    Compassionate clean responsive family-oriented care

I placed my dad at Quail Ridge for rehab and long-term care and have been very pleased - compassionate, well-trained staff (Taylor, nurses and aides) treat residents like family, the facility is clean, meals are good, and administration is responsive. Therapy success, hospice support, and engaging activities gave us peace of mind; I recommend Quail Ridge for attentive, family-oriented care.

    Summary of reviews

The reviews present a largely positive picture of Quail Ridge Living Center, with frequent praise for day-to-day caregiving, clinical rehabilitation, and leadership. Many families describe attentive nurses and CNAs, a supportive administrator who is visible and engaged, and staff who treat residents in a family-style manner. Rehabilitation outcomes and hospice/end-of-life care are cited as strengths; several accounts highlight successful therapy for orthopedic recovery and compassionate hospice support. Long staff tenure and a reportedly warm workplace culture are recurring themes, which stakeholders often link to continuity of care.

Staffing and interpersonal care are the facility's clearest strengths. Reviewers consistently commend hands-on staff, named CNAs and nurses, and administrative responsiveness. Activity programming is described as active and varied (bingo, dominoes, balloon volleyball, manicures, hair days), and the facility supports socialization through pet visits and communal dining. Several reviewers also noted helpful administrative functions such as insurance/Medicaid guidance and a responsive financial coordinator.

Dining and facilities receive mixed but generally positive comments. Many families praised meals and the dining experience, while a subset described boxed or non-homemade food and occasional missed meal service unless requested. Overall common areas and social spaces are often called clean and welcoming; however, a number of reviews raised sanitation and odor concerns in specific locations and described visible wear and tear in parts of the building, indicating uneven maintenance.

There are recurring operational concerns that prospective residents and families should examine directly. Multiple reviews point to inconsistent clinical monitoring (for example, glucose checks) and gaps in medication-administration processes. Some reviewers raised issues with nursing responsiveness and isolated safety practices—examples include unsafe gait-belt usage and bed placement near windows—suggesting opportunities to strengthen transfer and fall-prevention protocols. A few comments also referenced staff professionalism concerns, including inappropriate workplace behavior and privacy lapses such as resident information left accessible on computers.

Management is generally viewed positively for being accessible and proactive; several reviewers credit leadership with creating a family-oriented culture and supporting staff education. At the same time, some families said management addressed concerns when raised, which suggests an openness to corrective action. Given the mix of strong caregiving reports and specific operational weaknesses, prospective families should confirm current conditions during a visit, ask about staffing levels, clinical-monitoring protocols, medication-safety procedures, privacy safeguards, and recent state inspection findings. Overall, Quail Ridge appears to offer a high level of person-centered care with active programming and engaged leadership, tempered by actionable opportunities in clinical processes, privacy controls, and facility maintenance.

    About Quail Ridge Living Center

Quail Ridge Living Center sits in West Siloam Springs, OK, right on the Oklahoma side of State Line Road near Siloam Springs, Arkansas, and the building's single-level design helps residents get around easily, whether they're moving with a walker or rolling along. The place houses a 118-bed Skilled Nursing and Rehab Facility, making a home for folks who need all kinds of care, including skilled nursing, respite, long-term care, rehabilitation, assisted living, independent living, memory care, and even specialized hospice care, and they take Medicaid, Medicare, and private insurance, which gives families some peace of mind about payment. The Women's Memory Care Unit, which has a secured wing and 80% of its staff certified as Certified Dementia Practitioners, is a place where women with memory issues get extra attention and safety.

People here can spend time outside, working in the sunny community garden or relaxing out on the patio at the picnic table, and there are weekly and monthly activities, including church services, coffee and chatting, walking and rolling, birthday bingo, shuffleboard, and domino tournaments that give everybody chances to meet others and stay busy. The cheerful common areas make good spots for socializing or enjoying refreshments, and residents enjoy home-style meals in the dining room, plus they've got family-style dining and can bring their four-legged pets, which a lot of folks appreciate. There's also a beauty salon and barbershop, so folks can keep up with haircuts right on site, and both a psychologist and a podiatrist visit regularly, along with a full-time dietary manager to watch over nutrition.

Inside, residents get housekeeping services, free Wi-Fi, and cable TV, so things stay comfortable, clean, and connected, and transportation is provided for medical appointments or activities out and about. The facility offers outpatient therapy, diagnostic imaging, and several wellness programs, with staff who provide physical, occupational, and speech therapy, restorative care, and fall prevention. Nurses and an interdisciplinary team handle daily needs, offering care that fits what each resident needs, and they even help with Medicare applications. The atmosphere feels warm and friendly, partly because staff stick around and because people say the staff have a compassionate attitude. Quail Ridge Living Center gives seniors a place where they can enjoy time outdoors, take part in events, and live in comfort, with both the staff and the setting set up to help folks feel respected and cared for as they age.
