    Excellent care and supportive staff

I was very pleased with my mom's rehab stay. Housekeeping-especially Maria Rivera-kept her room spotless and added cheerful touches that brightened her days. The PT/rehab therapists (including Kayla Talley and Ramona's team) pushed just the right amount and were very encouraging; nursing and medical staff managed wound vac care, meds, and daily check-ins professionally. Meals met her needs, activities matched her interests, and the facility felt clean, safe, and welcoming-I would choose them again for rehab.

    Summary of reviews

The reviews for Windsor House at Liberty present a markedly mixed picture, with clear strengths in therapy, housekeeping, and individual staff performance alongside recurring operational and clinical concerns. Many families praised the facility's rehabilitation services, noting therapists who are skilled at progressing residents safely and who provide encouragement and individualized plans. Housekeeping and room maintenance were frequently singled out as strengths, and several reviewers described personable front-line staff and a generally welcoming environment. Meal service and dietary accommodation receive positive comments from families who experienced attentive kitchen staff and appropriate menu options.

At the same time, there are consistent themes indicating variability in how the facility operates day to day. Understaffing and staffing inconsistency are prominent concerns and are linked to delayed responsiveness to resident requests, longer waits for assistance, and uneven delivery of care tasks. Several accounts indicate gaps in clinical monitoring and escalation processes; these reports include delayed emergency response activation and situations where families intervened to obtain higher-level care. Medication-administration timing and documentation were described inconsistently across reviews, which contributes to concerns about clinical reliability.

Dining and nutrition present a mixed pattern: while some families praised the kitchen for meeting special dietary needs, others noted inconsistent adherence to prescribed diet textures and presentation. Meal-time staffing and assistance appear variable, with some residents receiving attentive support and others observed to lack timely help during meals. Activities programming was generally viewed positively when present, with events and interest-aligned activities contributing to resident engagement; however, specifics about schedule consistency and breadth of programming were less frequently detailed.

Facility condition and housekeeping mostly receive favorable comments—clean rooms, well-maintained common areas, and attractive design are cited—but there are isolated sanitation and odor concerns in some rooms or areas that families recommended inspecting in person. Management and communication practices show divergence: some families reported prompt, effective responses from administrators and social work staff, while others described inconsistent follow-through, misinformation during admission or discharge coordination, access-control issues, and problems with transportation or appointment logistics. There are also serious individual claims such as missing personal belongings and concerns about oversight; these have been raised by reviewers and suggest areas where inventory controls and property management should be clarified.

For prospective residents and families, the pattern is one of strong clinical and hospitality capability present in the facility, offset by operational variability tied largely to staffing and oversight. When evaluating Windsor House at Liberty, consider verifying current staffing ratios and emergency-escalation protocols, observing meal service and activity offerings, and clarifying admission/discharge and transportation processes. Asking about specific care plans, diet texture compliance procedures, medication administration practices, and property-inventory safeguards can help determine whether the facility's strengths align with an individual resident's needs.

    About Windsor House at Liberty

Windsor House at Liberty sits on Churchill Hubbard Road as a nursing home facility with a 3-star rating from the Centers for Medicare & Medicaid Services, and the community gets a 6.4 out of 10 rating, though online reviews give it a 1.5 out of 6 reviews, so some folks have had mixed experiences, and it's important to think about that when considering a place like this, but folks stay here on average about 93 residents a day, and the place has 110 certified beds in comfortable layouts, including studio, one-bedroom, and two-bedroom apartments, each with their own kitchenettes, good bright windows, and all utilities included in one monthly fee, which makes things a bit easier for families. The facility's owned and managed by Omni Manor, Inc. since 1995 and is affiliated with Windsor House, Inc., so it's been around for a while, and it offers assisted living, nursing home care, skilled nursing, respite care, memory care, and special Alzheimer's care in secure units with trained staff watching close by, plus they have 24/7 caregivers and nurses who take care of things like bathing, grooming, dressing, medication management, and helping folks with chronic conditions, and even though they have a nurse turnover rate of 51% which is a bit higher than some, they keep a staff-to-resident ratio with about 3.75 nurse hours per resident per day.

Windsor House at Liberty lets residents join in on social events and organized activities, they can visit the hobby room, salon, barber shop, chapel, club room, and library, or just use the common areas or beautiful outdoor spaces for relaxing, and the dining is restaurant-style, which some folks like for the variety. There's devotional services and outings, plus scheduled transportation, and residents get daily housekeeping and laundry services. For health and wellness, there's physical, occupational, and speech therapy, evaluation and treatment of illnesses, medication management, on-site lab, X-ray, and pharmacy services, dental, vision, podiatry, and audiology visits, as well as mental health counseling by licensed professionals, and the counselors focus on supporting emotional well-being for both residents and families through a mix of approaches.

Facility policies cover abuse prevention, and regular inspections aim to meet federal standards for infection control, though there have been some documented infection-related issues in the past. Staff provide restorative nursing and independent living programs for folks who want to keep or regain their independence, and the care philosophy tries to keep everyone treated with respect and as much independence as possible, so residents can feel at home while getting the support they need. Windsor House at Liberty hasn't posted exact prices yet. Emergency help is always available, just in case, and staff do their best to build a welcoming atmosphere, but it's always wise to visit and see if it suits your own needs and preferences.
