    Caring staff, improved memory care

I was hesitant at first, but this facility quickly put me at ease. The staff are amazing-patient, approachable, and genuinely family-like-and they go above and beyond for residents. The new memory care unit, strong activity program (frequent events, karaoke, daily games), and clean, welcoming environment show real improvement under the current administration. They're transparent, responsive to families, and I'd encourage a visit to see for yourself.

    Summary of reviews

Reviews of Oak Hills Care Center present a mixed picture with clear strengths at the caregiving level and recurring operational weaknesses at the facility-administration level. Many families and visitors highlight compassionate interactions from nursing staff and CNAs, describing staff who are attentive, respectful, and engaged with residents. The facility offers skilled nursing and rehabilitation services and several accounts praise the rehabilitation and memory-care programming. Multiple reviews describe a robust activities calendar — frequent groups, themed entertainers, karaoke, and daily games — which some families report contributes to resident engagement and satisfaction.

At the same time, a number of reviews describe persistent administrative and operational issues. Common themes include inconsistent leadership, organizational disorganization, and uneven communication with families and POAs; this includes difficulties reaching staff by phone, slow message follow-up, and transfers or discharges that lacked clear family consultation. Staffing shortages and turnover are frequently cited and are associated in reviews with delays in basic care tasks, inconsistent bathing schedules, long wait times for assistance, and variability in how promptly medications are administered.

Sanitation and facility condition reports are mixed: some reviewers describe clean, welcoming common areas and an active remodeling effort, while others describe cleanliness and odor concerns in specific rooms or units. Dining and nutrition receive similarly mixed feedback — several families report engaging meals and appropriate portioning, while others cite cold or greasy food, inconsistent adherence to dietary restrictions, and the absence of a dedicated dietitian or nutritional oversight. Personal-property management is another recurring concern; reviewers note missing or mishandled belongings, lost dentures, and room reassignments that caused confusion.

Medication management and security are salient issues in the reviews. Several accounts reference delays in administering medications, misdispensed pills, and more serious allegations involving narcotics and theft. Given the severity of those claims, reviewers express concern about pharmacy and controlled-substance controls and about safeguards for residents’ finances and personal items. Some reviewers also note instances of delayed clinical response or inadequate incident follow-up that contributed to family distress.

There are also indications of recent improvement: several families mention positive changes after management turnover, including an engaged activities director, renovated units, a new memory-care area, and administrators who are transparent and willing to involve external advocates. These improvements suggest the facility is capable of addressing certain operational gaps, but the pattern of mixed reviews implies that consistency remains a challenge. Prospective residents and families should weigh the facility’s apparent strengths in direct caregiving and activities against the documented operational risks — especially around communication, staffing consistency, medication controls, property security, and sanitation — and consider an in-person visit and direct conversation with administration and clinical leadership before making placement decisions.

    About Oak Hills Care Center

Oak Hills Care Center Skilled Nursing & Rehabilitation, also known as Oak Hills Living Center, is a licensed nursing home in rural Jones, Oklahoma, about five miles outside Choctaw and a short drive from Midwest City and Oklahoma City. The facility has 160 beds, usually with about three-quarters filled, and offers private suites, shared rooms, and studio layouts, all furnished with private bathrooms, air conditioning, cable TV, Wi-Fi, and telephones. The center has been serving the area since 1959 and sits among old oak trees and scenic land, giving it a quiet, peaceful setting.

This is a skilled nursing and rehabilitation center that takes care of people needing 24-hour medical support, nursing care, or help after hospital stays and surgeries. The nursing staff run 12 to 16-hour shifts, providing round-the-clock supervision, a 24-hour call system, and daily care like bathing, dressing, toileting, transfers, mobility help, and medication management. Teams include registered nurses, licensed practical nurses, certified nurse assistants, physical therapists, occupational therapists, speech therapists, physician assistants, nurse practitioners, social workers, dieticians, feeding assistants, pharmacists, and a medical director. The facility can handle people needing long-term care, post-acute care, dementia support, geri-psych care, behavioral care, and memory care. There's a dedicated memory care unit for people with dementia or those needing a more secure and supportive place.

Rehabilitation is important here, and there's a department for physical, occupational, and speech therapy. Rehab plans are designed to help each resident regain independence and get back on their feet. Restorative therapy, wound care, respiratory therapy, IV antibiotics, lab services, and x-ray services are all available on-site. They also help with diabetes diets, special dietary restrictions, and nutritional counseling, serving meals restaurant-style and preparing meals for all health situations.

Oak Hills Care Center offers daily and weekly activities, both community-sponsored and resident-led, like arts and crafts, games, music therapy, exercise, group counseling, planned day trips, and mental wellness programs. There are several common areas, including a dining room, fitness room, game room, small library, garden, outdoor spaces, beauty salon, computer center, and wellness center. Residents can join family and resident councils to share ideas about the community. The building has strong safety features, including secured units to prevent wandering, and provides both non-medical and medical transportation for outings and doctor appointments.

Support services include housekeeping, linen service, home making, and coordination with outside healthcare providers. Staff help everyone with activities of daily living at all hours. Oak Hills accepts Medicare and Medicaid but is not a Continuing Care Retirement Community. The company is privately owned, a for-profit limited liability company, and employs between 51 and 200 people.

Facility inspection grades have varied over time, with a recent overall B+, but past records show grades as low as F. The average review rating from families and residents stands at 6 out of 10, and the federal government's Centers for Medicare & Medicaid Services gives the nursing home a 1-star rating. Oak Hills Care Center Skilled Nursing & Rehabilitation is a long-standing place that focuses on both short-term and long-term care in a rural area, trying to offer comfort, security, and steady care for those needing skilled nursing or rehabilitation services.
