    Compassionate staff, loved one thrived

I had a very positive experience - the staff showed a genuine "I CARE" attitude, were compassionate, gentle, and went beyond their duties so my loved one felt safe and well cared for. The building and grounds were clean and warm, with an on-site cat and bird, frequent block parties, responsive nurses/CNAs/support staff, helpful office/Medicaid liaison and smooth appointment coordination. My family had real peace of mind; my loved one thrived and we strongly recommend this wonderful, caring team.

    Summary of reviews

Reviews for Heartsworth Nursing and Rehabilitation are mixed, with a clear divide between families who experienced attentive, compassionate care and those who identified serious operational problems. Positive accounts emphasize compassionate, goal-oriented nurses and aides, good hospice coordination, acceptance of VA benefits, and an on-site Medicaid liaison. Many families described a clean campus, active social programming (block parties, holiday events, trick-or-treat), a home-like feel with pets, and helpful administrative and housekeeping staff that supported family involvement and appointment coordination.

At the same time, a recurring set of clinical and operational concerns appears across reviews. The most prominent issues involve inconsistent clinical communication between shifts and gaps in care coordination, which reviewers linked to unanswered family questions and delays in clinically indicated interventions. Medication administration controls are another frequent concern: reviewers described delays, missed doses, or a need for repeated verification of physician orders. These describe systemic vulnerabilities rather than isolated interactions and suggest a need for stronger medication-safety processes and clearer handoff procedures.

Responsiveness to urgent needs and overall staff professionalism were described as variable. Several families praised staff as compassionate and family-oriented, while others described slow responses to medical needs or conduct that made them uncomfortable. One review contained an allegation regarding an inappropriate grooming incident; such reports elevate the importance of reviewing staff training, supervision, and complaint-handling processes. Equipment and supply limitations were also noted (for example, families supplying seating), and there are mentions of an aging physical plant with some areas appearing dated and in need of renovation.

Dining impressions are mixed: some reviewers found meals acceptable and timely, while others described poor food quality and inconsistent service. There were also isolated concerns about billing and fee transparency (rent increases, charges for cold meals). COVID-era challenges were mentioned as context in some reviews.

For prospective residents and families: the facility demonstrates real strengths in hospice partnerships, compassionate caregiving (in many instances), active programming, and campus upkeep. However, the documented patterns around medication administration, inter-shift communication, and variable staff professionalism merit careful inquiry during touring and contracting. Suggested follow-up areas include asking for current staffing ratios, medication-safety procedures and audit results, sample activity calendars, recent facility maintenance plans, and the facilityโ€™s process for escalations and family communication.

    About Heartsworth Nursing and Rehabilitation

Heartsworth Nursing and Rehabilitation sits at 1200 W Canadian Ave in Vinita, Oklahoma, and operates as a for-profit nursing home under Rivers Edge Operations LLC, which owns it fully, with David Ganz, S. Aryeh Retter, Yitzchok Oelbaum, and Avrohom Kravetz as the indirect owners since September 2023, and the management has David Ganz and Yitzchok Oelbaum at the helm nowadays, so folks who need both short-term or long-term care will find 146 certified beds, skilled nursing, physical rehabilitation, occupational therapy, speech therapy, and palliative care, along with clinical services meant to help diagnose diseases, plus a selection of amenities to support resident comfort, so there's always medical staff on hand to help people with different needs day or night, and Heartsworth Center for Nursing & Rehabilitation aims to help adults and seniors with complicated medical needs in a safe, supportive setting, focusing on careful, individualized care plans and a sense of home.

The place tries to enhance residents' quality of life with a broad range of care and therapy options, and they do make a point to handle medication, dietary needs, and specialized services as necessary, yet there've been some concerns, like a nurse turnover rate of 51.9%, which is a little lower than Oklahoma's state average of 56.5% but still significant, though nurse hours are reported at 3.89 per resident each day, just above the state's average of 3.8, which might give families a bit of insight into staffing.

Inspections over time highlight a few problems worth knowing, including 13 deficiencies found during the standard inspection on May 2, 2024, and a total of 33 deficiencies across all inspection reports, with issues noted in how the facility assesses residents and develops care plans, such as not always completing them within the proper timeframe or making sure a whole team of health professionals reviews and updates them, plus shortcomings in administration-including failing to let residents or their representatives know about arbitration agreements and the option to refuse-and a nutrition area where staff failed to buy approved food or store and serve it according to required standards, to go along with at least one recorded instance of an infection-related deficiency tied to stopping the spread of infections.

The facility is classified as a nursing home, offers group homes, and specializes in both nursing and rehabilitative care, and while they say they're committed to high standards, well-trained staff, and a welcoming environment with modern amenities and a focus on supporting diverse healthcare needs for seniors and adults, those inspection results and operational facts give a fuller, clearer picture for anyone looking at Heartsworth Nursing and Rehabilitation for themselves or their loved one, so if someone needs a range of clinical, therapy, and nursing care close to home in Vinita, Oklahoma, it's a place with sizable services, but families should look at all the details before making a decision.
