    Dramatic turnaround; clean, caring staff

I'm impressed by the dramatic turnaround at Canfield - the building is clean, smells fresh, and new management has been proactive and polite. The staff are genuinely caring, attentive, and went above and beyond during my rehab; activities (therapy dogs, events) and good meals made it feel like family. Communication and admission were smooth, and I would happily recommend this facility.

    Summary of reviews

Reviewer feedback for Canfield Healthcare Center shows a pronounced split between accounts of high-quality rehabilitative care and persistent operational concerns. On the positive side, many families highlight the therapy team and short-term rehabilitation services as particularly effective: clinicians and therapists are described as skilled, communicative, and instrumental in helping patients regain strength and return home. Numerous comments praise staff who provide personalized attention, create a family-like atmosphere, and engage residents through activities and visits such as therapy-dog programs. Several reviews also note improvements in cleanliness and a more welcoming reception experience following management changes.

Conversely, a sizable portion of feedback identifies recurring operational weaknesses that affect resident comfort and safety. Themes include inconsistent nursing care, delays in medication delivery, and lapses in timely assistance for activities of daily living. Reviewers describe supply and equipment-management problems (for example, delayed provision of appropriate mobility aids and bedding) and gaps in wound-care practices that correspond with pressure-injury concerns. Sanitation and odor-control issues in certain areas, as well as variable food quality and meal-service reliability, are also repeated concerns.

Staffing and leadership appear uneven across time and shifts. Multiple accounts contrast highly engaged, compassionate employees with reports of inattentive or poorly trained staff, suggesting inconsistency in staff performance and supervision. Several reviewers credit recent management changes with measurable improvements in housekeeping, communication, and overall tone, while other comments indicate ongoing problems with administrative responsiveness, care-plan communication, and admissions interactions. There are also allegations of regulatory scrutiny in connection with serious clinical incidents; these are significant and warrant direct verification with official sources.

For prospective residents and families: the facility demonstrates clear strengths in rehabilitation and in teams that provide warm, individualized care, but these strengths coexist with operational risks related to clinical consistency, sanitation control, equipment availability, and communication. When evaluating this facility, visitors should observe multiple shifts, review wound- and pressure-injury prevention protocols, ask about medication-administration procedures and staffing ratios, inspect common areas for cleanliness and odor control, and request documentation of any recent regulatory actions or corrective plans. These steps will help determine whether the current improvements cited by some families have been sustained across the facility.

    About Canfield Healthcare Center

Canfield Healthcare Center sits at 2958 Canfield Rd in Youngstown, Ohio, and has been open since January 1, 1983, with Mr. Matt Parkes as the administrator, and it falls under the CommuniCare Family of Companies, focusing on nursing home services, rehabilitation, hospice, and senior care, though it's not accepting new patients right now. The center offers nursing care with skilled staff available 24/7 for seniors who need advanced medical attention, and they have a Physical Medicine and Rehabilitation Clinic providing both short-term and long-term care, including physical therapy, wound care, hospice, and palliative care. People living with Alzheimer's or dementia can get help from their Memory Care service, which has individual care plans, a safe environment, and programs to support cognitive health and daily routines. There's also a subacute care service and different programs that focus on a person's emotional and physical health, and staff members speak English and some other languages, which helps folks feel at home. Seniors can join social, educational, and fun activities to keep up friendships and maintain quality of life, and they'll find comforts like housekeeping, laundry, and on-site meals once or twice a day. The community encourages residents to live actively and independently as much as they're able, and there's easy access to local medical offices, places to eat, shopping, and spots to go outside. Staff are described as warm and friendly, ready to lend a hand, and the center says it builds ongoing partnerships with residents and their families, always aiming to deliver high-quality care and create a good experience for both guests and residents. Canfield Healthcare Center also keeps an active Facebook page, but there's no further information available online about them, and details on their rooms or costs aren't provided.
