    Kind staff and excellent care

I placed my mom at Wilkins and couldn't be happier - the staff are kind, professional and attentive, the facility is clean and homey, therapy and activities are excellent, and administration stays communicative, which gives me real peace of mind. I highly recommend them.

    Summary of reviews

Wilkins Health & Rehabilitation Community elicits strongly polarized experiences. Many families emphasize the facility's strengths: staff are frequently described as compassionate, familyโ€‘oriented, and attentive, and nursing teams are credited with delivering reliable dayโ€‘toโ€‘day care. Rehabilitation services โ€” including physical therapy and speechโ€‘language pathology โ€” are repeatedly cited as effective, with concrete examples of mobility and functional improvements. Several accounts highlight assistance with admissions, appointments, and paperwork, as well as regular family updates that contribute to a sense of trust and peace of mind.

At the same time, a subset of reviews describes operational shortfalls that prospective families should evaluate. Medicationโ€‘management reliability is a recurring concern, encompassing both missed doses and staffโ€‘level questions about dosing practices. Safety and supervision issues are also present in some accounts, including falls and instances of residents leaving supervised areas; these point to gaps in transferโ€‘safety and monitoring protocols. Cleanliness and maintenance assessments are mixed: while many reviewers praise a wellโ€‘kept, homeโ€‘like interior, others describe sanitation and odor concerns and small or outdated room conditions. Reports of personalโ€‘property losses and uneven incident notification indicate weaknesses in security and family communication practices.

Activities and dining are generally positive elements: the facility runs a range of engaging, tailored programs and social events, and several families found the food acceptable. Longโ€‘term residents and families often describe a warm, community feel and continuity of relationships. Management impressions are inconsistent โ€” some reviewers praise supportive leadership and responsive administration, while others describe unresolved complaints and ownershipโ€‘level disputes. The surrounding neighborhood is also a consideration for some families.

Overall, Wilkins appears to offer strong rehabilitative care and a caring staff culture for many residents, but experiences vary enough that an individualized assessment is advisable. When evaluating this facility, prospective residents and families should ask targeted questions about medication protocols, staffing ratios, safety and elopement prevention measures, cleaning/maintenance schedules, propertyโ€‘security practices, and recent inspection or quality metrics. An inโ€‘person tour that observes therapy sessions, common areas, and staffโ€‘resident interactions, plus conversations with current families, can help clarify whether the facilityโ€™s strengths align with a particular residentโ€™s needs.

About Wilkins Health & Rehabilitation Community

Wilkins Health & Rehabilitation Community sits right in Duncan, Oklahoma, serving up senior living and nursing care with a strong focus on rehabilitation, long-term care, and memory support, so folks get both skilled help and some dignity in their routines. The place has 118 certified beds, a historical occupancy rate near 92%, and keeps nurse staffing at an average of 4.38 hours per resident each day, which means you'll often see a staff member nearby when you need help. They run a for-profit corporation owned mostly by Tony Wilkins, with James Brown and Tyler Wilkins also holding shares, and they try to keep their care rooted in local community involvement and family engagement, even though there's no formal family council. Owners and staff remain active in day-to-day life here, and they've built a team that includes a Director of Nursing, Assistant Director, therapists, and specialized care staff, all guided by a multidisciplinary approach to resident needs.

Residents can get help with just about all the basics-bathing, dressing, grooming, toileting, laundry, and walking or wheelchair use-and they offer dining services with regular mealtimes in a central dining room, along with housekeeping, medication support, and scheduled transportation for outings or appointments. Rooms come with comforts like cable TV, washers, dryers, kitchenettes, and safety supports including nurse call devices and sprinkler systems. There are some larger semi-private rooms for those needing a bit more space, a peaceful and homelike feel across the community, and safety features throughout for those who use walkers or wheelchairs.

Wilkins has a strong rehab program, including their Pathway Rehabilitation Center, providing inpatient rehab, IV antibiotic therapy, wound care, speech, occupational, and physical therapy, and mental health services, which all help residents get back on their feet or keep up with their health. Memory care is available too, with programs designed for seniors with Alzheimer's or other dementias, and staff get extra training to support those folks. Amenities help break up the day and include game rooms, activity stations, scheduled crafts and classes, fitness and recreation therapy, barbershop, beauty salon, as well as religious and educational programs, so there are opportunities to socialize or just relax, depending on each person's interests.

The facility takes Medicare and Medicaid and supports veterans with VA-certified care, so different people can find what they need. Inspection reports do mention past deficiencies-issues with safety and respiratory care came up in audits, but no severe (category G or higher) citations have shown up, and over several years, the place's compliance with rules earned them an A+ on facility inspections and a B+ grade overall. You'll find 24-hour emergency support, regular social events, and space for both quiet and activity, and a staff that seems to know most people by their first names, working to keep life steady and safe for everyone.
