control and wound-care protocols, reviewing staffing ratios and supervision practices for the desired unit, confirming meal schedules and sample menus, and asking how the facility enforces smoke-control and maintains communication systems. Speaking with the therapy team and with other families currently using the facility can provide additional context about consistency of care over time.

About The Pinnacle Rehab & Nursing Home

The Pinnacle Rehabilitation Center and Nursing Home, also known as Summit Villa Care Center, LLC, sits in Tallmadge, Ohio and has operated in some form since 1992, with the current management group from Northwood Healthcare Group LLC taking over in 2017, and although it's a for-profit place with strong oversight and many moving parts in ownership, people who live there mainly see a 75-bed facility focused on rehabilitation and nursing care with an average daily census of around 65, and it does have 5 stars from the Centers for Medicare & Medicaid Services for its nursing home service, but standard inspections haven't happened in more than two years and there have been at least nine deficiencies noted, including issues with environmental safety and with notifying families and doctors, none of which caused actual harm but have carried the risk for more than minimal harm if not addressed, and this seems pretty important if you pay attention to the details.

You'll find skilled nursing staff on-site around the clock, but the total nurse hours per resident per day average 3.37, which is a bit less than the state average of 3.7, though the nurse turnover rate is lower than most at 42.1%, so some consistency may be there, and you'll get the usual short-term, long-term, respite, and rehabilitation care types, including physical, occupational, and speech therapy, with specialized attention from a critical care pulmonologist, Dr. Vishal Sawhney, and an infectious disease physician, Dr. Badie Alnemr, plus wound and respiratory therapy, and a social services team that helps residents and families with everything from discharge planning to day-to-day help, which can make a difference for some.

The rooms are all studio layout, so that means private space isn't as open as you'll find in some other places, but the place does focus on creating a homelike atmosphere and keeping up with daily social activities and individualized care, though the environmental and resident rights deficiencies in inspections are worth considering, and while the facility holds an A+ rating from the Better Business Bureau, it's not actually accredited, so there's some paperwork but not all the reassurance you might expect, but for people looking for rehabilitation or ongoing skilled nursing services in the area, The Pinnacle does offer a centralized place with specific programs, licensed under Medicaid ID 0243863 with its current license good until early 2026, and there's an experienced administrator, Ms. Gerri Mc Gonagle, overseeing the day-to-day efforts to support senior wellness and recovery.
