    Caring home, excellent memory care

I'm grateful for the clean, homey atmosphere and genuinely caring staff who made my aunt feel like family. The memory-care wing felt like a real home, nursing/rehab and hospice were excellent, meals were good, and there are engaging daily activities (Nikki leads many of them). I'm impressed by the attention and comfort here and would recommend it.

    Summary of reviews

The reviews for Sand Springs Nursing & Rehab Center present a polarized picture: there are distinct strengths alongside recurring operational concerns. Positive commentary centers on the caregiving staff โ€” many families describe nurses and CNAs as caring, competent, and attentive, and several accounts highlight supportive hospice services and effective skilled rehabilitation. The memory-care neighborhood is frequently characterized as home-like, and activity programming (including daily engagement led by named staff) is noted as meaningful for residents. Renovation work and a well-maintained entry area contribute to an improved appearance in some parts of the building, and numerous reviewers described a calm, comfortable atmosphere for residents.

At the same time, a pattern of service inconsistencies emerges. Reviewers describe variability in care quality between shifts and units, with specific operational weaknesses that include delays in staff responsiveness to call lights, inconsistent bathing and clothing-assistance routines, and medication-administration delays or missed doses. These issues suggest gaps in day-to-day staffing reliability and clinical workflow controls. Staffing pressures are also evident: frontline workers are described as overworked at times, which reviewers tied to reduced engagement and slower response times.

Dining and therapy services show similar variability. Several accounts note that meals are sometimes late, lukewarm, or of inconsistent quality, while others praise the food. Therapy and rehabilitation are described as strong in some cases but inconsistently delivered in others, indicating uneven scheduling or follow-through. Facility maintenance and housekeeping are mixed as well: while common areas and recent remodels are praised, there are also descriptions of room-level sanitation concerns, odor issues, and pest-control concerns that in a few cases led to room relocations. Accessibility shortcomings in some bathrooms and a lack of regular showering assistance were also raised.

Communication and management are another area of divergence. Some reviewers express appreciation for new management initiatives and improved care under different leadership; others cite poor front-office professionalism, lost belongings, and concerns about communication tone with families. A subset of reviews raises serious allegations regarding misplaced belongings and possible financial misconduct; these are significant claims that would merit verification through facility administration or regulatory inquiry.

For prospective residents and families, the overall impression is mixed: the facility appears capable of providing compassionate, skilled care and meaningful activities, particularly in memory-care and rehab settings, but operational variability has produced meaningful service gaps for some residents. Key areas to investigate during a tour or intake visit include staffing levels by shift, medication-administration processes, bathing and hygiene schedules, meal-service timing and menus, pest-control and housekeeping protocols, and procedures for communicating with families and tracking resident belongings. Asking for recent inspection or complaint resolutions and speaking directly with current families and unit staff can help clarify whether positive changes under new management have addressed prior concerns.

About Sand Springs Nursing & Rehab Center

Sand Springs Nursing & Rehab Center sits in Sand Springs, Oklahoma, and offers nursing care for seniors who need help with daily life, medical care, and therapy. The center accepts Medicare and Medicaid, so many families can look at it for payment options, and as a for-profit limited liability company, it has a business-focused approach. Residents can pick private suites or shared rooms, with private bathrooms, air conditioning, telephones, internet, cable, and wifi. Housekeeping, linen service, and meal preparation happen every day, and staff help with medication, mobility, and keeping up with health appointments, with transportation to doctor visits arranged when needed. The building has protected areas so residents stay safe, and a 24-hour call system lets people get help any time.

The staff includes Registered Nurses, Licensed Practical Nurses, Certified Med Aids, Certified Nurse Aids, speech therapists, and a Medical Director, giving a range of medical and daily care options. Families and residents both have councils for sharing their opinions. The therapists handle physical therapy, post-surgery care, occupational therapy, and speech therapy, and specialists teach people how to swallow or talk again after strokes or heart attacks. Care and therapy go hand in hand here, and detailed chart documentation is checked by clinical consultants, which they call "The Second Mile" focus. There's hospice care, respite care, and inpatient rehabilitation, and long-term care services support people dealing with Alzheimer's, dementia, and other health conditions. The center sticks close to personal care plans, working with residents, families, and doctors, and the staff have nurse and CNA hours posted.

Daily life includes activities like BINGO, live music, church services, and ice cream socials to keep people social, active, and in good spirits, plus common spaces, large living areas, and outdoor spots for relaxing or visiting. The nursing home is clean, inviting, and offers private or shared living arrangements, and families are welcome for visits. Inspection reports show few problems, and none very serious, earning an overall C rating, an A for inspections, and a B-minus for long-term care. Amenities open daily from 10:00 am to 8:00 pm help make the place comfortable. Sand Springs Nursing & Rehab Center focuses on keeping seniors' quality of life strong, giving skilled care and a safe, steady home with therapy, activities, and people who know what they're doing, so families know their loved ones get the support they need, and the staff work to keep things running the right way.
